Treatment of common and plantar viral warts with human lymphoblastoid interferon-alpha--pilot studies with intralesional, intramuscular and dermojet injections.

Pilot studies with human lymphoblastoid interferon-alpha(Wellferon), utilizing the intralesional, intramuscular and dermojet routes, have been performed in 39 patients with common and plantar viral warts resistant to conventional therapy. Clinical efficacy, local and systemic toxicity, and effects on haematological and clinical chemistry parameters has been measured. Our results indicate that the intralesional route is the most favourable as it combines an acceptable therapeutic ratio with a procedure that may be performed quickly and with no prior preparation of the lesion.

Clinical importance of Campylobacter pyloridis and associated serum IgG and IgA antibody responses in patients undergoing upper gastrointestinal endoscopy.

Campylobacter pyloridis was isolated from 77% of 220 (35%) unselected adults undergoing gastroscopy. Isolation was significantly associated with histological gastritis (p less than 0.0001), duodenal ulcer (p less than 0.0001), and to a much lesser extent, with gastric ulcer (p less than 0.05). The relation between the isolation of C pyloridis and peptic ulcer seemed to be independent of coexisting gastritis. In those with no endoscopic or histological evidence of disease there was no relation between isolation and increasing age. Antibody responses to a whole cell sonicate of a strain of C pyloridis were measured by means of an enzyme linked immunosorbent assay (ELISA). Increased IgA (p less than 0.0001) and IgG (p less than 0.0001) antibody titres were found in patients with C pyloridis. Peptic ulceration or gastritis were present in 78% and 100% of patients with a high concentration of IgG and IgA, respectively, but in only 9% and 18% of those with low titres. These results provide further evidence for a possible pathogenic role of these organisms in gastric disease and suggest that immunological markers of their presence might be useful non-invasive indicators of disease.

Prophylaxis against herpes labialis with acyclovir cream--a placebo-controlled study.

Patients with 6 or more recurrences per year of herpes labialis were entered into a randomized, double-blind, placebo-controlled, crossover study. During the trial, they applied acyclovir cream for 16 weeks and placebo cream for 16 weeks to all previously affected areas 4 times per day, and were subsequently observed for a further 16 weeks with no treatment. Results from the 23 evaluable cases showed that although recurrences did occur whilst on acyclovir treatment, patients suffered from significantly fewer days of disease and significantly fewer attacks of cold sores when compared with placebo.

Failure of acyclovir cream in treatment of recurrent herpes labialis.

A double blind, randomised, crossover placebo controlled trial of 5% acyclovir cream, applied topically five times a day for five days, was carried out in 45 patients with recurrent herpes labialis. These patients had a total of 72 episodes, 34 of which were treated with the 5% acyclovir cream and 38 with placebo cream. Treatment was begun by the patients as soon as possible after the onset of prodromal symptoms. There was no significant clinical benefit from treatment with acyclovir cream compared with placebo cream. The median healing times were nine days with acyclovir cream, 10 days with placebo cream, and 13 days when no treatment was given. The possibility that the 40% propylene glycol cream base alone has a therapeutic effect must therefore be considered.

The effect of BW 540C, a novel anti-inflammatory agent, on ultraviolet-induced physical and histological changes in guinea-pig skin.

We have examined the effects of various topical formulations of BW 540C, a novel dual inhibitor of the cyclo-oxygenase and lipoxygenase pathways of arachidonic acid metabolism, utilizing the model of ultraviolet irradiation of guinea-pig ears. During a series of studies, BW 540C preparations were compared with a range of corticosteroids, a cyclo-oxygenase inhibitor and a placebo using both subjective and objective measurements. Results showed BW 540C base to be the best overall agent for treating the pathological changes seen in this model.

Squaric acid dibutyl ester in the treatment of alopecia areata.

A controlled trial of contact allergen therapy of alopecia areata (AA) using squaric acid dibutyl ester (SADBE) was performed on 17 patients. The right side of the patient's scalp was treated and the left side acted as an untreated control. Minimal signs of terminal hair growth were present in only 3 patients after 8-12 weeks of therapy. A majority of patients experienced adverse effects which led to the early withdrawal of 2 patients and the premature termination of the trial. We do not recommend the use of SADBE in the treatment of AA.

Geriatric anesthesia: minimizing the risk.

Aging is an all-encompassing, multifactorial process involving a decreased capacity for adaptation and a gradual decrease in functional reserve of all the body's systems. Aging itself is not truly a disease process but is instead an index of the probability of many disease states. Underlying disease is not always clinically evident in the elderly. Increased operative risk, when present, is attributable to identifiable disease, not simply to the aging process. A thorough understanding of the expected physiologic deviations from the norm must be the benchmark for the anesthesiologist, surgeon, and consultant in formulating a treatment plan. Deviations exceeding those expected must be dealt with cogently by this health care team. It is, therefore, extremely important to establish and maintain communication and understanding among the care team, the patient, and the family. With this approach, we can minimize the risks of anesthesia in the elderly.

L-cysteine, glycine and dl-threonine in the treatment of hypostatic leg ulceration: a placebo-controlled study.

A randomized, double-blind, placebo-controlled trial was carried out in 22 patients with hypostatic leg ulceration. Patients were treated topically with either a cream containing the amino acids l-cysteine, glycine and dl-threonine or the cream base alone (placebo). Most patients had their leg ulcers treated and dressed 3-times per week for 12 weeks. Clinical assessments were conducted at weekly intervals and data from 21 of the 22 patients were available for statistical analysis. The results revealed that the degree of healing and decrease of pain were significantly better in the group of patients receiving the amino acid combination. It would appear from this study that l-cysteine, glycine and dl-threonine in combination are of value in promoting would healing in hypostatic leg ulceration.

Evaluation of the morphanalysis machine in a clinical setting.

The experience of installing and using an Analytical Morphograph in a plastic surgical clinical setting is described. Initial problems have been resolved in safety and routine functioning. Although we have not managed to obtain the consistency of results claimed in the research field we have found the unit useful in the pre-operative, operative and follow-up phases of clinical patient management.

Assessing inflammatory acne vulgaris--correlation between clinical and photographic methods.

During the course of a controlled clinical study the patients' facial acne was assessed using a 0-10 visual-tactile grading system and by lesion counting. In addition, black-and-white photographs and colour slides were taken at each patient visit and subsequently assessed by a panel of three dermatologists. Scores obtained from each method of assessment were subjected to statistical analysis and correlations between the techniques were evaluated. We conclude that a well thought out clinical grading system is the best overall method as it provides a meaningful assessment with speed and reasonable accuracy.

The effects on wound healing of three amino acids--a comparison of two models.

We have utilized two different models to evaluate the role of the amino acids L-cysteine, glycine and DL-threonine in wound healing. Both studies used a double-blind, placebo-controlled, randomized treatment plan. The first model involved the use of blister bases created on human forearm skin by the ammonium hydroxide technique, whilst the second model was that of hypostatic leg ulceration. We conclude that the amino acids did not contribute to the healing of blister bases but that they were of benefit in hypostatic leg ulceration.

An assessment of the relationship between vasoconstrictor assay findings, clinical efficacy and skin thinning effects of a variety of undiluted and diluted corticosteroid preparations.

We have evaluated a number of diluted and undiluted topical corticosteroid preparations in terms of vasoconstrictor potency, clinical efficacy and skin thinning effects. We conclude that, for the materials tested so far, an area under the curve measurement obtained from serial readings of the vasoconstrictor assay allows reasonably accurate predictions to be made concerning the relative clinical efficacy and skin thinning potential of these agents. If further work with a wide range of topical corticosteroids confirms our findings, the value of the vasoconstrictor assay as a predictive model will be enhanced.
