[Various factors influencing the local anesthetic effect of articaine].

In a field study, the reduction in the anaesthetic effectiveness of Articain was investigated in relation to various influencing parameters. Both in the ZMK-Klinik (Dental Hospital) of Würzburg as well an in various independent practices, the local anaesthetic Articain was given to over 2000 patients in the course of dental treatment. With 12.1% of all test-subjects, inspite of the initial positive effectiveness upon application of the nerve-blocking anaesthetic, touch sensitivity and/or painfulness was established in the area concerned. Statistical evaluations of the recorded data of the test-subjects investigated showed a significantly higher quota of anaesthetic failure in the case of heavy cigarette consumption, chronic exposure to inhaled toxins as well as in the case of the intake of medicines of certain pharmaceutical types. Apart from that, a connection was also established between reduction in the effectiveness of the local anaesthetic and various treatments as well as the way in which the local anaesthetic was applied and the site of injection. Possible causes will be presented and discussed in relation to information from the literature on the subject.

[Experience in mandibular fracture treatment with osteosynthesis during the growth period].

The experiences in the treatment of 19 fractures of the mandibular body in 14 children and 30 similar fractures in 23 adolescents are reported. The average hospitalization of the conventionally treated children lasted 19.5 days compared with 14.8 days in the osteosynthesized group, which originally suffered from heavier fracture dislocations. Intermaxillary fixation (IMf) was maintained for 23 days on an average in the conventionally treated and 14 days in the osteosynthesized children, where it was necessary only in 4 out of 7 cases. In 2 children an injury of dental roots of altogether three teeth had to be diagnosed by x-ray, probably caused by 7-mm-screws used earlier. In order to avoid injury of germs especially in the region of the canine, we are now using screws of a maximum length of 5 mm for miniplate osteosynthesis.

[The efficacy of paracetamol (Tylenol) and acetyl salicylic acid (Aspirin) in treating postoperative pain].

In a randomized, double-center, double-blind, parallel group study the analgesic effect of a single dose of paracetamol (1000 mg) and acetyl salicylic acid (1000 mg) was compared with placebo in patients with moderate to severe postoperative pain following the surgical removal of a wisdom tooth. The most important finding was the statistically significantly shorter period until the onset of the action of paracetamol as against acetyl salicylic acid.

Functional and anatomic aspects of the orbitotomy in craniofacial surgery.

The orbitotomy has a central importance in craniofacial surgery. It is indicated for premature craniosynostoses, malformations and tumor surgery. The aim of the treatment is to correct functional disturbances and aesthetic impairments. Anatomical structures must be respected. Functional and anatomical aspects of the orbitotomy in craniofacial surgery are described.
