Visual and semantic support for paired-associates recall in young and older adults.

A paired-associates task was used to investigate the degree to which young and older adults benefit from visual and semantic support for recall. Respondents were asked to recall pictorial or verbal items that were either semantically related or unrelated. As has been previously observed in spatial memory tasks, older adults benefited to a greater degree from pictorial materials than did young adults, but only for semantically related items. No such effect was observed for unrelated items. In a second experiment, the imposition of a speed requirement at retrieval eradicated the unequal recall facilitation effect for the older participants. The results of this study are consistent with Craik's (1986) environmental support theory and suggest a link between visuospatial loss and cognitive speed loss in the normal aging process.

Dental photograph standardization for case documentation.

Photography has become an important tool for case documentation, patient education, and case presentation. Standardization of dental photographs is especially important for accurate documentation of the changes that occur as a result of treatment. A protocol for standardization based on anatomic landmarks and patient and camera positions is presented. Specific photographic views are recommended for complete case documentation. The need for written consent before photographing a patient is also discussed.

Acute dental pain, Part 1: Diagnosis and emergency treatment.

The diagnosis and emergency treatment of acute dental pain that arises from periodontal or pulpal involvement are presented. Part I of this two-part series investigates periodontal-related emergencies that manifest as abscesses, cysts, and necrotizing ulcerative gingivitis (NUG). An attempt is made to clarify the differences among these emergencies by presenting their commonly associated symptoms and suggesting a logical approach to diagnosis and treatment.

Acute dental pain, Part II: Diagnosis and emergency treatment.

Part II of this two-part series differentiates and explores endodontic-related emergencies with reversible and irreversible pulpitis. Indications and contra-indications for vital pulp therapy are explained, and treatment is outlined. The inflammatory process involved in irreversible pulpal disease is summarized, and the clinical signs, symptoms, and treatment of irreversible pulpitis (with and without acute periradicular involvement, with pulp necrosis, and acute periradicular abscess with and without cellulitis) are discussed.

Mucous membrane pemphigoid: a case report.

A protracted 30-year course of oral mucous membrane pemphigoid, with repeated exacerbations and remissions, is reported in an 83-year-old white woman. Its clinical, microscopic, and immunologic features are discussed in this article for differential diagnosis from other mucocutaneous vesiculo-bullous diseases.

Ossifying fibroma of the maxillary sinus: a case report.

A case of ossifying fibroma of the maxillary sinus that occurred in a 45-year-old white female is reported. The lesion's radiographic, histologic and clinical behavior are examined. The clinical and radiographic features of ossifying fibroma distinguish it from monostatic fibrous dysplasia despite histologic similarities. The uncommon location of this ossifying fibroma in the maxillary sinus accounts for its large size, aggressive behavior and widespread osseous destruction. The prognosis is excellent after complete enucleation of the ossifying fibroma has been achieved. The benign fibro-osseous lesions of the jaws share similarities in radiographic and clinical appearance, histogenesis and histopathology, and consequently, pose difficulty in classification and treatment. Common histologic features of these lesions include an active proliferation of fibroblats, young and mature collagenous connective tissue, focal areas of mineralization which may resemble small cemeticles and/or irregular bone trabeculae, and multinucleated giant cells. Differential diagnosis of benign fibro-osseous lesions can therefore be made if clinical behavior, radiographic features, and hematologic changes are correlated with the histologic picture. Representatives of this group include true fibrous dysplasia, ossifying fibroma (both central and peripheral types), osteoid osteoma, osteoblastoma, cementifying fibroma, florid osseous dysplasia, proliferative periostitis of Garré, focal sclerosing osteomyelitis and osteitis deformans (Paget's disease).

Mucous membrane pemphigoid: a disease of the elderly.

Mucous membrane pemphigoid (MMP) is a vesiculo-bullous, mucocutaneous disease that generally affects elderly people. The diagnosis of mucous membrane pemphigoid may be overlooked since the disease is often confined to the mouth and desquamative gingivitis may be the only symptom. Gerodontic patients with this condition are often diagnosed as having "desquamative gingivitis" without obtaining the proper therapy to control chronic pain and effect disease remission. The discomfort associated with eating may prevent many elderly patients from obtaining adequate nutrition. Topical and/or systemic corticosteroid therapy are the only available methods of managing this condition. Immunologic studies support the autoimmune nature of MMP and infer that it may be a variant of bullous pemphigoid.

A new and indirect working die technique for fabricating temporary restorations.

A time- and labor-saving indirect method for fabricating temporary restorations that features removable dies (coined the indirect working die technique), is introduced and described. It takes advantage of the accuracy of the traditional indirect method, as substantiated in the literature, but employs new methods and uses materials in a novel way.

Inflammatory papillary hyperplasia: supraperiosteal excision by the blade-loop technique.

Inflammatory papillary hyperplasia (IPH) is a benign, irreversible, persistent, and usually painless lesion of the oral mucosa that is the result of epithelial proliferation. Many surgical methods of treatment have proven to be not totally satisfactory in case of surgery, completeness of tissue removal, healing time, or patient comfort in the postoperative period. A relatively simple and effective procedure is proposed for the supraperiosteal excision of this lesion from the palate, using a razor blade cutting element and handle, called the blade-loop knife, or the Paquette knife handle. The blade-loop technique minimizes trauma and results in a short and comfortable postoperative period.
