[Epidemiology of mental disorders in Spain: methods and participation in the ESEMeD-Spain project].

INTRODUCTION: The ESEMeD-Spain study is part of a European project on the epidemiology of mental disorders, which is also part of a World Health Organization initiative called World Mental Health Surveys. The main objectives of the study are to estimate the prevalence of mental disorders in Spain; to analyze their association with sociodemographic variables and predisposing and protective factors; to evaluate their impact on the quality of life; and to describe and evaluate the health care received by people with mental disorders. METHODS: A representative sample of the noninstitutionalized Spanish population older than 18 years is interviewed. Sample size is 5,500 people. The instrument used in the interviews is the Composite International Diagnostic Interview (CIDI), a questionnaire designed to be used by lay interviewers that allows the diagnosis of psychiatric disorders. Strict quality control of field work has been established. Furthermore, approximately 200 subjects are re-interviewed by a psychiatrist using the Structured Clinical Interview for DSM-IV (SCID) and several questionnaires to measure clinical severity. RESULTS: Field work started in September 2001 and finished in November 2002. Up to Ap ril 2002, more than 4,300 people have been interviewed. Preliminary response rate has been 68.7%. Comparison of the age and gender of the study sample with the general Spanish population shows high representativeness. CONCLUSIONS: The ESEMeD-Spain project will produce un - precedented scientific information about the epidemiology of mental disorders in Spain.

[Renal actinomycosis: presentation of a case].

Renal actinomycosis is rarely suspected and is difficult to diagnose, limited to a few cases in any series. A 56-year-old man with renal actinomycosis and Rendu-Osler disease is described. Long-term therapy with penicillin resulted in a clinical cures. A case report is presented.

[Effect of in vitro infection with dengue virus (DEN-2) on various cellular immune response functions in the mouse].

When Dengue type 2 virus (DEN-2) is put in contact with spleen cells from DBA/2 mice that had been stimulated with Concanavalin A, it was found a decrease in the incorporation of (3H) Thymidine. Furthermore it was observed that the number of Antibody forming cells (Plaque forming cells) against SRBC was decreased, when lymphocytes from DBA/2 mice spleen in culture, had been stimulated with sheep red blood cells (SRBC) in vitro and then infected with DEN-2 virus and the interleukin-1 (IL-1) biosynthesis was quantified in the thymocyte system it was shown that macrophages produced high levels of IL-1 compared with non-infected cells, and that this increased levels could be similar to that produced when macrophages are stimulated with lipopolysaccharide form E. Coli (LPS). The above mentioned results suggest that DEN-2 virus is able of altering some functions of the immune response concerning T and B lymphocytes. Furthermore, the infection of P388D1 cells induce in the first 24 hours an over production of IL-1 that could be the reason why in the natural infection in humans, patients run a fever in the beginning of the viremia caused by DEN-2 virus related with the property of IL-1 reported as endogenous pyrogen.

[Leiomyoma of the bladder as a cause of unilateral ureteral obstruction associated with uterine leiomyoma and polycystic ovary].

A case of a female patient with bladder leiomyoma associated to uterus containing several leiomyomatous nodules and polycystic ovaries is presented. The diagnostic convenience that ecography can provide, showing neoformations and their definition with regard to neighbouring organic structures, is highlighted. The existing relationship between uterine leiomyomas and estrogens is outlined, as well as the action of GnRH agonists on such leiomyomas after long-term therapy.

[Gynecomastia. Apropos of 122 cases].

Pathological data from 122 cases of gynecomastia are studied. According to the importance of epithelial hyperplasia and of specialised stroma, 3 types are described. Every type is compared with the age of patients, the duration of the illness and occasional etiologic circumstances. It is concluded that these 3 types are not evolutive stages, that type II is the most frequent at any age and that there is no relation between etiology and histological type, except between oestrogenic therapy and type I. The alterations of connective tissue, especially breakdown of elastic fibers, are important.

[Autistic syndrome associated with refractory temporal epilepsy].

Autism is a psychopathological disorder of childhood characterized by severe lapses in behaviour and cognition and excessive presence of extravagant unsuitable behaviour that affects a child's development. It is associated with different conditions of the Central Nervous System, especially a high incidence of epilepsy. We report a case of autistic syndrome with refractory temporal epilepsy, describing its clinical onset, (type of seizures, focal anomalies on the EEG, evidence of relation between limbic disfunction and autism.

Drug-related Henoch-Schönlein Purpura.

We report the case of a patient with Henoch-Schönlein Purpura related to a treatment with cefuroxime and diclofenac who presented important systemic manifestations including a glomerulonephritis with IgA mesangial deposits. Skin testing with beta lactam antibiotics and diclofenac were negative in immediate and late reaction as well as RAST test to penicillins G and V. No cautious administration of drugs was done because of the illness severity. Although a reaction to diclofenac could not be excluded we thought that the more probably implicated drug was cefuroxime because the patient referred a purpuric rash after the intake of cephradine for a mastitis, ten years ago.
