Adenine arabinoside in severe thrombocytopenia due to hemorrhagic varicella.

An 18-year-old man, admitted because of severe bleeding manifestations due to varicella thrombocytopenia was administered 800 mg/day adenine arabinoside (Ara-A) intravenously for 5 days. 2 days after the start of this treatment the bleeding phenomena became markedly reduced and a progressive increase in the platelet count was noted thereafter. Although a spontaneous improvement is known to occur in patients with varicella thrombocytopenia, the rapid clinical improvement after the Ara-A administration is suggestive for its beneficial effect in the presented patient.

Familial cryoglobulinemia and C4 deficiency.

The coexistence of cryoglobulinemia and C4 deficiency was found in one family. Twelve subjects were examined, most of them being asymptomatic. Cryoglobulins were found in 9 members and its type was defined in 6 of them, 3 having mixed IgM-IgG and 3 having monoclonal IgM cryoglobulins. The concentration of the third component of the complement system was normal in 10 subjects, while the concentration of the fourth one was found to be below the lower limit of normal in 9 subjects. Only C4 deficiency--and not cryoglobulinemia--was HLA-linked. There was no evidence of HBs antigenemia. The results of this study are discussed in the light of the pertinent literature.

The activities of enzymes of purine metabolism in murine thymus dependent lymphocytes.

Measurement of the activities of purine metabolizing enzymes in murine T cell subpopulations showed that these activities differed markedly among T cells of different levels of functional maturity. The activities of adenosine deaminase and deoxyadenosine phosphorylation were highest in immature, PNA + thymocytes, while the activities of purine nucleoside phosphorylase, ecto-5'-nucleotidase and deoxyguanosine phosphorylation were highest in mature, splenic T cells. These enzymes' activities can be used as biochemical markers for T cell of different degree of maturation.

Hodgkin's and non-Hodgkin's lymphomas masquerading as "idiopathic" liver granulomas.

Three patients presented with liver granulomas and fever. An intensive investigation was initially negative. Corticosteroids were administered with dramatic clinical improvement. Ten months to 3 years later, upon tapering the corticosteroid dose, the initial manifestations reappeared. In two patients Hodgkin's lymphoma was discovered and in a third histiocytic lymphoma. Thus, the diagnosis of lymphoma should be considered in patients with idiopathic liver granulomas, fever, and hepatosplenomegaly without lymph node enlargement, who respond initially to steroid administration but relapse upon tapering the dose.

Life-threatening ventricular arrhythmias in septicemia.

Sixteen episodes of ventricular tachycardia and/or fibrillation, 12 of which occurred during shaking chills, were recorded in six patients with septicemia. All patients were greater than 60 years of age and had suffered a previous myocardial infarction. Patients who survived the condition sustained no further arrhythmias during a follow-up period of 1 to 4 years, despite the fact that no antiarrhythmic medication was administered. It is suggested that patients greater than 60 years of age who had suffered a previous myocardial infarction should be carefully monitored during septic episodes and especially during shaking chills, since these may represent vulnerable periods facilitating the precipitation of potentially lethal arrhythmias.
