[Severe hepatotoxicity caused by amiodarone: description of a case].

Amiodarone is useful for the treatment of ventricular and supraventricular arrhythmias, but it has been associated with several adverse effects. Amiodarone-induced hepatotoxicity is a frequent complication and often consist in a mild and asymptomatic elevation of liver function tests, although severe cases have been described. A case of severe amiodarone-induced hepatotoxicity slowly resolving after discontinuation of the drug and in which liver biopsy was crucial for diagnosis is reported. Although there are no pathognomonic histopathological findings, phospholipidosis is the morphologic hallmark of amiodarone hepatotoxicity.

[Antimicrobial activity of ozonized water in determined experimental conditions].

Ozone is a potent disinfecting agent which has been used successfully in water treatment, as well as to cure wounds. It can also be used in dental clinics, in laboratories, and in hospitals to clean and disinfect work surfaces and instruments. In this paper we have carried out a study of the antimicrobial activity of ozonized water. Antimicrobial activity in ozonized water on bacterial suspensions and contaminated materials was meaningful and depended fundamentally on concentration and time of exposure. On buccal flora, one rinse alone had no effect, but various successive rinses led to substantial reductions in the number of colonies of bacteria. Ozone had a similar effect, although more pronounced, on the flora of the hands. Ozonized water placed in an open dish kept up antimicrobial activity for the first 20 minutes, but after 30 minutes this activity decreased substantially.

[Unusual tumor of the thyroid: amyloid goiter].

The goiter amyloid is an infrequent complication in the expanded symptomatic range of the generalized amyloidosis. Much more habitual alteration among the reactive secondary amyloidosis: it's characterized by a fast and progressive growth of the thyroid which can bring about compressive problems over the closest cervical structures. In the same way, it can be observed regarding to neoplastic process of the thyroid gland; either carcinoma medullary or another type. It is necessary its with a clear differentiation. We present you a case of a male suffering systematic reactive amyloidosis, secondary to bronquiectasys in the bosom of a pneumoconiosis, with an increase of serum figures of IgA.

[Idiopathic hemochromatosis. Review of the literature and study of 17 patients].

Idiopathic hemochromatosis is an inherited and primary disease. Its definition requires: a) a family history of abnormalities of iron storage; b) an association with HLA; c) inadequate iron absorption in relation to iron stores in liver and other territories; d) the absence of any other known causes of iron overload. The recent evidence of its high family related frequency and the existence of an effective treatment, demand the understanding of this entity and the strongest medical effort towards the diagnosis in the pre-cirrhotic stages. We present here the results from the study of 17 patients diagnosed of idiopathic hemochromatosis over a seven year period in the University Clinic of Navarra. Our aim is to review this subject and to compare our findings with those described in the current literature.

[Primary hepatic actinomycosis].

We present a patient complaining of abdominal pain during eighteen months, localized in left subcostal region, weight loss, asthenia and anorexia for the previous 5 months and fever one month before his admission. He presented painful hepatomegaly, leukocytosis, with an increased percentage of mature and inmature neutrophils, anemia, elevated erythrocyte sedimentation, thrombocytosis, hypergammaglobublinemia and dissociated cholestasis. Complementary exams demonstrated the presence of a poorly vascularized occupying process, localized in the left hepatic lobe. Following laparotomy and biopsy-resection of the process in hepatic left lobe, staining revealed isolated, Gram + filaments and histopathological study showed multiple micro-abcesses displaying actinomyces-like characteristics. There was no evidence of any other pathological process except for previous history of dental abscesses and extractions, which could represent the primary focus for hematogenous dissemination. There was satisfactory response to erythromycin.

Immune complexes in epidemic (type A) hepatitis. Detection by three methods using laser nephelometry.

Thirty-eight children with acute epidemic hepatitis (Type A) and 22 children in "contact" with the disease, 5 of whom subsequently developed hepatitis, were studied. Three technics based on laser ray nephelometry were employed to investigate the presence of circulating immune complexes. Our results indicated that circulating immune complexes are present in the serum during the course of epidemic hepatitis. These were demonstrated in 79% of the patients. The capacity to fix complement diminishes as the disease evolves toward the convalescent phase. We also detected a similar percentage of immune complexes (80%) in the patients during the incubation period. In the subjects in "contact" with the disease who subsequently did not develop hepatitis, the immune complexes were shown to have a greater capacity to fix complement, and the percentage of immune complexes present was higher (94%). The specificity of the immune complexes was verified by correlating the antivirus A antibody activity with the percentage of C4 in the precipitates obtained by polyethylene glycol (3.5%).

[Upper digestive hemorrhage in a patient with von Recklinghausen neurofibromatosis].

Neurofibromatosis (Von Recklinghausen's disease) is uninherited as an autosomal dominant trait. It is characterised by the development of tumors in diverse sites, which may be benign or malignant. The case of a 39 year old woman with Von Recklinghausen's disease is discussed. She presented with a 3 year history of episodes of melaena and iron deficient anaemia. Mesenteric angiography demonstrated a hypervascular tumour in the jejunum. It was surgically excised with an anatomopathological diagnosis of neurofibroma.

Interrelationships between systemic hemodynamics, urinary sodium excretion, and renin-angiotensin system in cirrhosis.

We studied the interrelationships between systemic hemodynamics, sodium excretion and the renin-angiotensin system in 28 nonazotemic cirrhotic patients on a sodium restricted diet. Renal hemodynamics were also assessed. The patients were divided into three groups. Group A comprised 9 patients without ascites or edema; group B comprised 8 patients with ascites and relatively high sodium excretion (41.9 +/- 12.9 mmol/day); and group C comprised 11 patients with ascites and very low sodium excretion (4.8 +/- 12.9 mmol/day). There were no significant differences in urine flow, glomerular filtration rate or effective renal plasma flow between the three groups of patients, although renin and aldosterone levels were significantly increased in group C. Groups A and B did not differ in hemodynamic parameters and no differences were found between the three groups in heart rate or in plasma volume. Group C, however, showed significantly higher cardiac index and lower arterial pressure and systemic vascular resistance. Plasma volume was inversely related to systemic vascular resistance, and natriuresis correlated significantly with both cardiac index (inversely) and systemic vascular resistance (directly). In addition, renin and aldosterone levels were inversely correlated with both mean arterial pressure and systemic vascular resistance. The systemic hemodynamic disturbances in nonazotemic cirrhotics is paralleled by the impairment in sodium homeostasis, suggesting that the decrease in systemic vascular resistance is the primary event leading to hypotension, high cardiac output and the activation of the renin-angiotensin system in these patients.
