In vitro studies on the influence of carbomers on the availability and acceptability of estradiol gels.

The objective of the present study was to evaluate the pharmaceutical properties of estradiol (CAS 50-28-2) gels: pH, viscosity, texture, spreadability, evaporation of solvent and transcutaneous diffusion from carbomer-based formulations. This study was performed with the aim of measuring possible pharmaceutical differences as a function of the nature of the polymers used. The results obtained show a similarity of behaviour for the Estreva gels made with carbomers Carbopol 1342 (C 1342) and 1382 (C 1382). The reference gel, commercially available in France and made with the carbomer Carbopol 934 (C 934) is different in terms of its rheological criteria.

Psychotherapy for the personality disorders: working with traits.

Empirical research confirms clinical impressions that personality disorders are difficult to treat in psychotherapy. Personality pathology interferes with the therapeutic process, largely due to problems in alliance formation. Future models of treatment need to be built on a broader theory of the etiology of the personality disorders. These conditions are best understood as pathological amplifications of normal personality traits. The temporal stability of personality disorders can be accounted for by the genetic influence on these traits. Treatment involves helping patients to develop more adaptive ways of using their underlying traits.

Predictive value of Doppler umbilical artery velocimetry in a low risk population with normal fetal biometry. A prospective study of 2016 women.

OBJECTIVE: To assess the predictive value of Doppler umbilical artery velocimetry in a low-risk population with normal fetal biometry. STUDY DESIGN: Multicenter prospective study in 17 hospitals with prenatal clinics in France. Two thousand sixteen women who, before 28 weeks gestation were defined as at low risk after routine consultation and after ultrasound. Doppler umbilical artery velocimetry was performed between 28 and 34 weeks gestation. Confounding factors were used to perform multivariate regression. RESULTS: 1903 cases were analysed and 192 (10.1%) had an abnormal Doppler Resistance Index (RI). The abnormal Doppler group contained a significantly higher frequency of severe and moderate small for gestational age infants (SGA), both severe and moderate with a sensitivity of 25.5 and 18.8% respectively. There was no difference in hypertensive disorders or criteria of fetal distress. Mean birth weight was very significantly lower in the abnormal group (162 g). Birth weight was very significantly linked to RI after taking into account confounding variables in the multiple linear regression model (continuous relationship). After multiple logistic regression, the odds ratio associated with an abnormal Doppler result, adjusted for all the confounding factors, was 2.3 (95% CI 1.5-3.7) for moderate SGA and 3.5 (95% CI of 1.8-7.1) for severe SGA. CONCLUSION: Low umbilical Doppler RI is predictive with moderate or severe SGA in a low-risk population with normal fetal biometry, even when the information generally available in clinical practice and ultrasound parameters are taken into account. There is a continuous relationship between RI and birthweight. This predictive value cannot, however, lead to an improvement in neonatal health unless effective measures to prevent SGA exist and umbilical Doppler should not be used in low-risk population on a routine basis.

Influence of estrogenic status on the lipolytic activity of parametrial adipose tissue in vivo: an in situ microdialysis study.

Ovarian hormones have been shown to modulate the metabolism of adipose cells obtained from adipose tissue of different animals. The aim of this study was to better understand the short- and long-term influences of estrogens on the in vivo lipolytic response of rat parametrial fat pads, determined by measurement of extracellular glycerol concentrations using in situ microdialysis. Possible direct effects of estrogens on lipolysis were studied by perfusion of a potent estrogenic analogue such as moxestrol. Moxestrol (10(-6) M) failed to increase glycerol concentrations in estrus, diestrus, or 8-day ovariectomized animals. However, the basal glycerol concentrations and the lipolytic responses stimulated by 10(-6) M isoproterenol were decreased in parametrial fat pads of diestrus, compared with estrus, rats. Greater decreases in basal and stimulated glycerol concentrations were observed in rats that had been ovariectomized for 8, 15, or 30 days. In ovariectomized rats, isoproterenol-induced lipolysis was restored to the levels observed in diestrus animals by a daily injection of 17 beta-estradiol for a period of 7 days. These results implicate estrogens as long-term modulators of in vivo basal and stimulated lipolytic responses of rat parametrial fat pad.

Childhood trauma as an etiological factor in the personality disorders.

This article critically reviews the problems concerning the relationship between traumatic events in childhood and personality disorders in adulthood. Attributing adult psychopathology primarily to environmental factors is problematic. Personality traits are heritable. Children are resilient to the long-term effects of trauma. All studies of trauma in personality disordered patients suffer from retrospective designs. Only a minority of patients with severe personality disorders report severe childhood trauma. The effects of trauma in the personality disorders can be better understood in the context of gene-environment interactions.

Increases in the effects of albendazole on Echinococcus multilocularis metacestodes by the dipeptide methyl ester (Phe-Phe-OMe).

Three months after infection with Echinococcus multilocularis, Mongolian gerbils were given either the dipeptide methyl ester (Phe-Phe-OMe) or a combination of Phe-Phe-OMe plus albendazole to treat alveolar echinococcosis. Each drug was given orally at the daily dose of 50 mg/kg of body weight following various administration regimens. Histologic and ultrastructural studies of parasites recovered from infected gerbil tissues showed that the dipeptide methyl ester increases the effect of albendazole.

[Clinical value of umbilical Doppler. Results of controlled trials in high risk and low risk populations].

OBJECTIVE: In order to assess the clinical utility of the use of arterial umbilical Doppler velocimetry, we performed meta-analysis in high and low-risk pregnancies with randomized controlled trials. We included in the low-risk meta-analysis the data of the french multicentric study submit to publication. STUDY DESIGN: Exhaustive review of published and non published trials was performed; quality of trials was assessed before inclusion in meta-analysis. Mantel-Haenzel statistic method was used. We used odds ratio with 95% confidence interval (CI) to present results. RESULTS: Meta-analysis in the high-risk population performed with 13 trials (n = 9162) shows a 36% significant reduction in perinatal-deaths in the Doppler group. The reduction in deaths was also observed in perinatal deaths after exclusion of lethal malformation (OR = 0.62, CI:[0.45; 0.86]) and in stillbirths (OR = 0.54, CI:[0.29; 0.91]). There is no difference in maternal, obstetrical and other perinatal outcomes. Meta-analysis in the low-risk population performed with 4 trials (n = 11451) does not show any benefit in the Doppler group, in particular in perinatal deaths (OR = 0.90, Cl:[0.50; 1.60]). CONCLUSION: Use of umbilical Doppler velocimetry in high-risk pregnancies involve a one third reduction in perinatal mortality. This proved effect should induce obstetricians to performed systematically an umbilical Doppler in pregnancies "at fetal risk". On the other hand, there is no indication to perform an umbilical Doppler in an unselected or a low-risk population.

[Success and failure in the treatment of patients with borderline personality disorder].

This paper reviews the main treatment options for patients with borderline personality disorder. Pharmacological interventions are of marginal value, and hospitalization has not been shown to prevent suicide in this population. Psychodynamic psychotherapy has not been proven to be effective, but dialectical behavior therapy yields symptomatic improvement. Borderline patients present many special difficulties in therapy, particularly their chronic suicidality. Future developments in treatment could involve improved drug treatment combined with cognitive therapy.

Effect of nomegestrol acetate on estrone-sulfatase and 17beta-hydroxysteroid dehydrogenase activities in human breast cancer cells.

It is well recognized that estradiol (E2) is one of the most important hormones supporting the growth and evolution of breast cancer. Consequently, to block this hormone before it enters the cancer cell or in the cell itself, has been one of the main targets in recent years. In the present study we explored the effect of the progestin, nomegestrol acetate, on the estrone sulfatase and 17beta-hydroxysteroid dehydrogenase (17beta-HSD) activities of MCF-7 and T-47D human breast cancer cells. Using physiological doses of estrone sulfate (E1S: 5 x 10(-9)M), nomegestrol acetate blocked very significantly the conversion of E1S to E2. In the MCF-7 cells, using concentrations of 5 x 10(-6)M and 5 x 10(-5) M of nomegestrol acetate, the decrease of E1S to E2 was, respectively, -43% and -77%. The values were, respectively, -60% and -71% for the T-47D cells. Using E1S at 2 x 10(-6) M and nomegestrol acetate at 10(-5) M, a direct inhibitory effect on the enzyme of -36% and -18% was obtained with the cell homogenate of the MCF-7 and T-47D cells, respectively. In another series of studies, it was observed that after 24 h incubation of a physiological concentration of estrone (E1: 5 x 10(-9)M) this estrogen is converted in a great proportion to E2. Nomegestrol acetate inhibits this transformation by -35% and -85% at 5 x 10(-7)M and 5 x 10(-5)M, respectively in T-47D cells; whereas in the MCF-7 cells the inhibitory effect is only significant, -48%, at 5 x 10(-5)M concentration of nomegestrol acetate. It is concluded that nomegestrol acetate in the hormone-dependent MCF-7 and T-47D breast cancer cells significantly inhibits the estrone sulfatase and 17beta-HSD activities which converts E1S to the biologically active estrogen estradiol. This inhibition provoked by this progestin on the enzymes involved in the biosynthesis of E2 can open new clinical possibilities in breast cancer therapy.

Cultural factors in the emergence of borderline pathology.

Most of the research and clinical reports concerning borderline personality disorder (BPD) come from highly developed countries. Although BPD can also be diagnosed in developing societies, it is likely that this form of pathology is more prevalent in North America and Europe. However, the personality traits and psychological risk factors underlying borderline personality may also be found in individuals from developing countries. The hypothesis of this paper is that social protective factors suppress the development of these traits into diagnosable personality disorders. This process is illustrated by cases in which borderline pathology emerged only after immigration.

Risk factors for borderline pathology in children.

OBJECTIVE: To examine risk factors associated with borderline pathology in latency-age children. METHOD: The subjects were 98 children assessed for day treatment. Borderline subjects were identified in a chart review using the Child Diagnostic Interview for Borderlines, which divided the sample into borderline (n = 41) and nonborderline (n = 57) groups. Functional levels were assessed by Children's Global Assessment Scale scores. The risk factors were also rated by chart review; all subjects were given a cumulative abuse score and a cumulative parental dysfunction score. RESULTS: Both groups demonstrated severe functional impairment. The risk factors that differentiated the borderline group were sexual abuse, physical abuse, severe neglect, and parental substance abuse or criminality. Sexual abuse and severe neglect were significant in multivariate analysis. Cumulative abuse and cumulative parental dysfunction scores were both higher in the borderline group. CONCLUSIONS: The findings indicate that the risk factors in borderline children are similar to those found in adults.

Defense styles, hostility, and psychological risk factors in male patients with personality disorders.

The purpose of this study was to compare ratings of defense styles and hostility in male patients with personality disorders, to determine whether either of these is related to a borderline diagnosis, or to specific psychological risk factors. Subjects with personality disorders were divided into borderline personality disorder (BPD) (N = 61) and non-BPD (N = 60) groups were given both the Defense Style Questionnaire (DSQ) and the Buss-Durkee Guilt-Hostility Inventory (BDHI). Risk factors were measured by a developmental interview and the the Parental Bonding Index. The BPD group reported more maladaptive and image distorting defenses on the DSQ, as well as higher ratings on the BDHI. Ratings on both dependent measures were strongly related to diagnosis, and more weakly to measures of psychological risk factors. Thus, in men with personality disorders, immature defense styles and high levels of hostility are strongly associated with a borderline diagnosis.

Antisocial personality disorder: a biopsychosocial model.

OBJECTIVE: To propose an etiological model of antisocial personality disorder that is grounded in empirical data. METHOD: Recent research findings are reviewed that clarify our understanding of the etiology and course of antisocial pathology. RESULTS: Neither biological nor psychological factors fully account for the development of this disorder. Epidemiological studies show that there are strong cross-cultural differences in its prevalence, pointing to the importance of social factors in its etiology. Outcome research shows that antisocial personality only partially remits with time, and that most patients continue to be dysfunctional in later life. No treatment modality has been shown to be effective. CONCLUSIONS: All these empirical findings can be accounted for by a biopsychosocial model of antisocial personality disorder.
