Use of computer drug-drug interaction program for detection of adverse drug reactions and related therapeutic problems.

Pharmacies currently using computers to detect drug-drug interactions may not be fully utilizing the computers' programs. Potential drug-disease interactions and some potential adverse drug reactions not traditionally defined as interactions can be detected by extending the basic concept of cross-referencing. For example, certain clinically important ADRs are readily detected by cross-referencing drugs which are often used to treat the results of specific ADRs with certain causative agents. The potential therapeutic implications of such a system when restricted to clinically significant ADRs can be readily appreciated, especially in situations where the pharmacist's time in patient-care areas is limited. This concept is currently being practiced at Bayfront Medical Center, and has added a new dimension to the clinical services provided by the pharmacy. It seems likely that detection of therapeutic situations in this manner may be appropriate in other hospital settings.

Metabolism of the dyestuff intermediate 2,4-diaminoanisole in the rat.

1. 2,4-Diamino[ring-U-14C]anisole.2HCl administered intraperitoneally to rats is excreted chiefly via the urine (79 and 85% of the dose in 24 and 48 h, respectively). The isotope in the faeces was 2.1 and 8.9% of the dose at 24 and 48 h. 2. The major metabolic pathway was acetylation of the amine groups(s), resulting in 4-acetylamino-2-aminoanisole and 2,4-diacetylaminoanisole. 3. Oxidate pathways yielded 2,4-diacetylaminophenol (O-demethylation), 5-hydroxy-2,4-diacetylaminoanisole (ring hydroxylation), and 2-methoxy-5-(glycol-amido)acetanilide or its isomer (omega-oxidation). 4. These major metabolites were excreted in the urine both as free and glucuronic acid conjugates.

Comparison of the metabolism of 2,4-toluenediamine in rats and mice.

The excretion, distribution, and metabolism of 2,4-toluenediamine (TDA) have been compared in rats and mice. The elimination of TDA metabolites into urine was faster and more complete in mice than in rats. However, the feces of rats accounted for a greater percentage of the dose in rats than in mice. The distribution of metabolites in tissues were considerably lower in mice than in rats. The major urinary metabolites observed in the rat were 4-acetylamino-2-aminotoluene, 2,4-diacetylaminotoluene, 4-acetylamino-2-aminobenzoic acid. The major metabolites in the mice were 4-acetylamino-2-aminobenzoic acid, 4-acetylamino-2-aminotoluene and 2,4-diacetylaminobenzoic acid.

The nucleotide sequence of oocyte 5S DNA in Xenopus laevis. II. The GC-rich region.

The primary sequence of the GC-rich half of the repeating unit in X. laevis 5S DNA has been determined in both a single plasmid-cloned repeating unit and in the total population of repeatig units. The GC-rich half of the repeating unit contains a single long duplication of 174 nucleotides. The duplicated segment commences 73 nucleotides preceding the 5' end of the gene and terminates at nucleotide 101 of the gene. The duplicated portion of the gene, termed the pseudogene, differs by 10 nucleotides from the corresponding portion of the gene, and the remaining duplicated sequence of 73 nucleotides differs by 13 nucleotides. The plasmid-cloned repeating unit differs from the dominant sequence in the total population repeating units by 6 nucleotides in the GC-rich region. Evidence is provided that most of the CpG dinucleotides in 5S DNA are at least partially methylated.

Reproductive counseling in patients who have had a spontaneous abortion.

A detailed morphologic study was made of spontaneously aborted embryos and fetuses from 472 patients. These patients were followed through 638 subsequent pregnancies. The patients were divided into four groups, depending on their previous obstetric history. The outcome of the subsequent pregnancies was correlated with the previous obstetric history of the patient and the developmental status of the aborted specimen. Patients who had never had a live child in the previous obstetric history were identified as a high-risk group. Patients who had had live births and pregnancy loss previously were found to be at significant risk of a subsequent premature infant following the late abortion of a fetus.

A new variety of hereditary sensory neuropathy.

A pedigree with a new form of hereditary sensory neuropathy is described. Ataxia and scoliosis rather than loss of pain and ulcerating acropathy are the principal clinical feature. Analysis of the pedigree suggests a dominant mode of transmission with variable age of onset and perhaps reduced penetrance.

Antibody responses in normal infants and in infants receiving chemotherapy for congenital neuroblastoma.

Three infants with congenital neuroblastoma received a primary series of diptheria-pertassis-tetanus (DPT) immunizations during and after courses of chemotherapy with immunosuppressive medications. Serum IgG, IgA and IgM levels and antidiphthria and antitetanus antibody responses were measured and compared with those of normal infants of similar age. Protective levels of antibody were achieved by the study patients as well as by the control group. These results support the view that children with malignancies who are receiving chemotherapy should not be denied immunization with inactivated vaccines.

A pseudogene structure in 5S DNA of Xenopus laevis.

The 5S DNA of Xenopus laevis, coding for oocyte-type 5S RNA, consists of many copies of a tandemly repeated unit of about 700 base pairs. Each unit contains a "pseudogene" in addition to the gene. The pseudogene has been partly sequenced and appears to be an almost perfect repeat of 101 residues of the gene. The order of components in the repeat unit is (5') long spacer--gene--linker--pseudogene (3') in the "+" strand (or H strand) of the DNA. The possible function of the pseudogene is discussed.

An epidemiological study of congenital reduction deformities of the limbs.

A retrospective study that was undertaken to determine why more infants had been born with congenital reduction deformities of the limbs is reported. The increase was detected by the Congenital Anomaly Surveillance System in 1969. Ninety-three infants born in Alberta and British Columbia in 1969-71 with congenital reduction deformities were matched with 93 normal infants and 93 infants with congenital malformations of a different type. The mothers of all infants were interviewed to obtain information on factors thought relevant to the establishment of a causal relationship. Although there were some significant differences between cases and normaland/or abnormal controls, this number of differences might have been expected by chance. No single causal relationship was established.

Genetic analysis of the nephrogenic diabetes insipidus defect in SWV mice.

Females of the SWV mouse strain had an age-dependent nephrogenic diabetes insipidus. The distribution of water intakes in the F1, F2 and backcross to each parental strain indicated that at least the polydipsia was determined by a few major dominant genes; rather than by a polygenic system. The Smirnof and chi square analyses revealed more than one major gene was involved. A hypothesis was presented that there are two dominant genes (one fully dominant and one codominant) which may be linked. The expression of these major genes was influenced multiplicatively by environmental factors and the water intakes approached a continuous distribution. The defect was sex influenced: SWV males had only a very mild urine concentrating defect but the reciprocal crosses showed no X-linkage. The kidney weight of the SWV females correlated significantly (P less than 0.05) with the polydipsia in the F1, BC SWV and F2 progeny and the use of both traits provided a qualitative measure for classification. The polydipsia and enlarged kidneys probably represent pleiotropy.

Child automobile restraints: evaluation of health education.

An experimental design was used to test the effectiveness of three modalities of instruction in pediatricians' offices. Parents in the experimental and control groups were asked if their children wore a car seat restraint on the way to the office. The parents in the experimental group were encouraged to have their children use restraints via one of the following methods: a pamphlet alone; a pamphlet and verbal instruction by the pediatrician; or a pamphlet, verbal instruction, and a brief slide-tape show. The parents were contacted by telephone, mail, or in person two weeks later and asked if their child used a restraint on their last car trip. There were no statistically significant changes in behavior in any of the gourps. Although the group that received all forms of instruction had the highest mean score, the greatest increase occurred with the control group.
