We describe a novel method by which phagocytosis, digestion and killing of Paracoccidioides brasiliensis yeast cells by polymorphonuclear leukocytes or other phagocytic cells may be estimated simultaneously. Suspensions of P. brasiliensis (yeast-like phase) were sonicated, counted and incubated at 37 degrees C with known numbers of phagocytes. Control preparations contained no phagocytic cells. At given intervals samples were incorporated into gelatin nutrient medium and droplets of the mixtures were incubated at room temperature. Live yeast-like P. brasiliensis germinate in vitro and produce filaments. After incubation, droplets may be melted and examined under phase contrast optics, or the cells may be washed and stained by a variation of Papanicolaou's method. Digested P. brasiliensis, intact but non-germinating yeasts and filamented (viable) yeasts may be identified and counted. Killing and digestive abilities of phagocytes may be estimated by the difference between values obtained from phagocyte-containing and control preparations.

Endemic Chediak-Higashi Syndrome occurs in a restricted geographic area (Pregonero, State of Táchira, Venezuela). Neutrophils from these patients were unable to digest Candida albicans in vitro, but showed normal or increased metabolic activities. This finding supports the view that the endemic syndrome is bona fide Chediak-Higashi Syndrome.

Fifty-five patients underwent ultrasound-guided fine-needle aspiration from the pancreas. Adequate material was obtained from 33 of the 41 patients with pancreatic malignancy. A cytological diagnosis of cancer was made in 31 (94%) of these; the overall clinical success rate was 75.6%. There were no false-positive results or complications. The sensitivity of the procedure may be increased by repeating the punctures.
The normal stimulatory response that the TRH has on the TSH in children, is inhibited by the previous administration of bromocryptine (p less than 0.05). Under the same circumstances there was a significant increase in the release of the GH.

Diabetes is an emerging epidemic throughout the world. In our city alone, there are approximately 25,000 known diabetics (5% to 10% type 1) among a total population of 1.7 million inhabitants, and the incidence is increasing among all age groups. Islet transplantation is a potential treatment for type 1 diabetes mellitus. For this reason, we intended to establish an islet transplantation program. This required competent and well-trained professionals, a specially planned facility adhering to rigid regulations regarding safety and sterility, and a detailed study of the ethical laws and rules involving transplantation. In this article, we describe the process including any difficulties or barriers encountered due to limited resources in a developing country. We also describe all stages of personnel training and the necessary equipment and work area of a similar specialized center following the guidelines of the Brazilian National Agency for Health Care. Finally, we discuss our expectations for the initial phase of our islet transplantation program.

OBJECTIVE: To evaluate AIDS case reporting in the Spanish regions as compared with the AIDS deaths registered in mortality statistics, in order to identify possible deviations indicative of underreporting. METHODS: We carried out an ecological analysis taking each region as a unit. We compared incidence and mortality obtained from the AIDS reporting system with the HIV/AIDS deaths obtained from the mortality statistics of the Spanish Institute for Statistics. The 1986-1998 period was analysed globally, then the analysis was repeated for the 1995-1998 period. RESULTS: In the 1986-1998 period there was a good correlation (r = 0.93) between the AIDS incidence rates and HIV/AIDS mortality rates obtained from death statistics. Some regions presented an AIDS incidence lower than expected according to their mortality rate when it was compared with the national average, with Asturias (27%), Comunidad Valenciana (26%), Andalucia (20%), Ceuta (18%) and Cantabria (13%) standing out. Taking as a reference the five regions with the highest completeness, the underreporting in Spain was of 13%. In the 1995-1998 period these deviations increased moderately. In the 1986-1998 period, 18.9% less deaths were notified to the AIDS reporting system in comparison with death statistics, showing a great variability between regions. CONCLUSIONS: In Spain the reporting level of AIDS cases is acceptable for the aims of surveillance, although in some regions measures to improve it should be taken. The notification of deaths to the AIDS reporting system presents great deficiencies.
