Usher's syndrome is an autosomal recessive disease characterized by congenital sensorineural hearing loss and retinitis pigmentosa (RP). Evidence for genetic heterogenicity within a single pattern of transmission of RP, be it autosomal recessive, dominant or sex-linked recessive, has been demonstrated by clustering patterns within and between families of specific clinical parameter weightings. Fishman, et al have noted corresponding evidence for subtypes of Usher's Syndrome, of paramount importance in mounting molecular level studies elucidating pathogenesis and management. Our study is of 25 consecutive patients studied by clinical investigation, including audiometric, vestibular and electro-ocular tests. Evidence for and against horizontal as well as proximal to distal receptors is weighed against loss of both visual and cochleo-vestibular systems. The clinical relevance is discussed.
Immunocompetence of the middle ear was studied in guinea pigs in two series of experiments. The first, afferent phase, consisted of grafting iso and allografts in the guinea pig's middle ear to sensitize the animals which were later to receive a skin graft on the back. The second, efferent phase, first sensitized the animals with a skin graft on their back and later in the middle ear. The results show a rather low capacity of the middle ear to elicit a second-set rejection of the skin grafts. This lack of sensitization is explained by the authors by the concept of tolerance.
Intrapetrous ceruminomas are rate tumors; only four cases have been reported in the literature. These had characteristics of cerebellopontine angle tumors. Our case had the characteristics of a tumor of the jugular foramen.
Statistical comparison is established between two methods of stapedectomy, the first one being the "Teflon-interposition", the second being Schuknecht's method (gelfoam and preformed wire). There is statistical evidence that the Teflon-interposition method is better. The results are also compared with those of other workers.
A review of the literature on benign diffuse external otitis was done to throw light on the problems of pathogenesis, etiological factors, and various forms of treatment. A prospective study done on normal adult ear canals and a prospective study of 26 new cases of benign external otitis was done according to clinical criteria elaborated by Senturia to show that Coly-Mycin Otic is a drug to be recommended in the treatment of this disease.
A new therapeutic approach is proposed for chronic purulent otitis media. This clinical condition is often a recurring phenomenon in spite of all forms of treatment. Ten cases are reviewed and the results encourage the authors to pursue this form of treatment consisting of a longterm antibiotherapy of Ampicilline and Cloxacilline in a further study.
