[Clinical value of a new method of therapeutic control of heparin].

A modification of thrombosis time (TT) for therapeutic control of heparin (TTDT), is described. A useful dose of heparin increases plasma TT, which on dilution with fresh plasma rich in platelets (250 +/- 50 x 10(9)/1) at a dilution of 1:4 to 1:8 (+/-20%), causes the increased plasma TT to return to normal values. Three coagulation tests used simultaneously were tried: plasma recalcification time, partially activated thromboplastin time, and TT, used simultaneously in 30 samples of platelet rich fresh normal plasma with different concentrations of heparin (0.05 to 1.0 mu/ml). Results show that modified TT is the most sensitive and reproducible test to detect varying heparin dosage. Fifty two patients without coagulation deficiencies and with various clinical thrombi, and 22 with generalized intravascular coagulation (CID) treatment with 100mu of heparin XK of body weight every 4 to 6 hours, were studied. Results are described. Advantages of TRDT for therapeutic control of heparin are discussed.

Treatment of acute leukemia during pregnancy: presentation of nine cases.

The hematologic and obstetric problems of nine pregnant patients with acute leukemia are analyzed. Seven patients received chemotherapy during various trimesters of pregnancy, four during the first 10 weeks, and there was no evidence of congenital malformations in any of their offspring. Six of nine babies are alive and healthy with a normal growth 2 months to 15 years after birth, one child was stillborn, and two children died 21 and 90 days after birth. These results were compared with others in the literature and the conclusions are that pregnancy is not a contraindication for treatment of acute leukemia, that cytotoxic drugs may not cause congenital malformations, and that the management of pregnant patients with acute leukemia should be carefully planned.

[Hepatic biopsy in acute leukemia].

In 25 patients with acute leukemia that presented chantes in liver function tests, 29 liver biopsies were done by percutaneous needle or peritoneoscopy and directed needle, to determine the nature of the lesion. In one case leukemic infiltration was found; 16 patients showed nonspecific lesions, one, granulomatous hepatitis, one, viral hepatitis; one showed no changes in the biopsy. In these twenty cases the treatment was not modified, the survival time was from 15 to 57 months (median 25 months), and 14 an still in remission. Five patients (20%) showed drug-induced hepatitis; in these it was necessary to modify to chemotherapy, the survival time in four cases was 2 to 9.6 months (median 4.8 months) and one is still in remission after 36 months of treatment. It is concluded that hepatic biopsy may be very useful in evaluating the undesirable effects of treatment of acute leukemia and in making therapeutic decisions. The risks of this procedure and the contraindications are similar in patients with acute leukemia and those with other diseases.
