Recurrence of lattice corneal dystrophy type 1 in the corneal grafts of two siblings.

Amyloid was identified by light and electron microscopy within corneal grafts of two male siblings from a family with lattice corneal dystrophy type 1. These deposits indicate a recurrent disease within the donor tissue, and we believe this reflects an infiltration of the grafts by genetically abnormal host corneal fibroblasts. One of the patients developed bilateral recurrent disease in the grafts eight and 13 years after penetrating keratoplasty. His brother required a regraft 16 years after a penetrating keratoplasty. Although both of these patients required regrafts because of impaired visual acuity, the decrease in visual acuity in one case was not solely the result of reaccumulation of amyloid, but was at least in part caused by a plaque of fibrous tissue behind the cornea.

Defective natural cytotoxicity in patients with cancer: normal number of effector cells but decreased recycling capacity in patients with advanced disease.

Patients with advanced cancer exhibited lower natural cytotoxicity against K562 target cells in a standard short-term chromium release assay than did patients with localized malignancy or normal individuals. Although natural killer (NK) activity of advanced cancer patients could be augmented by nylon column depletion of adherent cells and enriched further by fractionation of discontinuous Percoll gradients, the NK level attained remained below that obtained from PBL of normal donors treated by the same procedures. The pattern of NK activity obtained on the Percoll gradients was the same for all individuals studied. The proportion of large granular lymphocytes (LGL) paralleled NK activity and was highest in the peak NK fraction in all individuals tested. Advanced cancer patients with low NK activity showed no decrease in number of LGL from that of other cancer patients or normals. Similarly, there was no decrease in target binding cell number. Using a single-cell assay in agarose, we found that the number of active NK cells was the same for all patients, whether with localized or advanced malignancy, and normal subjects. In a 3-hr time period, there were no differences in the rate of killing in agarose. The maximum killing potential (Vmax) of the NK-deficient advanced cancer patients in a 51Cr-release assay was significantly lower than that of normals or other cancer patients. The observed defect in natural cytotoxicity of these patients thus appeared to be due to a reduced recycling capacity.

Evidence for a "mute" catalytic subunit of cyclic AMP-dependent protein kinase from rat muscle and its mode of activation.

An isoenzyme of the catalytic subunit of type II cyclic AMP-dependent protein kinase from rat muscle is reported which coelutes with the classical catalytic subunit but differs from it in isoelectric point (pI 8.7 vs pI 9.1) and is enzymmatically inactive. After reaction with a heat- and acid-stable component of the protein kinase modulator fraction from the same tissue, the "mute" isoenzyme displays a high activity when assayed on isoelectric focusing gels. This activation process does not occur through proteolytic degradation and is not characteristic of a turnover-type reaction. The data imply direct interaction between the isoenzyme and a modulating protein which may subsequently be separated from the enzyme without reversal of the activation. The modulator protein thus appears to act as a template, inducing a conformational change. The implications of such a mute isoenzyme and its control through small modulator proteins are discussed.

Circular dichroic evidence for a conformational change in a cytochrome b--c1 complex by uncoupling agents.

Circular dichroic spectra of the cytochrome b--c1 complex exhibit bilobe formation typical of exciton splitting in the presence of uncoupler. Bilobe formation occurs if both cytochrome c1 and cytochrome b are fully reduced. The fully oxidized and ascorbate-reduced complexes are not altered dichroically by uncouplers. The exciton splitting induced by uncoupler is consistent with heme--heme interaction: specifically, interaction between the two cytochromes b in the complex.

A prospective study of the rehabilitation of the above-knee amputee with rigid dressing. Comparison of immediate and delayed ambulation and the role of physical therapists and prosthetists.

Twenty-four above-knee amputees (AKA) treated with rigid plaster dressings were randomized to receive ambulation on pylon within 24 hours of surgery or at time of suture removal. Casts were applied by physical therapists in half of each group, and by a prosthesist in the other half. There were no statistically significant differences among the groups with respect to age, sex, stump length, gait characteristics and wound healing. Time to prescription of final prosthetis was similar in all groups. Specifically, there was no delay in the group ambulated immediately. The immediate ambulators did have significantly greater stump pain requiring more analgesia than the group ambulated after suture removal. Patients with case applied by physical therapists used their prostheses more than patients with cases applied by prosthetists. There was no detectable difference among treatment groups with regard to participation in therapy, acceptance of prosthesis, and psychological status. Recommendation for management of AKA's include: use of rigid dressing; ambulation on pylon after suture removal; utilization of physical therapist for application of rigid dressings and alignment of pylon.

Circular-dichroism studies of the cytochrome b-c1 complex of Saccharomyces cerevisiae.

1. Circular dichroism studies on the Soret region of the cytochrome b-c1 complex of yeast reveal a change in the dichroism of cytochrome c1 depending on the redox state of cytochrome b, indicating a conformational interaction between both cytochromes. 2. This interaction is not influenced by binding of the inhibitor antimycin A to the complex, so that the interaction does not appear to be involved in the mechanism of electron transport through the complex. 3. Antimycin A binding causes a complex set of changes in the CD spectrum of the complex, which can be attributed to a severe and specific distortion of the environment of the chromophore of cytochrome b.

Structure of a cytochrome b-c 1 complex from Saccharomyces cerevisiae YF.

1) An isolation and purification procedure is reported for an active cytochrome b-c1 complex from Saccharomyces cerevisiae. The complex acts as an antimycin A-sensitive duroquinone-cytochrome c reductase and contains cytochromes b and c1 at a concentration of 8 nmol/mg protein and non-heme iron at a concentration of 15 nmol/mg protein. 2) Difference spectra at room temperature and at 70 degrees K show that the preparation is free from contamination with cytochromes c or aa3. Assays of enzyme activity indicate the absence of any of the other catalytic functions normally associated with the mitochondrial respiratory chain. 3) On dissociation and separation on sodium dodecylsulfate-polyacrylamide gels the complex gives rise to seven bands corresponding to subunit polypeptide molecular weights of 43 000, 40 000, 32 000, 24 000, 22 000, 20 000 and 18 000. These appear in a regular stoichiometry of 1:1:3:1:1:1:1.

Tissue culture of human breast tumours.

Several media have been used in an attempt to assess the most appropriate combination of ingredients to effect good cell growth from human breast tumours. Good epithelial cell growth has been obtained in 60 out of a group of 160 tumours successfully cultured in four types of basic media. The other tumours were fibroblastic in nature or contained a mixture of epithelial and fibroblastic cells.

Tyrosine-induced eye and skin lesions. A treatable genetic disease.

Tyrosine-induced eye and skin lesions in man are an autosomal, recessive, inherited syndrome associated with tyrosinemia, tyrosinuria, and increased urinary excretion of tyrosine metabolites. Patients have mild to severe keratitis and erosive and hyperkeratotic lesions on the palms and soles. The degree of involvement was variable in the small number of patients studied. Mental retardation is frequently a part of the syndrome. A low-tyrosine low-phenylalanine diet lowers blood tyrosine level and leads to healing of the skin and eye lesions. Early dietary treatment may prevent mental retardation.

A test experience with a machine-processed electrocardiography diagnosis: the recognition of "normal" and some specific patterns.

Five hundred ECG's, machine processed by the commercially available IBM Bonner-Mt. Sinai System, have been reviewed. The diagnostic printouts were of value as an adjunct to the physician's interpretations, but were not sufficiently reliable--in regard to either sensitivity (identification of the abnormal or borderline) or specificity (accuracy in predicting typical abnormalities)--to allow us to condone any claim for it being an independent reliable diagnostic service to noncardiologist clinicians. Despite this desclaimer for current diagnostic reliability of ECG computer analyses, specifically the IBM-Bonner system, we continue to be of the firm conviction that progress through this method of ECG analysis will have much to contribute in future years.

Multiple personality. A single case study with a 15 year follow-up.

A review of the literature relating to multiple personality is presented together with a study of a single case of hysterical aetiology that demonstrates the development of multiple personality from a fugue amnesic state. A re-examination of this case after 15 years without significant psychotherapeutic intervention demonstrates a tendency towards remission. The view that multiple personality is a form of fugue, not necessarily hysterical, in which an alternative personality is adopted and that this behaviour is reinforced by the attention that it receives, is discussed.
