A retrospective analysis was carried out of 311 reconstructions of major defects following head and neck cancer treatment. Three hundred thirty-one flaps were used; they included cutaneous flaps from 1972 to 1979 and myocutaneous flaps (MCF) after 1979. The aim of this study was to compare the healing patterns of the two types of flaps used in similar circumstances. Even though the use of myocutaneous flaps reduced necrotic complications, there was no significant improvement in overall healing. In this type of reconstruction, local conditions as well as more general factors have greater prognostic significance. Apart from considerations of reliability, other criteria have led to myocutaneous flaps being the treatment of choice in cervicofacial cancer repair. However, specific indications for the use of cutaneous flaps remain.

Anatomical data from 20 laboratory dissections are employed to describe a composite flap associating skin, the latissimus dorsi muscle, and ribs, important anastomoses existing between the intercostal and inferior scapula vascular systems. Two cases were treated by single-stage extensive reconstruction of the mandibular region employing this osteomusculocutaneous flap of the latissimus dorsi.

After recalling the descriptions of classical authors, 100 dissections conducted to the study of the origin of the phrenic nerve (nervus phrenicus). It appears as variable and asymetrical. The presence of an accessory phrenic nerve has been rarely seen. The classical proximal anastomoses of this nerve are inconstant or rare.
Echotomography of the parotids in 88 patients was successful in detecting normal parotid morphology or inflammation of the parenchyma (parotitis) in 100 p. cent of such cases, and detecting the precise localization of tumors (adenopathies or intraparotid tumors) up to 3 mm in diameter in 93 p. cent of cases. Though cystadenomas do not provide a specific image, their appearance is typical. Positive diagnosis was possible in 85 p. cent of mixed tumors of 1 cm diameter or more, and in 5 out of 7 cases of malignant lesions. The diagnostic value of this examination is markedly limiting that of parotid sialography.

The authors report a case of giant diverticulum of the sigmoid colon, a very rare complication of diverticulosis of the colon, since only 38 cases have been reported in the literature. The diverticulum was seen at barium enema in a 66-year-old man complaining of digestive symptoms consisting of lower abdominal pain, tenesmus, diarrhea and fever. Segmental resection of the sigmoid colon including the site of implantation of the diverticulum with end-to-end anastomosis at the same operative stage led to definitive cure. On the basis of a study of the literature, the authors define the clinical, diagnostic and pathogenic characteristics of the condition.

Dacryorrhinostomy is a last-chance operation for severe palpebral and canalicular damage which can lead to loss of tissue. The reconstruction of a delicate tube remains a gamble because of the absence of normal structures. The multiplicity of the procedures proposed clearly demonstrates the difficulty in obtaining a functional result. The excellent results obtained with this procedure incited the authors to prolong their study and increase the number of cases in order to confirm the validity of their findings in a large series.
This procedure seems to be a valid one, as after using it in 21 cases over a period of one and a half years, there was only 1 failure due to a too hasty removal of the gauze wick. Furthermore, in 12 cases, a catheter was applied to the lacrimal puncta to irrigate the anastomosis with rifocin and hydrocortisone for several days. The advantages of this method appear to be simplification, and less risk of stenosis by using a full flap, when compared to dacryocystorrhinostomy.
After 5 years of experience of surgical repair of the lacrimal duct, the authors consider that secondary repair using arterial grafts has now become a reliable and valid procedure in all cases where part of the duct, at least, still remains.
