We describe the case of a patient with systemic lupus erythematosus, treated by corticosteroids, who presented during two successive pregnancies with serological reactivation of toxoplasmosis associated with fetal lesions. The first infected fetus died in utero with signs of hydrops. The second fetus was treated in utero with a combination of sulfadoxine and pyrimethamine, administered to the mother, and is now well. The increasing number of immunocompromised pregnant patients with immunity to Toxoplasma gondii may lead to a higher risk of reactivation of maternal toxoplasmosis and congenital infection.
We report a case of thyrotoxicosis periodic paralysis (TPP), occurring as a complication of a Grave's disease in a 31 year-old Caucasian male. It has been suggested that the membrane Na-K pump was involved in the pathogenesis of this complication. In our patient, before treatment, the activity of erythrocyte Na-K-ATPase was significantly decreased, as compared with healthy subjects (228nmol Pi/mg prot/h versus 298 + 60 nmol Pi/mg prot/h) and went back to normal levels post treatment. The activity of this enzyme seems to be prone to genetics factors as well as environmental ones. This would explain the higher incidence of TPP in male and in asiatic people. However, other reports emphasize the role of Na-K-pump-independent potassium influx, which would be more specific of TPP.

We analyzed the mitochondrial DNA of patients with polymyalgia rheumatica, a disease frequently associated with mitochondrial myopathy. In an attempt to study the deletions, we have developed a qualitative PCR method using a highly thermostable polymerase in order to amplify multiple mitochondrial DNA large fragments (up to 12 kb). PCR serves to observe both deleted and normal fractions of the mitochondrial DNA. We found multiple deletions of the mitochondrial DNA in all of the patient muscles. Although these muscles harbored many ragged red fibers, we found no point mutations of the tRNA(Leu)(UUR)) and the mutation at nucleotide position 8344 was not present.

The authors report the case of a sixty-five year old woman initially suffering from a thrombocytopenia. The patient was diagnosed as having an autoimmune disease with a lupus anticoagulant, positive antinuclear antibodies and negative anti-DNA antibodies. She then developed an encephalopathy which was fatal despite corticosteroids. Brain pathology revealed a vasculitis with some giant cells, evoking a granulomatous angiitis of the central nervous system. These clinical and biological features suggest a systemic lupus erythematosus with vasculitis or a primary granulomatous angiitis of the central nervous system. Taking into account the clinical manifestations and the presence of a lupus anticoagulant, we finally preferred to identify it as a primary antiphospholipid antibodies syndrome, despite absence of anticardiolipin antibodies. Contrary to thrombosis, vasculitis is rarely associated with an anticardiolipin antibody or a lupus anticoagulant. However, vasculitis in the course of primary antiphospholipid antibodies syndrome has been reported previously as in this case report.

The antisynthetases syndrome associates lung disease, polymyositis, Raynaud's phenomenon and frequently polyarthritis. Anti-amioacyl-tRNA synthetase antibodies are present. The most common is anti-Jol. We report a case with severe arteritis of the finger with multiple zones of skin necrosis which responded favourably to corticosteroids given in bolus and cyclophosphamid. Finger arteritis, in addition to classical Raynaud's phenomenon, may be a new manifestation of the anti-synthetases syndrome.
The authors report four cases of cardiac amyloidosis, the diagnosis of which was confirmed by endomyocardial biopsy. They underline the value of immuno-histological studies with labelling of the biopsy fragments with anti-transthyretin and anti-light chain immunoglobulin antibodies. This approach provides a more precise typing of amyloidosis and a more accurate evaluation of the prognosis.
OBJECTIVE: To assess the significance of an isolated vasculitis of vasa vasorum on temporal artery biopsy. METHODS: A diagnostic procedure was performed with 28 patients presenting clinical and biological inflammatory process and an isolated vasculitis of vasa vasorum on temporal artery biopsies. RESULTS: Panarteritis nodosa was diagnosed in 5 cases: polymyalgia rheumatica was evidenced in 4 cases; temporal arteritis was strongly suspected in 2 cases; various types of underlying disease were found in 12 patients; a noncharacterized systemic disease was found in 5 cases. CONCLUSION: Isolated vasculitis of vasa vasorum on temporal artery is a meaningful lesion frequently indicating a systemic vasculitic disorder.

In a patient suffering from rheumatoid arthritis, we report the first simultaneous occurrence of two side effects of low-dose methotrexate: an acute megaloblastic anaemia and a pneumonitis. A combination of methotrexate suspension, folinic acid and corticosteroids led to recovery. The correlation between the haematologic and pneumologic toxicity is discussed.
