Acquired immunodeficiency syndrome manifesting as prostate nodule secondary to cryptococcal infection.

We report the first case of acquired immunodeficiency syndrome (AIDS) presenting as prostate nodularity secondary to mycotic granulomatous prostatitis which mimicked prostatic cancer on digital rectal examination. Transrectal ultrasonography revealed hypoechoic areas in the peripheral zone, which on biopsy specimen were found to represent cryptococcal infection. Subsequent serologic evaluation of the patient confirmed human immunodeficiency virus (HIV) positivity and later AIDS developed in the patient.

Incidental ileal carcinoid associated with multiple urologic malignancies.

Although associated primary neoplasms frequently occur with carcinoid tumors, associated nonendocrine urologic malignancies are uncommonly encountered. We report the first case of an incidental ileal carcinoid associated with bilateral renal cell carcinoma and prostatic adenocarcinoma.

Fournier's gangrene in children.

Necrotizing fasciitis of the genitalia is a rare urologic emergency that is especially uncommon in children. We report a case of Fournier's gangrene in a four-year-old boy and analyze the data from 55 previously reported cases. Pediatric cases have been successfully managed with a more conservative surgical approach and have had a significantly lower mortality rate than adult cases.

Infection of colonized cat fleas, Ctenocephalides felis (Bouché), with a rickettsia-like microorganism.

We report the ultrastructure of a rickettsia-like microorganism in a colonized population of the cat flea Ctenocephalides felis (Bouché). The microorganism occurs principally in the cytoplasm of midgut cells, but similar microorganisms were detected in the tracheal matrix, muscle, hypodermis, ovaries, and the epithelial sheath of the testes. The microorganism has a well-defined cell membrane consistent with rickettsia and measures 0.25-0.45 microns in diameter with lengths up to 1.5 microns. It was observed repeatedly in fleas of 1 laboratory colony, including newly emerged non-bloodfed specimens, but not in specimens from several other sources.

Myocardial metabolic alterations after contrast angiography.

Contrast media used during angiography are known to produce transient alterations in cardiovascular physiology. However, little information is available concerning what alterations, if any, occur in myocardial metabolism after contrast angiography. Sixteen patients with symptoms of ischemic heart disease undergoing elective left ventriculography were studied. Coronary sinus and arterial blood samples were obtained for free fatty acids, glucose and lactate before and after performing left ventriculography with Renografin-76. Coronary blood flow was determined by the thermodilution technique. Five minutes after ventriculography, the arterial level of free fatty acids had decreased by 18.0 +/- 4.9 percent (mean +/- standard deviation) from the baseline (before angiography) samples (probability [p] less than 0.001). Associated with this decrease in arterial free fatty acids was an increase in the myocardial uptake of this substrate. At 5 minutes after left ventriculography, the free fatty acid uptake had increased 48.5 +/- 33.0 percent compared with the baseline value (p less than 0.001). After the injection of contrast medium, there was no significant change in the arterial levels of glucose or lactate. However, significant decreases in the myocardial uptake of glucose and lactate were demonstrated (-72.5 +/- 44.5 percent [p less than 0.001] and -43.2 +/- 22.9 percent [p less than 0.001], respectively) at 5 minutes. The changes in arterial free fatty acids and in the myocardial uptake of the various substrates persisted throughout the sampling period of 20 minutes after ventriculography. These results demonstrate that contrast medium significantly alters myocardial metabolism. These metabolic alterations persist longer than the hemodynamic changes induced by contrast angiography.

Differential pulse polarography of the catalytic waves in serum from patients with urological cancers.

Catalytic waves obtained from the perchloric acid-soluble fraction of serum in the presence of hexaminecobalt trichloride appear to conform to a Langmuir adsorption isotherm. Wave heights for sera from patients with untreated urological cancers significantly exceed those from normal subjects. In general, treatment with diethylstilbestrol decreased the wave height. Effects of some other chemotherapeutic agents on wave heights are also noted.

Mechanisms of spontaneous tachycardia termination in a patient with the Wolff-Parkinson-White syndrome and dual atrioventricular nodal pathways.

A patient with the Wolff-Parkinson-White syndrome and recurrent bouts of paroxysmal supraventricular tachycardia under-went electrophysiologic studies. These studies revealed evidence of dual atrioventricular nodal pathways and a septal accessory pathway. The tachycardia circuit involved anterograde conduction over a slow atrioventricular nodal pathway and retrograde conduction over the accessory pathway. Spontaneous tachycardia termination was frequently observed, was almost always abrupt, and was associated with a beat-to-beat decrease in the A-H interval. In this patient, the mechanism for spontaneous tachycardia termination appeared to be an abrupt shift in anterograde conduction from the slow to the fast pathway, advancing the tachycardia so that the block occurred in the accessory pathway (or atrium). To our knowledge, this mechanism of spontaneous tachycardia termination has not been described previously. Use of agents to facilitate fast atrioventricular nodal conduction (i.e., atropine) may be effective in tachycardia termination for these patients.

Computed tomography: a new method for diagnosing tumor of the heart.

An unsuspected metastasis of liposarcoma of the heart was detected on a computed tomographic (CT) examination of the chest. Further CT study with i.v. contrast administration gave excellent delineation of the tumor. Compared with two-dimensional echocardiography and angiocardiography, CT was best at showing the myocardial and intrapericardial extension of the tumor, but did not show movement of the tumor during the cardiac cycle. CT is a useful noninvasive method for evaluating cardiac metastasis. Its advantages and disadvantages with respect to echocardiography and angiocardiography are discussed.

Admission procedures: A survey of current methodology.

A survey was conducted by the Department of Medical Technology at Wayne State University to determine current admission procedures for student selection. With more applicants applying for clinical education than can be accepted, the selection process becomes critical. Questionnaires were sent to institutions throughout the United States with accredited programs in medical technology. Among the findings, trends indicate the use of grade point average, separate evaluation of science grades, use of an admissions committee, and a dependence on references and transcripts in selecting students. In addition, the study shows that in all the accredited programs surveyed, including universities, hospitals, Veterans Administration hospitals, and others, an average of 82 students apply each year for the 15 available positions, a ratio of 5.5:1.

Slide- tape sequence: a teaching aid in instrumentation.

A study at Wayne State University involving 34 medical technology students was conducted in an effort to provide supportive evidence for the use of selected audiovisual material in the teaching of instrumentation. The study did not conclusively demonstrate the effectiveness of a slide-tape sequence in this university instrumentation course. However, the methodology and evaluative procedures may prove helpful to universities and hospital-based programs in determining the usefulness of their audiovisual material.
