[Pseudo-alcoholic hepatitis and cirrhosis caused by amiodarone (Cordarone)].

Amiodarone is an antiarrhythmic drug widely used to treat a variety of supraventricular and ventricular arrhythmias. However its drawback is a very slow elimination and very frequent adverse effects (thyroid, pulmonary, neurologic, ocular, dermatologic, hepatic disorders). We describe a patient who developed a pseudoalcoholic liver disease and a cirrhosis after use of Amiodarone for a long period of time.

[Peritoneal carcinomatosis].

Searching for the primitive tumor of a peritoneal carcinomatosis (PC) is no easy task even considering the multiple modern diagnostic procedures at our disposal. Without the primary site of a neoplasm it is often difficult to establish the prognosis and a treatment regimen. In PC the most frequent primitive tumors are ovarian neoplasms in women and gastrointestinal neoplasms in men. We present a case report of a 70-year-old Caucasian woman who was referred to our department for diagnosis and management of a PC. A fine-catheter aspiration cytology of the peritoneal implants was performed and provided histologic evidence of an ovarian papillary type carcinoma. This case is suggestive of what has been called in recent literature extraovarian peritoneal serous papillary carcinoma. Absent or insignificant histology of the ovaries of primary in-situ ovarian carcinoma is one diagnostic criterion. This patient had no evidence of ovarian disease considering ultrasound imaging and computer tomography. Unfortunately, since both laparotomy and laparoscopy were contraindicated, histology of the ovaries is lacking.

Potassium transport by amino acid permeases in Saccharomyces cerevisiae.

Deletion of the potassium transporter genes TRK1 and TRK2 impairs potassium uptake in Saccharomyces cerevisiae, resulting in a greatly increased requirement for the ion and the inability to grow on low pH medium. Selection for mutations that restored growth of trk1Delta trk2Delta cells on low pH (3.0) medium led to the isolation of a dominant suppressor that also partially suppressed the increased K+ requirement of these cells. Molecular analysis revealed the suppressor to be an allele of BAP2 that encodes a permease for branched chain amino acids. The suppressor mutation (BAP2-1) converts a phenylalanine codon, highly conserved among the amino acid permease genes, to a serine codon in a region predicted to lie within the sixth membrane-spanning domain. Generation of the analogous mutation in the histidine permease produced an allele, HIP1-293, that similarly suppressed the low pH sensitivity of trk1Delta trk2Delta cells. Suppression of trk1Delta trk2Delta phenotypes by BAP2-1 or HIP1-293 was correlated with increased Rb+ uptake. The presence of the substrate amino acids enhanced but was not essential for suppression of trk1Delta trk2Delta phenotypes and increased Rb+ uptake. The conserved site altered by the suppressor mutations appears to be important; his4 HIP1-293 cells show an increased requirement for histidine compared with his4 HIP1 cells.

Clinical, endocrine, and metabolic effects of two doses of gestrinone in treatment of pelvic endometriosis.

OBJECTIVE: Our purpose was to determine and compare the efficacy and hormonal and metabolic effects of 1.25 mg with 2.5 mg of gestrinone given twice a week in the treatment of mild and moderate pelvic endometriosis. STUDY DESIGN: A phase II, prospective, randomized, double-blind study involving 11 patients given gestrinone 1.25 mg (five patients) or 2.5 mg (six patients) orally twice a week for 24 weeks was performed. Revised American Fertility Society scores were determined by laparoscopy before and at the end of treatment. Serum hormone (free thyroxine, free testosterone, estradiol, progesterone, follicle-stimulating hormone, luteinizing hormone), sex hormone binding globulin, and lipid concentrations were measured before, throughout, and for 6 months after treatment. Quantitated computerized tomography of thoracic 12 through lumbar 4 vertebral bodies were determined before, at the end of, and 6 months after treatment. RESULTS: Gestrinone 2.5 mg significantly reduced the endometriosis implant score from 10.3 +/- 2.8 to 3.8 +/- 0.8 (p = 0.05). Both doses significantly reduced serum progesterone and sex hormone binding globulin levels. Estradiol, free testosterone, free thyroxine, follicle-stimulating hormone, and luteinizing hormone levels were not significantly affected. Spinal bone increased significantly by 7.1% with 2.5 mg but lost significantly by 7.1% with 1.25 mg gestrinone; these changes had not reversed completely 6 months after stopping treatment. CONCLUSIONS: In mild to moderate pelvic endometriosis 2.5 mg of gestrinone twice a week was more effective and had a more positive effect on bone mass than did 1.25 mg of gestrinone.

Effect of spatial ability and sex on inter- and intrahemispheric correlation of EEG activity.

Inter- and intrahemispheric correlation of the EEG activity at rest was computed in two groups of men and women, between 17 and 21 years old, with extreme degrees of spatial ability (SA) evaluated by the Spatial Relations Subtest of the Differential Aptitudes Test (DAT). Interhemispheric (INTERr) and intrahemispheric (INTRAr) EEG correlations were computed by means of Pearson product-moment coefficients for 5 EEG bands after digitally filtering with an FFT. Women showed significantly higher INTERr of alpha 1 between left and right centrals, lower INTRAr between right frontal and right central regions and lower INTRAr within the left than in the right hemisphere. High SA subjects showed lower INTERr between left and right frontal derivations and higher INTRAr between frontal and parietal and between central and parietal regions of both hemispheres. Sex interacted with SA in INTRAr of alpha between right frontal and right temporal regions with high SA women showing lower INTRAr than low SA women and than men. The present results indicate a different inter- and intrahemispheric functional organization in men and women and in subjects with high and low spatial ability.

Physiological basis for the high salt tolerance of Debaryomyces hansenii.

The effects of KCl, NaCl, and LiCl on the growth of Debaryomyces hansenii, usually considered a halotolerant yeast, and Saccharomyces cerevisiae were compared. KCl and NaCl had similar effects on D. hansenii, indicating that NaCl created only osmotic stress, while LiCl had a specific inhibitory effect, although relatively weaker than in S. cerevisiae. In media with low K+, Na+ was able to substitute for K+, restoring the specific growth rate and the final biomass of the culture. The intracellular concentration of Na+ reached values up to 800 mM, suggesting that metabolism is not affected by rather high concentrations of salt. The ability of D. hansenii to extrude Na+ and Li+ was similar to that described for S. cerevisiae, suggesting that this mechanism is not responsible for the increased halotolerance. Also, the kinetic parameters of Rb+ uptake in D. hansenii (Vmax, 4.2 nmol mg [dry weight]-1 min-1; K(m), 7.4 mM) indicate that the transport system was not more efficient than in S. cerevisiae. Sodium (50 mM) activated the transport of Rb+ by increasing the affinity for the substrate in D. hansenii, while the effect was opposite in S. cerevisiae. Lithium inhibited Rb+ uptake in D. hansenii. We propose that the metabolism of D. hansenii is less sensitive to intracellular Na+ than is that of S. cerevisiae, that Na+ substitutes for K+ when K+ is scarce, and that the transport of K+ is favored by the presence of Na+. In low K+ environments, D. hansenii behaved as a halophilic yeast.

Extradural dermoid tumours of the posterior fossa.

Dermoid tumours in children usually occur in two locations: at the anterior fontanelle and on the occipital squama. An exceptional site of origin for a posterior fossa dermoid cyst is the extradural space. There are only six previous cases of this situation reported in the literature. A series of 103 subscalp and calvarial masses in children were reviewed and three children are reported with extradural dermoids of the posterior fossa, which communicated with the skin through midline occipital dermal sinuses. All three children were seen after the rapid growth or the formation of an abscess in a previously noted occipital subcutaneous mass present since birth. Although computed tomography or magnetic resonance imaging showed the dermal sinus and the intracranial tumour, these studies were unable to ascertain the intradural or extradural nature of the tumours, their exact origin only being established at operation. Histopathological study showed preclinical signs of infection in the two patients that had not yet formed an abscess. It is suggested that early neurosurgical treatment of these neoplasms should be done to prevent the development of severe intracranial infection. The previously reported simplicity of surgical removal of occipital extradural dermoids was not confirmed in this series.

The capacity to transport potassium influences sodium tolerance in Saccharomyces cerevisiae.

The capacity to transport potassium and to discriminate between the different alkali cations has been found to affect sodium tolerance in Saccharomyces cerevisiae. Mutants with a defective capacity to transport K+ were more sensitive to high concentrations of Na+ because they accumulated more Na+ and less K+ than wild-type cells which showed high discrimination between K+ and Na+.

Inter and intrahemispheric EEG correlation as a function of sleep cycles.

1. Inter (INTERr) and intrahemispheric (INTRAr) EEG correlation were assessed in 8 young male adults during wakefulness (W) with eyes closed before going to sleep, and during stage 2 (S2), stage 4 (S4) and paradoxical sleep (PS) of the first three sleep cycles during the second night spent at the laboratory. 2. Pearson product-moment correlation were calculated between EEG signals of each pair of electrodes (C3, C4, F3, F4, T3, T4) for every 0.5 Hz from 1.5 to 15 Hz. 3. INTERr and INTRAr of 1.5-6.5 and 11-15 Hz were significantly higher during stage 2 and 4; INTERr of 1.5-6.5 Hz was also higher during PS in cycle 1 and 2, whereas INTERr and INTRAr of 7-10.5 Hz were lower than during wakefulness. 4. INTRAr of S2 and S4 approximated, whereas INTRAr oF PS moved away from W over successive sleep cycles. 5. These data show that cortical changes during sleep are also observed in functional differentiation between cortical sites. Inter and intrahemispheric differentiation is attenuated during stage 2 and 4 while during PS only interhemispheric differentiation is attenuated but intrahemispheric differentiation is accentuated compared to wakefulness. This pattern of cortical differentiation may be of relevance for the understanding of mental activity changes during sleep.

The RGD-dependent and the Hep II binding domains of fibronectin govern the adhesive behaviors of amphibian embryonic cells.

In early amphibian development, interactions between fibronectin and both ectoderm and mesoderm cells are critical in the progression of gastrulation movements. In the Pleurodeles waltl embryo, it has been established that ectoderm cells of the animal hemisphere organize a fibrillar-extracellular matrix containing fibronectin. Mesoderm cells migrate along the blastocoel roof using these fibronectin fibrils as substratum. Fibronectin is an adhesive glycoprotein which possesses multiple cell-binding domains. From previous studies, it is clear that amphibian ectoderm and mesoderm cells interact with fibronectin in an RGD-dependent manner, whereas the contributions of RGD-independent domains in the adhesive behaviors of gastrula cells has not been defined. To study this question, we have used bacterially expressed Pleurodeles waltl fibronectin-fusion proteins. The approach consisted of in vitro adhesion assays with either isolated cells or tissue fragments of embryos dissected at the onset of gastrulation. Tissues were obtained from regions of the embryo which represent presumptive ectoderm cells or from the dorsal-marginal zone which contains cells of the presumptive cephalic, chordal and somitic mesoderm. The results show that both the RGD-dependent and the Hep II domains of fibronectin mediate attachment and spreading of isolated cells. Both regions cooperate to control the proper expansion of a sheet of dorsal mesoderm cells. The Hep II domain promotes the migration of cells ahead of the mesoderm-cell sheet.

Prevalence and characteristics of Escherichia coli serotype O157:H7 and other verotoxin-producing E. coli in healthy cattle.

From February to July of 1994, 328 faecal samples from 32 herds were collected and verotoxin-producing Escherichia coli (VTEC) found on 84% of the farms. The proportion of animals infected varied from 0-63%. VTEC were recovered from 52 (20%) of 257 cows and from 16 (23%) of 71 calves. Although the VTEC belonged to 25 different serogroups, 7 (O8, O20, O22, O77, O113, O126 and O162) accounted for 46% of strains. Nearly 45% of the strains. Nearly 45% of the 83 bovine VTEC strains belonged to serogroups associated with haemorrhagic colitis and haemolytic uraemic syndrome in humans. However, only 2 (2%) of 83 VTEC strains isolated from cattle belonged to enterohaemorrhagic E. coli (EHEC) serotypes (O26:H11 and O157:H7), and only 8 (10%) were positive for the attaching and effacing E. coli (eae) gene sequence. Polymerase chain reaction (PCR) showed that 17 (20%) of VTEC strains carried VT1 genes, 43 (52%) possessed VT2 genes, and 23 (28%) carried both VT1 and VT2 genes. Characterization of VTEC isolates revelated a heterogeneous population in terms of serogroup and toxin type in the positive herds. This study confirms that healthy cattle are a reservoir of VTEC, but, the absence of eae genes in most bovine VTEC strains suggests that they may be less virulent for humans than eae-positive EHEC.

Time course of reaction time and EEG while performing a vigilance task during total sleep deprivation.

Nine young adult male (23-30 years old) paid volunteers were subjected to total sleep deprivation (TSD), after two consecutive nights in the laboratory, for 40 hours (from 0800 hours on the first day to 2400 hours on the following day). Oral temperature (OT), reaction time (RT) in a visual vigilance task, and electroencephalogram (EEG; C3, C4, T3, and T4) while performing the task were recorded every 2 hours during TSD and after recovery sleep. One second of EEG, before target and non-target stimuli for every subject and condition was visually inspected, and artifact-free epochs were Fourier transformed. Absolute power (AP) was calculated for 4-20 Hz (full band) and for theta, alpha 1, alpha 2, and beta 1. Analyses of variance (ANOVAs), with TSD and time-of-day as factors, showed the following significant results. TSD induced an increase in RT and AP of the full band at C3 and C4, of all bands at C3, of theta at T3, and of beta 1 at T4 (p < 0.009 for all comparisons). No time-of-day effects nor interactions were found. OT was not affected by TSD. All variables returned to baseline values after recovery sleep. RT and EEG power showed a linear increase with accumulating hours of wakefulness. The increment in RT also correlated with the increase in EEG power. The results demonstrate that the increment in RT is associated with the increase in AP, particularly in the left central cortex; that the EEG may be used to identify sleepiness; and that EEG during task performance is more sensitive to TSD than during relaxed wakefulness.

Renal-hepatic-pancreatic dysplasia: an autosomal recessive malformation.

We report two brothers with a cystic malformation of the kidneys, liver, and pancreas. In both cases the malformation was fatal and the children died shortly after birth. The pathological findings, consisting of multicystic dysplastic kidneys, dilated and dysgenetic bile ducts, dilated pancreatic ducts, and polysplenia, correspond to those reported by Ivemark as renal-hepatic-pancreatic dysplasia. Many polymalformation syndromes include cystic affectation of these three organs, so this syndrome could be an isolated entity or a final common pathway of response of these organs to a variety of developmental disturbances, which could also include splenic abnormalities. We propose an autosomal recessive pattern of inheritance for renal-hepatic-pancreatic dysplasia.
