Foetal and maternal plasma levels of steroid sulphates in human pregnancy at term.

Umbilical cord arterial and venous, and maternal peripheral vein blood samples were collected during 20 deliveries after uncomplicated pregnancies. Using a gas chromatographic method, oestriol, oestriol sulphate, progesterone and a number of neutral steroid mono- and disulphates were quantitated in the cord plasma samples and a number of neutral steroid mono- and disulphates in the maternal plasma samples.

Leg lengthening by osteotomy and gradual distraction: an experimental study.

Osteotomy and gradual distraction of the distal radius was performed on 12 sheep. After 3-6 weeks of daily distraction by external fixation, lengthening of 0.5-1.8 cm was achieved. Three weeks after distraction was begun, striae of new bone, organized in the direction of the distraction, were noted radiologically. The bony consolidation developed from the osteotomized bone ends to the center of the distraction area. Medullary structures also contributed to bone formation in the distraction area. Collagen bundles, organized in the direction of distraction, formed a template for the developing bone. The medullary canal remodeled 16 weeks after the operation. Solid bone formed after the distraction procedure with no additional bone transplantation.

Biological factors and predictability of bracing in adolescent idiopathic scoliosis.

Biological factors that could have a predictive value in treating idiopathic scoliosis by brace were studied in 107 patients (102 girls and 5 boys). The mean age of the patients was 14 years 6 months, and the mean Cobb angle of the curves was 36 degrees at start of treatment. The Boston brace was used for an average of 1 year 6 months, and mean follow-up time after weaning was 3 years. Scoliosis with an apex of Th 10-12 proved to be the most favorable for the final result, with a mean correction of 2 degrees. All other curves remained unchanged. Patients who had a period of rapid growth just before or during treatment had a better final result than the others. The final result was also better when treatment was started before menarche.

Cervical spine in diastrophic dysplasia: an MRI analysis.

In this cross-sectional study, we evaluated the cervical spine in patients with diastrophic dysplasia (DD) by using magnetic resonance imaging (MRI). From 90 (57 female, 33 male) patients of different ages (0.3-56.0 years), T1- and T2-weighted images were obtained. The craniocervical junction and status of the medulla were examined, and the transverse areas of dural tube and medulla were measured. Alignment of the cervical spine, vertebral abnormalities, and disc changes also were evaluated. The cervical spinal canal was moderately narrowed, particularly in adults. The transverse areas of the medulla and especially of the dural tube were smaller compared with a normal population (p < 0.0001). There was no stenosis of the foramen magnum in patients with DD, but the spinal canal was narrowed. Degenerative changes were common. In all age groups, intervertebral discs were dark and disc heights were narrowed. Three (3%) patients (two children, one adult) had cervical kyphosis. Compression of the medulla was noted in association with severe cervical kyphosis in one child and one adult. Typical findings of the cervical spine in DD were exceptionally wide foramen magnum, narrowed spinal canal and early degenerative changes, and in older age groups, especially midcervically narrowed spinal canal. Severe cervical kyphosis caused medullar compression. The intervertebral discs developed abnormally. Abnormal disc structure may be one etiologic factor in the development of cervical kyphosis. Early and rapid progression of the degenerative changes is a normal finding in patients with DD, regardless of their age, and this also explains the stiffness of the cervical spine in clinical examination.

Bone formation and remodeling after symmetric and asymmetric physeal distraction.

Bone formation after symmetric and asymmetric physeal distraction was studied in growing sheep. After gradual distraction by external fixation, separation of the growth plate occurred. Fluorescence microscopy, microradiography, and histomorphometric methods were used to study neometaphysis formation. The bone was formed from the inner layer of the periosteum to encircle the distraction area, where the osteogenesis occurred according to the collagen bundles. At consolidation, the distraction area was composed of lamellar trabecular and partly woven bone. The bone trabeculae in the distraction area were thicker than in the control radius. After asymmetric physeal distraction, spontaneous correction of the formed angulation occurred through remodeling and asymmetric physeal growth. Although local physeal changes were noted histologically, enchondral growth usually continued after distraction.

Ultrasonography of the patellofemoral joint in diastrophic dysplasia.

Plain radiographs do not provide adequate information in diastrophic dysplasia (DD) because of abnormal ossification and severe deformation of the knee joint. Radiographically, the lateral femoral condyle is hypoplastic, and the position of patella is difficult to define. In clinical examination, the patella is inferior and lateral. In this study, the anatomy of the patellofemoral joint in DD is described using ultrasound. Both knees of 13 patients with DD were examined. The alignment of patella, the angle of femoral sulcus, and the size of both femoral condyles were measured. Student's paired t test with two-tailed significance probability was applied for differences in mean values, and their 95% confidence limits were computed. The femoral sulcus angle averaged 129 degrees and was deeper than that (142 degrees) in normal knees. The lateral femoral condyle was smaller than the medial condyle. The patella was aligned lateral to the bottom of femoral sulcus. In 10 degrees knee flexion, the patella remained laterally in the sulcus with no true patellar dislocation. Vertical position of the patella averaged 2.9 mm distal to the femorotibial joint. The study showed that, in patients with DD, the anatomic complex of the femoral condyles and patella deviates laterally, and the patellofemoral joint is deformed with a hypoplastic lateral condyle, a deep femoral groove, and a distally aligned patella.
