[Diagnosis of myxomas of the left auricle by ultrasonic echography].

The two case histories of myxoma of the left auricle reported in this paper illustrate how echography is a diagnostic method which is non-invasive, simple, and relatively accurate provided that different angles of incidence are used, and especially the "root aorta-left auricle" incidence. It is indicated especially in cases with variable mitral murmurs on auscultation, but also where there is a very large left auricle, heart failure of unknown cause, or even an apparently straightforward case of mital stenosis. Moreover, as one of these case histories demonstrates, the technique allows both short and long term postoperative follow-up to be carried out with ease and safety, so that a possible recurrence can be picked up later.

[A genealogical study of Steinert's disease (author's transl)].

As a result of retrospective genealogical studies on 41 cases of myotonic dystrophy, the authors have found: 1) sound arguments to support the hypothesis of anticipation often discussed in association with this disease; 2) a high rate of mortinatality in the progeny of affected females; this aspect merits being taken into consideration in genetic counselling and should be reported to obstetricians. Its pathogenetic determination still remains obscure; 3) a relative frequency of illegitimacy in familes with affected fathers. On the basis of the low fertility associated with the disease, the authors postulate that new mutations may not be rare. Finally, they emphasize two little known symptomatic features, the first being the frequency of lithiasis, the second the frequency of neoplasms. This latter observation, however, still requires confirmation.

[Study of the action of acebutolol on the parameters of cardiac contraction measured by ultrasonic echography].

Recording by means of echography the motions of the posterior wall of the left ventricle makes it possible to compare in subjects acting as their own controls the amplitude of the movement, contraction and decontraction time of the myocardium before and after medication. This report concerns the variations recorded in 10 healthy patients before and after oral administration of 200 mg/day of acebutolol for 10 days. A statistically significant decrease of 8-9 p.cent of the various parameters was found. It probably reflects the negative inotropic action exerted by the drug.

[Determination of myocardial contractility using ultrasonic echography].

Ultrasonic echography makes possible the recording of movements of the septum and posterior wall of the left ventricle. On graphic records of good quality it is thus possible to measure accurately the transverse diameter of the left ventricle throughout systole and to determine the average and maximal speeds of decreases in this diameter, which are proportional to the VCF. Echography, a non-invasive technique, thus gives data with regard to myocardial contractility of similar value to the information provided by cineangiocardiography. In those cases where it is not possible to obtain such tracings, an approximation may be made, validity of which appears to be confirmed by clinical experience.

[Value of the determination of gamma glutamyl transpeptidase in the detection of hepatic metastases of bronch-pulmonary cancers].

The authors have dosed gamma glutamyl transpeptidase in 42 patients with primary bronchopulmonary cancer, of whom 14 had liver metastases. In these 14 patients the level of gamma glutamyl transpeptidase was always high, except in 1 case; while in the remaining lot, a moderate and mostly belated increase was noticed in only 32 percent of cases. The dosing of gamma glutamyl transpeptidase present some interest in the detection of liver metastases in primary bronchopulmonary cancers, specially when coupled with the dosing of alcaline phosphatase. Interpretation of results should take into account the existence of an associated liver-gall bladder involvement and of a recent surgical operation; both factors being capable of raising the level of gamma glutamyl transpeptidase.

[Pharmacodynamic study of myocardial contraction using ultrasonic echography during isoproterenol perfusion in the normal subject].

An echographic study, both before and under isoproterenol infusion, was performed on 50 normal subjects. The movements of the posterior wass of the left ventricle were found to be averagely increased of 62% as regards the amplitude, 114% for the mean speed and 124% for the maximum speed of contraction. The transcerse diameter of the left ventricular cavity decreased by 5.25%. The circumferential fibres shortening speed expressed in circumference by second increased by 128%. An uninvasive method, echography, as cineangiography, makes it possible to assess the myocardial contractility on the basis of measurement of the average speed of circumferential fibres and of the diameter of the left ventricle.
