Butane abuse associated Encephalopathy.

Butane contained in household products is easily available for abuse and is not subject to legal prosecution in Germany. The toxicological properties of butane mainly affect the heart and the CNS. A serious pathophysiological mechanism is asphyxia due to the replacement of oxygen by butane. We report an abusive butane inhalation in a 15-year old girl, resulting in cardiopulmonary resuscitation and subsequent development of severe brain damage. After reviewing the medical literature and questioning German toxicological information centres it became obvious, that abuse of butane is an increasing problem. We give an survey about the complications associated with butane intoxication.

Adrenal renin expression and its role in ren-2 transgenic rats TGR(mREN2)27.

Transgenic rats, termed TGR(mREN2)27, have been generated carrying the mouse ren-2 renin gene. As intended the gene was expressed highly within the adrenal gland offering the opportunity to study the intraadrenal renin-angiotensin system in vivo. Concomitantly with increased adrenal renin activity, the production of adrenocortical steroids are elevated and the regulation of aldosterone production is impaired in TGR(mREN2)27. Furthermore, the transgenic rats develop severe sodium dependent hypertension. Since kidney and plasma renin concentrations and plasma angiotensins are low in the transgenic model, those factors cannot account for the altered regulation of aldosterone production. Instead the experiments with TGR(mREN2)27 demonstrate the functional role of the local adrenal renin-angiotensin system.

Population and sex differences in arm circumference and skinfold thicknesses among Indo-Pakistani children living in the East Midlands of Britain.

Arm circumference, triceps and subscapular skinfold thicknesses of 2224 Indo-Pakistani children aged 3-10.9 years living in Nottingham and Leicestershire were measured. The children were classified into populations according to country of origin, and religion, prior to analysis. Significant between-population differences were shown for all three variables, the effect being stronger in males than in females. Sex differences in arm circumference and triceps skinfold thickness were greater than those found in the British reference populations, whilst the patterns of fat distribution differed from the British standards for all the Indo-Pakistani populations, male and female.

Subacute bacterial endocarditis due to Actinobacillus actinomycetemcomitans.

Sixteen documented cases of Actinobacillus actinomycetemcomitans endocarditis have been reported in the past 15 years. The characteristic granular growth and the fastidious nature and slow-growing character of this organism decrease the yield of positive blood cultures. Two recently observed cases of subacute endocarditis due to Actinobacillus are reported, one in a patient who required surgical intervention for complications of his disease and the other case associated with an aortic prosthetic valve. The first patient had late embolic complications which are commonly seen with Actinobacillus endocarditis. A review of the literature; including a synoptic table with clinical failures, treatment, and outcome is presented. Unless special care is taken to isolate these slow growing organisms, these cases will be misclassified as culture negative endocarditis.

Depression dependent parkinsonism: case report.

With increasing depression a patient's Parkinsonian tremors increased. With successful treatment of the patient's depression with imipramine, the tremors disappeared, but slowly began again after drug noncompliance resulted in recurrence of depression. Evidence suggesting a relationship between depression and extrapyramidal dysfunction is discussed. The need for a greater understanding of the role of the extrapyramidal system in emotion and behavior and the connections between the extrapyramidal system to limbic and cortical structures are noted.

Toxic epidermal necrolysis.

Although Ms Weeks was supported throughout this period, we were concerned that she would require further psychological help. At the end of her stay she decided that she did not require help. However, she knew that if at a later date she changed her mind, it would be arranged. Ms Weeks was offered an appointment with the Red Cross camouflage service, which was on hand to offer advice about appropriate techniques to hide the hyperpigmentation on her face and other exposed areas, but this again was declined. Ms Weeks was in ICU for three-and-a-half weeks and her skin healed well. She was left with multihyperpigmented skin, which over time may resolve itself.

After the examination: care of low vision patients beyond ocular services.

1. A low vision program should assist patients by assessing their living conditions, daily routines, and personal interests; providing practical suggestions to enhance safety and independence; advising them on availability of programs, and assisting in emotional and psychological rehabilitation of patients and families. 2. Within the ophthalmic profession, a bridge is needed between the importance of the psychosocial aspects of patient care and the importance of the medical and scientific research community in their quest for advanced technology and treatment. 3. It is important not to instill a false sense of hope that vision will be restored. Excellent prognostic signs for social adjustment and independence begin with a realistic acceptance of blindness.

[Origin and course of renal arteries. A CT study for determining optimal projection plane in renal artery angiography].

In an attempt to optimize renal artery angiography we performed a CT-study. In 500 patients we determined the angle at which the arteries arise from the aorta and their course. Both renal arteries are in one plane, which is tilted 13 degrees ventrally on the right and 13 degrees dorsally on the left. More distally we found an angle of 35 degrees on the right and an angle of 29 degrees on the left in the dorsal direction. These findings were independent of age and sex of the patient.

The neurologist's use of rating scales, EEG, and tranquilizers in dealing with hysterical symptoms.

This article is the result of an effort to help the neurologist deal with the hysterical patient on both a diagnostic and therapeutic level. The conclusion reached is that use of major tranquilizers may be needed to keep hysterical symptomatology under control while diagnostic investigation is proceeding and, if the diagnostic studies are negative, may be used to help ameliorate symptoms in patients unwilling to see a psychiatrist.

Hypogammaglobulinaemia in systemic lupus erythematosus: report of a case with evidence for spontaneously activated T suppressor cells.

A patient with hypogammaglobulinaemia associated with systemic lupus erythematosus (SLE), a healthy HLA-A, B, C, D-identical and mixed lymphocyte culture (MLC)-negative sibling, and two mutually HLA-A, B, C, D-identical siblings were investigated. Blood mononuclear cells from the patient contained a high proportion of T lymphocytes with the suppressor/cytotoxic phenotype, and in vitro no development of Ig-secreting cells was observed in response to pokeweed mitogen (PWM) or to Epstein-Barr virus (EBV), as opposed to cell cultures from the siblings. In cell cultures from the two healthy HLA-identical siblings, T-lymphocytes as well as monocytes/macrophages (M phi's) could be replaced with corresponding cells from the sibling without major alterations of the pokeweed mitogen (PWM)-induced B cell response. In PWM-stimulated co-cultures of B cells from the patient with healthy HLA-identical T cells, moderate numbers of IgM-secreting cells developed, but not IgG- or IgA-secreting cells. T cells from the patient co-cultured with healthy HLA-identical B cells suppressed their Ig-secretion; this effect was abolished by irradiation of the T cells. The in vitro generation of T suppressor cells by concanavalin A (ConA) was normal. No evidence for abnormal suppressor function of monocytes/macrophages was obtained. Thus in this patient, spontaneously activated T suppressor cells as well as defective B cells were associated with hypogammaglobulinaemia.
