Obesity and respiratory infection in infants and young children.

A controlled trial has been carried out to test the widely held "clinical impression" that overweight infants have a greater liability to respiratory infections than those of normal weight. Two matched groups of children aged between 3 months and 2 years were studied, containing children whose weight was above the 90th percentile, or was between the 25th and 75th percentiles, at the start of the trial.Data from 120 children of the overweight group were available for analysis, of whom 47 experienced at least one respiratory infection during the trial and 73 did not. Of children remaining over the 90th percentile at the end of the trial 19 had suffered respiratory infections and 28 had not. One baby in the overweight group suffered a "cot death" from acute bronchiolitis. In the control group, 103 cases were analysed-23 had suffered a respiratory infection and 80 had remained free of respiratory infection throughout the trial. These figures are statistically significant and suggest that obesity in infants and young children is associated with an increased incidence of acute respiratory infections.

Children in adult intensive therapy units.

From experience in the Northampton/Kettering area 9 to 10% of all patients in a general hospital requiring care in an intensive therapy unit were aged 12 years or under. Fifty-nine children were admitted to the intensive therapy unit of Northampton General Hospital between May 1967 and August 1969. Of these, 22 had been injured in road traffic accidents, five were surgical emergencies, five had meningitis, four status epilepticus, and four respiratory infections. All of the 30 families interviewed were in favour of their child being admitted to the unit, and none considered that the experience had had any lasting adverse psychological effect on the child. It is suggested that certain carefully selected child patients do benefit from the facilities of an intensive therapy unit, and for this reason such units must be designed, equipped, and staffed with this in mind.

An activated Harvey ras oncogene produces benign tumours on mouse epidermal tissue.

Studies of the mutagenic action required for specific chemicals to produce benign or malignant tumours suggest that in mouse skin at least two genetic events occur before carcinoma formation. The isolation of an activated form of the c-rasH gene from skin papillomas has provided evidence that this gene may be a target for the first mutation, which could constitute the initiating mutation in skin carcinogenesis. In vitro studies indicate that the v-rasH gene of Harvey murine sarcoma virus (Ha-MSV), a replication-defective transforming retrovirus, could impart a conditional initiated phenotype on cultured keratinocytes by blocking their ability to differentiate terminally and arresting them in a late basal cell stage of maturation. We now show that when the Ha-MSV v-rasH gene is introduced into cultured keratinocytes by a defective retroviral vector, skin grafts constructed with cells carrying the mutated ras oncogene produce papillomas on athymic nude mouse recipients. Furthermore, the expression of the exogenous oncogene seems to be regulated at the transcriptional level in the differentiated portions of the benign tumour.

Extended storage of human platelets at 22 C: changes in glycogen and adenine nucleotide metabolism.

The energy metabolism of human platelets stored at 22 C with continuous agitation has been studied. Freshly prepared platelets contain 9.7 mg of glycogen, 9.1 mu moles of ATP, 3.7 mu moles of ADP, 0.9 mu moles of AMP per 10(11) platelets and have an adenylate charge of 0.8. In the first 72 hours of storage there is no appreciable change in the glycogen content, adenylate charge and rate of ATP pool labeling. Therefore, on the basis of these results, the early impairment of function reported by others cannot be ascribed to alterations in the platelet energy metabolism. Upon further storage, however, there is a progressive loss of adenylate and glycogen while hypoxanthine accumulates extracellularly.
