Cranial ultrasound assessment of gestational age in low birthweight infants.

The anatomical features of the cerebral hemispheres of 97 infants were studied by ultrasonography. A scoring system was devised based on anatomical and ultrasound appearances and compared with gestational age calculated from the date of the mother's last period. There was a significant positive correlation between gestational age and ultrasound score. Cranial ultrasonography is an accurate and reproducible method of assessing gestational age in low birthweight infants.

Low plasma triiodothyronine concentrations and outcome in preterm infants.

A major association has been found between low plasma triiodothyronine concentrations in preterm neonates and their later developmental outcome. Plasma triiodothyronine concentration was measured longitudinally in 280 preterm infants below 1850 g birth weight. Babies whose lowest recorded concentration was less than 0.3 nmol/l had, at 18 months' corrected age, 8.3 and 7.4 point disadvantages in Bayley mental and motor scales and a 8.6 point disadvantage on the academic scale of Developmental Profile II, even after adjusting for a range of antenatal and neonatal factors known to influence later development. Low concentrations of triiodothyronine were strongly associated with infant mortality, but not after adjusting for the presence of respiratory illness. There was no association between plasma triiodothyronine concentrations and somatic growth up to 18 months, and no association with necrotising enterocolitis or later cerebral palsy. Data on postnatal changes in plasma triiodothyronine concentrations are presented for reference purposes. While cited reference ranges for plasma triiodothyronine concentration appear suitable for well infants above 1500 g birth weight, smaller or ill babies often have very low values for many weeks. Our data are relevant to the debate on endocrine 'replacement' treatment in premature babies.

The effect of iron deficiency on early oral carcinogenesis in the rat.

In order to further investigate the known influence of iron deficiency on 4NQO oral carcinogenesis in the rat, groups of iron-deficient and iron-sufficient Charles River white rats were painted with carcinogen for 8 weeks or 14 weeks and then left untreated for 32 weeks or 26 weeks respectively. Tumour development and epithelial dysplasia were assessed at the time of killing. Animals painted for 14 weeks showed more severe dysplasia than those painted for 8 weeks, but no significant differences were noted between corresponding iron-deficient and iron-sufficient groups.

Antinuclear antibody-negative systemic lupus erythematosus-how common?

A review of five years' DNA-binding antibody results in a routine service laboratory revealed 38 patients who had high DNA-binding capacity (DNA-bc) but no antinuclear antibodies (ANA). On retrospective case note analysis, 22 patients (58%) were thought to have systemic lupus erythematosus (SLE), although only six (16%) fulfilled the preliminary classification criteria of the American Rheumatism Association (ARA). Our findings indicate that ANA-negative SLE is commoner than generally realised and lead us to recommended the measurement of DNA-bc in every case where clinically appropriate.

Teaching surgical skills in Ethiopia.

An innovative process of training non-medically qualified staff to perform emergency surgery is described. The benefits to the rural communities are balanced against the potential threat to the surgical establishment in Ethiopia.
