Correlation of plasma lipid fractions with colorimetrically determined glycated hemoglobin in a nondiabetic population.

To evaluate the incidence of protein glycation (measured as glycated hemoglobin) on plasma lipid levels in a free-living population, a colorimetric method has been used to detect 5-hydroxymethylforfural (HMF) released from glycated hemoglobin (GHb) and enzymatic methods have been used to evaluate total cholesterol, high-density lipoprotein (HDL) cholesterol, fasting glycemia, and total plasma triacylglycerides. These results have been used to calculate low-density lipoprotein-(LDL) cholesterol levels with the Friedewald formula. Results show that a positive correlation exists in men, but not in premenopausal women, between GHb and fasting glycemia (P less than 10(-14], GHb and total plasma cholesterol (P less than .001), GHb and LDL cholesterol (P less than .0001), and GHb with the atherogenic index total/HDL cholesterol (P less than .0001), whereas a negative correlation was shown between GHb and HDL cholesterol (P less than .05). Fasting glycemia does not correlate with cholesterol lipoprotein fractions, but correlates well with total triacylglycerides. These correlations are significant when they are adjusted by age and body mass index. It has also been observed that the positive correlation between glycation of hemoglobin and total cholesterol occurs constantly at all glycohemoglobin levels, and affects all cholesterol fractions analyzed. This suggests an increased atherogenicity with increasing glycohemoglobin levels. Comparisons between groups with high and low GHb levels show variations in the order of 6.5% of total cholesterol levels and a major proportion in the different cholesterol fractions. The higher-range "glycated" populations show total cholesterol, LDL cholesterol and HDL cholesterol values significantly different from the lower-range "glycated" population.(ABSTRACT TRUNCATED AT 250 WORDS)

Verrucous carcinoma of the vulva. A clinicopathologic and immunohistochemical study of five cases.

The clinicopathologic features of five cases of verrucous carcinoma of the vulva and their staining pattern with antikeratin monoclonal antibodies AE1 and AE3 were compared with those of conventional squamous cell carcinoma. Two patients had local recurrences but none died of the tumor. AE1 and AE3 antibodies stained the entire epithelial thickness in both verrucous and squamous cell carcinoma, but in the former the positivity was uniform and homogeneous everywhere, while in squamous cell carcinoma the positivity was extremely disorganized and patchy. The pattern of expression of monoclonal antibodies AE1 and AE3 confirms that verrucous carcinoma is an extremely well-differentiated squamous neoplasm in contrast to squamous cell carcinoma, which is heterogeneous from a viewpoint of differentiation.

Meningo-angiomatosis: a case report.

We report a case of Meningo-angiomatosis, a rare hamartomatous lesion of the cerebral cortex. The main clinical and pathological features of this entity are discussed.

Fatty acids of plasma and red blood cell lipids in a normal population.

A detailed study of the fatty acid composition of each of the lipids present in plasma and red blood cells of 62 healthy subjects of our area (Barcelona and surrounding counties), by coupling thin-layer and gas chromatography techniques has been made. The results are presented as normative data for comparison with those found in pathological situations. No significant sex differences were found. With increasing age, there was a tendency for the proportion of linoleic acid to decrease. Correlation analyses between the fatty acid composition of different lipids suggested that the interchange of fatty acids between plasma and cells mainly affects the phosphatidylcholine of the latter.

Dégénérescence systématisée optico-cochléo-dentelée.

During the first year of life a girl, the first child of non-consanguineous parents, developed a progressive neurological disease characterized by blindness, severe hypotonia, impaired motor development, microcephaly and mental retardation. Later she became unresponsive to auditory stimuli. She died at the age of 13 months. Post-mortem examination revealed atrophy of the optic nerves and chiasm, lateral geniculate nuclei and superior colliculi, degeneration of the cochlear nerve roots, dorsal and ventral cochlear nuclei, superior olives, lateral lemnisci, medial geniculate nuclei and inferior colliculi and atrophy of the dentate nuclei, hilus and superior cerebellar peduncles. These were consistent with dégénérescence systématisée optico-cochleéo-dentelée. Additional abnormalities included degeneration of the posterior columns in the spinal cord, gracilis and cuneate nuclei and medial lemnisci, atrophy of the spinocerebellar tracts and Clarke's columns, cerebral cortex, cerebellum, chiefly involving the rostral vermis, inferior olivary nuclei and demyelination of the sural nerves.

Effect of antigen used in the detection of anti-Candida albicans antibodies.

Antigens of different origins were used in the investigation of anti-Candida albicans antibodies. This can influence the results obtained. We have assayed three different antigenic preparations weekly for 8 weeks in the study of anti-C. albicans antibodies induced by cutaneous, digestive, and systemic inoculations with C. Albicans ATCC 26555 in rabbits free of specific antibodies, and using indirect immunofluorescence (IIF), direct agglutination (DA) and counterimmunoelectrophoresis (CIE) as serological methods. In IIF and DA, two antigens were used (C. albicans ATCC26555 and C. albicans NCPF3153). In CIE we also used a third commercial antigen. All three somatic antigens were used at three different concentrations. Using IIF and DA the titres obtained with both antigens were similar in different inoculations. The IIF was somewhat earlier in the detection of antibodies, and the titre reached was higher when the antigen used was obtained from the inoculated strain. The detection of precipitins by CIE was in most cases only positive with the antigen obtained from the homologous strain, the highest level being reached in the systemic inoculation.

Effect of cutaneous and digestive colonization in the induction of anti-Candida albicans antibodies: experimental study.

Candida albicans colonization induces antibodies, which must be taken into account in the serological diagnosis of candidiasis. In order to determine the degree of this effect, an experimental study in rabbits free of specific anti-Candida antibodies by cutaneous and digestive inoculation has been carried out. The evolution of humoral response was studied over 8 weeks by indirect immunofluorescence (IIF), direct agglutination (DA), counterimmunoelectrophoresis (CIE) and double diffusion (DD). The cutaneous colonization detectable by culture was maintained until the second week in 70% of the animals and the presence of antibodies detectable by IIF and DA was observed after the 2nd week. The highest antibody titre by IFF and DA was 1/64, and was reached in the 5th week, with a tendency to drop in the following weeks. Precipitins were only detected by CIE in 15% of the animals in the 7th week. Elimination of yeast in stools continued only in 20% of the animals in the 2nd week of the experiment. Antibodies were detected by IIF and DA after the 2nd week, with the highest titres detectable by IFF in the 5th week. Precipitant antibodies detectable by CIE appeared in 15% of the animals in the 8th week.

[Non-gonococcal urogenital infections: etiology].

1,109 patients suffering from non-gonococcal urogenital infections have been studied. 63.4% of the patients had genital discharge. Microorganisms were found in 685 patients (61.8%), only one agent being isolated in 73.6% and two in 22.1% of the cases respectively. The rest of the patients had an association of three or more microorganisms. In women, the most frequently identified microorganism was Candida sp (29.9%), followed by U. urealyticum (20.5%) and G. vaginalis (19.7%). In men, U. realyticum (40.1%) was the most frequent followed by C. Trachomatis (21.4%) and M. hominis (16.6%).

Atypical vaginal adenosis and cervical ectropion. Association with clear cell adenocarcinoma in diethylstilbestrol-exposed offspring.

Knowledge of the evolution of clear cell adenocarcinomas of the vagina and cervix in diethylstilbestrol-exposed progeny has remained elusive despite the known topographical association of these tumors with the tuboendometrial form of vaginal adenosis and cervical ectropion. Twenty radical hysterectomy or radical hysterectomy and vaginectomy specimens of clear cell adenocarcinoma of the vagina or cervix were serially blocked in toto; tuboendometrial epithelium was found in the vagina in 19 (95%) of the cases, usually in greatest concentration at the margin of the tumor, particularly inferiorly. Foci of atypical tuboendometrial epithelium were identified in 16 (80%) of the cases, almost always immediately adjacent to the tumor. Twenty-five of a second group of 47 specimens in which only substantial portions of the vagina or cervix adjacent to a clear cell adenocarcinoma were available for examination also had foci of atypical tuboendometrial epithelium. The frequency with which atypical tuboendometrial glands in the vagina and cervix were associated with the carcinomas and the proximity of the former to the latter provide strong evidence that atypical vaginal adenosis and atypical cervical ectropion of the tuboendometrial type are precursors of clear cell adenocarcinoma.

Endometrioid stromal sarcomas of the ovary. A clinicopathologic analysis of 23 cases.

The clinical and pathologic features of 23 endometrioid stromal sarcomas of the ovary were analyzed. The patients' ages ranged from 20 to 76 years (average, 54 years). The most common presenting symptom was abdominal swelling or pain. In 12 cases the tumors were unilateral and in 11 cases they involved both ovaries. Four of the tumors were Stage I, nine Stage II, and eight Stage III; two were Stage IV because of the presence of pulmonary metastases. In nine of the cases there was a prior, synchronous or subsequent endometrial stromal sarcoma of the uterus. Nineteen tumors with less than 10 mitotic figures (MF) per 10 high power fields (HPF) were classified as low grade and the remaining four tumors with 12 to 30 MF per 10 HPF were designated as high grade. Only 2 of the 19 patients with low-grade tumors are known to have died of their disease and 9 of those whose tumor had spread beyond the ovary at the time of presentation were alive one or more years postoperatively. Three of the four patients with high-grade tumors died of their disease within 4 years.

The release of acetylcholine and of catecholamine from the cat's adrenal gland.

The cat's adrenal gland was perfused in situ with Krebs solution containing eserine; the amount of acetylcholine and of catecholamine released was measured. Splanchnic nerve stimulation (5 Hz for 2 min) increased the release of acetylcholine and catecholamine; the molar ratio of evoked release of catecholamine to acetylcholine was 122 +/- 8. It is suggested that this amplification is achieved because a chromaffin cell granule contains more mediator than does the acetylcholine quantum that releases it. The release per impulse of catecholamine during splanchnic nerve stimulation at 30 Hz was less than that released by stimulation at 1 or 5 Hz. This depression is attributed to a presynaptic failure, because the release of acetylcholine was similarly frequency dependent. The release of catecholamine was linearly related to the release of acetylcholine over the range tested, indicating that the input-output relationship at the splanchnic-adrenal medullary junction is linear. During continuous stimulation of the splanchnic nerve (5 Hz), catecholamine release declined to a level that was 32 +/- 2% of the initial output. This fatigue is attributed primarily to a postsynaptic depression, because the release of acetylcholine was maintained at 71 +/- 6% of its initial level. The presence of eserine in the perfusate was necessary for the release of acetylcholine to be detected, but in the presence of eserine catecholamine release was 90 +/- 10% that in the drug's absence. It is concluded that released acetylcholine is hydrolysed at some distance from its site of release and action. Giant-cell tumor-like proliferation associated with a papillary transitional cell carcinoma of the renal pelvis.

A 56-year-old man with painless hematuria was found to have a papillary transitional cell carcinoma of the renal pelvis with an accompanying nodule composed of multinucleated and mononuclear cells, resembling a giant cell tumor of bone. The nodule was well demarcated from the carcinoma and most likely represents a response to hemorrhage into a cystic cavity. A classification composed of four groups is proposed for extraosseous tumors having features resembling giant cell tumors of bone. This grouping should help in predicting prognosis and avoid the overdiagnosis of sarcoma or undifferentiated carcinoma.
