[Endophthalmitis after cataract surgery].

Endophthalmitis is one of the most serious complications after surgery of cataract. The reported incidence is 0.07 to 0.58%. The authors evaluate a group of 34 cases of endophthalmitis from a total of 13,247 eyes operated on account of cataract in 1990 to 1999 at the ophthalmological department of the Central Military Hospital. They describe the method used and the results. The conclusions are summarized as follows: 1. Early pars plana vitrectomy in developing endophthalmitis improves the chances of maintaining satisfactory visual functions. 2. After the change to ambulatory surgery the authors did not record a higher percentage of postoperative endophthalmitis. 3. As a rule surgical solution of endophthalmitis does not call for IOL.

[Personal experience with treatment of vascular diseases using the Leksell gamma knife].

The application of radiosurgical treatment in ophthalmology is a new and not quite common procedure. The existence of 80 departments in the world with more than 15,000 treated patients suggests however that the therapeutic method is being extended and that its application in ophthalmology will be more extensive in future. The most numerous indication groups for treatment with a Leksell gamma-knife(LGK) worldwide are vascular malformations (44%), benign tumours (33%) and malignant tumours (22%). Other groups include among others functional disorders and metastatic processes (15).

[Clinical experience with Spersadex comp. gtt].

The preparation Spersadex comp. gtt. was tested on three groups of patients (60 patients, 109 eyes) in order to assess its effectiveness in indications outlined by the manufacturer. The preparation has a very favourable antimicrobial, and antiphlogistic action. It proved very effective also in the treatment of chlamydial infections and in the postoperative treatment the combination of the antiphlogistic effect and the antibiotic action proved very useful, though tested only in small groups of patients. Spersadex comp. gtt. is a very effective preparation, well tolerated by patients and it supplements in a useful way the range of eye drops by a new antibiotic-corticoid combination.

[ProVisc--a new viscoelastic material for surgery of the anterior segment. A multicenter clinical study].

Within the framework of a multicentric clinical study which was implemented at nine selected departments a new viscoelastic material ProVisc of Alcon company was tested during extracapsular extraction of cataract with implantation of posterior chamber intraocular lenses. A total of 100 eyes of 100 patients were operated and all pre- and postoperative data were recorded in uniform protocols. The selection of suitable patients and the type of surgical procedure was within the responsibility of the surgeon. The type and frequency of surgical and postoperative complications was similar as when other viscoelastic material ProVisc can be recommended as safe and effective in modern surgery of cataract.

[Suture technic in perforating keratoplasty and postoperative astigmatism].

The authors evaluate postoperative vision, value of astigmatism and keratometry in 25 patients operated in 1992 by perforating keratoplasty, incl. 11 where the disc was fixed by interrupted stitches and 14 by continuous diagonal stitches. After evaluation of the influence of interrupted stitches and continuous diagonal stitches on postoperative astigmatism in perforating keratoplasty the authors recommend the use of diagonal suture. The reason is in the first place the uniform spreading and good adaptation of the transplanted cornea, as well as slighter traumatization and easier removal of stitches. An important role is played also by the larger number of measurable radii of the corneal curvature with regard to possible postoperative correction of astigmatism either by added stitches or differentiated adjustment of the tension of the continuous stitch.

[Experience with a nanosecond Nd-YAG laser].

The author summarizes general experience with the three-year use of a PV-135 Nd-YAG laser of Lasertek Co. A total of 1817 operations were performed in 1134 patients. The ratio of successful operations was more than 91%, improved eye-sight in optic operations was achieved in 63.8%.
