Effect of a luteinizing hormone-releasing hormone agonist (Buserelin) on steroidogenesis of cultured human preovulatory granulosa cells.

In order to ascertain whether there is a direct effect of luteinizing hormone-releasing hormone (LH-RH) agonists on ovarian steroidogenesis, human preovulatory granulosa cells were cultured in the presence of an LH-RH agonist (Buserelin, Hoechst Roussel Pharmaceuticals, Paris, France). Cultures were performed without serum but with precursors of estrogens and progestins. In basal conditions, Buserelin, at 1 ng/ml, increased estradiol and progesterone (P) secretion, while at concentrations of 10 and 100 ng/ml, no effect was observed. In the presence of follicle-stimulating hormone (FSH) or LH, Buserelin at 1 and 10 ng/ml did not modify the cell's steroidogenesis. However, at a concentration of 100 ng/ml, Buserelin decreased LH stimulation of P secretion. These results suggest that LH-RH agonists, at concentrations in the range of those obtained in clinical use, can modulate ovarian steroidogenesis by direct action.

Randomized trial of two media used for in vitro fertilization.

Culture media are important components of IVF. Their selection must meet the need for efficiency, and also economic and practical requirements. From these standpoints, we compared two widely used media: Ham's F10 supplemented with cord serum and Menezo B2 used without serum. The test followed a randomized protocol using two series of 159 and 162 oocytes. Since no difference was seen in efficiency assessed from cleavage rate, the discussion focuses on cost and ease of use.

Choice of an ovarian stimulation protocol according to the follicular puncture method in an in vitro fertilization programme.

In order to ascertain the adequacy of ovarian stimulation protocols with a type of follicular puncture, 126 women undergoing in vitro fertilization received either combination clomiphene/hMG or hMG alone according to a randomized test protocol. Within both groups patients for whom a pelvic examination was required had laparoscopies, while others had transvaginal ultrasonically guided punctures as far as possible. Clomiphene/hMG was more efficient than hMG alone as assessed from the cleavage rate (68% vs. 54%; p less than 0.01) and the pregnancy per attempt rate (16% vs. 5%; p less than 0.05). Laparoscopic punctures were more efficient than ultrasonically guided punctures (mean number of recovered oocytes: 4.8 +/- 2.6 vs. 3 +/- 2.5; p less than 0.001), but slightly better results were achieved by this latter method in ongoing pregnancy per puncture rate (18% vs. 8%; NS). With ultrasonically guided punctures, stimulation by clomiphene/hMG allowed better oocyte recoveries (3.8 +/- 2.5 vs. 2.3 +/- 1.9, p less than 0.05). Such results constitute an argument for preferential use of the clomiphene/hMG stimulation protocol with ultrasonically guided punctures.

High density lipoprotein and low density lipoprotein utilization by human granulosa cells for progesterone synthesis in serum-free culture: respective contributions of free and esterified cholesterol.

Human preovulatory granulosa cells cultured in serum- and gonadotropin-free medium secreted progressively less progesterone as time elapsed. Addition of purified high density lipoproteins (HDL) as well as low density lipoproteins [very low density (VLDL) plus low density lipoproteins (LDL)] restored optimal synthesis of progesterone, and HDL was as effective as VLDL + LDL. The use of cholesterol doubly labeled lipoproteins allowed calculation of the proportions of free and esterified cholesterol converted into progesterone. Granulosa cells used either free or esterified cholesterol from VLDL + LDL. In contrast, HDL-esterified cholesterol was a poor substrate for progesterone synthesis, while HDL-free cholesterol was used preferentially. LH increased the use of both kinds of lipoproteins without changing the way in which they were used. Pretreatment of HDL by purified phospholipase A2 increased the conversion of free cholesterol into progesterone. Similar treatment of VLDL + LDL had little effect on progesterone secretion. We conclude that HDL as well as VLDL + LDL can provide cholesterol to human preovulatory granulosa cells and that utilization of HDL-cholesterol may depend on gonadotropin (LH) and enzymatic (phospholipase) regulation.

[Evaluation of the economic burden in the respective indications of in vitro fertilization and tubal microsurgery at a regional hospital center].

The authors have calculated the cost of obtaining and maintaining an intrauterine pregnancy to term by IVF and by tubal surgery according to the four classical indications. They have worked this out from local experience. The indications are: reversal of sterilization, surgery on proximal lesions, surgery on distal lesions and multifocal lesions. IVF is superior to surgery where the lesions are multifocal and possibly also in proximal blocks. But surgery is indicated in a large number of distal blocks and particularly in reversal of sterilization. The authors point out how difficult it is to extrapolate from national or international statistics. The choice must depend particularly on the experience of the local teams.

[Triploidy].

Using their own experience which they described in 1984 and reviewing as well the literature of the condition since that time has led the authors to suggest that triploidy should be considered as a pathological entity with three clinical forms: early abortion, mid-trimester termination and the birth of triploidy fetuses either alive or dead. They discuss the clinical features, the ultrasound appearances, the pathological and anatomical details and the cytogenetics of triploidy. They differentiate this condition from trophoblast disease and from the central hydropic conditions which occur with normal caryotypes. They emphasize that there is a difference in the way classical molar pregnancy evolves and repeat again that the term "embryonic mole" should be avoided.

IgG subclasses and Gm allotypes of anti-D antibodies during pregnancy: correlation with the gravity of the fetal disease.

The IgG subclasses and the Gm allotypes of anti-D antibodies were studied in 103 isoimmunized pregnancies with Rh-positive infants. No allelic exclusion phenomena were observed in patients heterozygous for the studied genes. An analysis of the correlation between the severity of fetal disease and the type of anti-D antibodies shows a greater hemolytic action of the IgG1 subclass and, within this group, those marked by the Gm(4) allotype. These results suggest that the prognostic value of anti-D antibody titers during pregnancy could be modulated by a study of the IgG subclasses and their allotypes.

Lipoprotein and phospholipid distribution in human follicular fluids.

Human preovulatory follicular fluids, obtained in the course of stimulated cycles, were analyzed for their lipid and protein compositions. By combining different methods, a single class of lipoprotein, high-density lipoprotein, was detected in all cases. The phospholipid distribution revealed an accumulation of lysophosphatidylcholine. These data are discussed in view of the possible roles of the follicular fluid during fertilization and in relation to the high levels of estradiol and progesterone measured in those fluids.

A fluorimetric enzyme-linked immunoassay (FEIA) for measurement of anti-D antibodies.

A new enzyme-linked immunoassay for the determination of anti-erythrocyte antibodies is described. A fluorogenic substrate of alkaline phosphatase, the 4-MUP, was used to reveal the antigen-antibody reaction. This substrate was chosen in order to avoid interferences with haemoglobin, since it emits a light with a wavelength outside the absorption spectrum of haemoglobin. This method reaches a sensitivity in the order of 1 ng/ml. Sera from 66 Rh-immunized pregnant women and 20 control subjects have been studied. The specificity and reproducibility of the results and their good correlation with the biological and clinical parameters of the hemolytic disease of the newborn confirm the validity of the method, encouraging its routine use.

[Initial trials in the treatment of feto-maternal alloimmunization in the Rhesus system with oral administration of erythrocyte antigens].

A group of 14 pregnant women with severe Rh-iso-immunization were treated by the administration of oral antigens (stromas). The aim of the treatment was to try to induce a modification of the immune response of these patients in order to minimize the fetal or neonatal consequences of their Rh-iso-immunization. The progress and outcome of the treated pregnancies are compared with those of a group of women presenting an Rh-iso-immunization of comparable severity treated according to standard methods. Stroma therapy is found to be without any danger for the mother or fetus. Even though there are 9 surviving infants in the treatment group and only 5 in the control group, the analysis of the results does not show any statistically significant difference in the two series. The results of this preliminary study would seem to justify the creation of a therapeutic trial.

Cytochemical profile of peripheric blood lymphocytes in 7 to 27 weeks' pregnancy.

A comparative study of alpha-naphthyl (nonspecific) esterase (ANAE), acid phosphatase (AcP), and beta-glucuronidase (beta-GLUS) in the blood lymphocytes was carried out on 26 healthy pregnant women (7 to 27 weeks' amenorrhea) and on 32 normal nonpregnant women, all 58 of whom were 19 to 34 years old. In pregnant women, we found a significantly lower percentage of ANAE-positive lymphocytes (after 11 weeks' amenorrhea), a strong reduction in AcP, and a slight decrease in beta-GLUS. The modified enzyme levels of circulating lymphocytes in the early stages of pregnancy seem to be related to the depressed cell-mediated immunity.

[Selection and characterization of human acrosome-reacted sperm: application to subzonal insemination].

Acrosome reacted spermatozoa were selected with immunobeads coated with an antibody directed against the inner acrosomal membrane (GB24) and then detached immunologically using an anti-Fab'2 antibody. Using this method, we have shown that acrosome reaction occurs in the most morphologically normal spermatozoa and is followed by a loss in motility and a decrease in longevity. Moreover, when injected in the perivitelline space of hamster oocytes, the selected spermatozoa had a higher fertilization rate than unselected spermatozoa, confirming that acrosome reaction is necessary for the fusion with the oocyte plasma membrane and that the selection method does not alter the fertilizing ability of the spermatozoa. Therefore, this method represents a new way for understanding the sperm functions.

[Microinjection in France: report of the BLEFCO multicentric study].

In order to evaluate the assisted fertilizations in France, BLEFCO association has analysed 436 and 1,781 attempts for SUZI and ICSI respectively. For SUZI, the transfer rate was 62% and the pregnancy per attempt rate was 10%. For ICSI, whatever indications, the transfer rate was 86% and the pregnancy per attempt rate was 21%. ICSI in cases of altered sperm without previous IVF represented 46% of ICSI attempts. No relationship between the transfer rate and the indication of ICSI has been observed. However, in case of unexplained fertilization failures, the pregnancy per attempt rate is significantly lower when compared to the other indications.
