A survey of treatment outcomes with removable partial dentures.

The patient's satisfaction with removable partial denture (RPD) therapy has become an increasingly important factor in prosthetic treatment. This study examined patients' satisfaction with RPDs in relation to some socio-economic variables, patients' habits of wearing and cleaning RPDs, comfort of wearing RPDs and different RPDs characteristics. A questionnaire was devised for the purpose. Two hundred and five patients were required to assess satisfaction with RPDs. They graded RPDs, depending on the level of satisfaction, on scale ranging from 1 to 5. A dentist determined Kennedy classification, material and denture support, denture base shape, number of missing teeth and evaluated denture construction. Majority of the patients were satisfied with the prosthesis. The patients of a higher education level gave lower grades (P < 0.05) to aesthetics of maxillary RPDs. Almost half of the patients were wearing RPDs during the day. Most of the patients cleaned RPDs three times a day. A significant difference was found between the patients' grades for comfort of wearing mandibular RPDs and number of missing teeth and between hygiene of mandibular RPDs and habits of cleaning them. Majority of the patients treated with RPDs were satisfied with the prosthesis. Dissatisfaction was related to mastication, esthetics, number of missing teeth and maintenance of oral hygiene.

Bite force in subjects with complete dentition.

Bite force is the condition, expression and measure of the masticatory function. The purpose of this study was to examine, by means of a newly constructed electronic gnathodynamometer, the values of maximal bite forces in subjects with complete dentition, the time in which they express 50% and 75% respectively of the total forces value, and the shape of the bite curve during testing. The obtained data was statistically analyzed with respect to gender and age. Analysis of the variance confirmed the finding that there was no statistically significant correlation between the values of forces and subjects' age, but there was a statistically significant difference between males and females in the values of the bite forces in the front segment, as well as between the values of the force on anterior and posterior teeth. The correlation between the time T1 posterior right and T1 posterior left, and between T1 and T2 for anterior teeth are statistically significant. Analysis of the bite curves suggests that males "bite" shorter than females with a sharper peak of the curve. Numerical values and bite curves should be a diagnostic factor in the further follow-up of subjects or in the choice of prosthodontic therapy.

Morphometric characteristics of toothless lower jaw ridge as a bed for total lower jaw prosthesis.

Analysis was made of 44 solid casts of toothless lower jaws. Stereophotogrammetry, a measurement procedure in photogrammetry, was used as a method in the measurement of solid casts. Adequate instruments and method of measurement made it possible to objectify anatomical and morphological structures of toothless lower jaws. The anterior width of the ridge in the region of canines was 22.70-35.50 mm. The posterior width in the first molars region was 43.30-59.10 mm, with statistically significant difference between the genders. The width at the level of posterior ridge of mandibular tubercle was 52.10-70.00 mm, with statistically significant difference between the genders. The length of the alveolar ridge between the symphysis and the tangent line of posterior ridge of mandibular tubercle in the medial line was 34.70-49.00 mm, without statistically significant difference between the genders. The surface of the toothless ridge is at the same time the surface of the total lower prosthesis bed. The size of the surface depended on the resorption and relation between the alveolar ridge and the surrounding active structures; it ranged between 103.21-205.50 mm2. The mean value being 145.99 mm2. The values obtained in our study are in approximate accordance with the reference literature data. They are the results of accurate measurements of solid casts.

Quantitative thin-layer chromatographic identification of dental base alloys.

In the corrosively aggressive medium of the oral cavity, the use of identical dental alloys requires identification of the existing metal construction. One of the methods allowing this identification is quantitative thin-layer chromatography with anodic sampling. Using a 4.5-V battery and suitable electrolytes, seven dental alloys for fixed and removable dentures based on cobalt were analysed. Chromatograms of alloy samples were developed with a mixture of acetone and 2 M HCl. Scanning of the thin-layer chromatographic spots produced chromatographic curves and the area under the curve was proportional to the content of cobalt in the alloy studied. Regression analysis showed a very high coefficient of correlation (r=0.999) between the area of the spot and the proportion of cobalt.

The simulation and calculation of the fatigue of the lower complete denture in function by means of the finite element analysis.

This paper presents a finite element analysis, by which the stress distribution during function of a lower jaw acrylic-resin complete denture can be shown. The geometry of a prosthesis has been approximated by measuring, its elementary dimensions, as well as the dimensions of the characteristic cross-sections, on an already completed denture. Based upon these dimensions a mesh, (a three-dimensional network diagram) of finite elements was made. The results obtained are presented both graphically and numerically. The equivalent fatigue stresses, according to von Mises' energetic theory of firmness, expressed in MPa (or in N/mm2), are also shown. The results obtained enable us to gain an insight into lower complete denture fatigue, and could also be used when optimizing the shape of a denture. Furthermore, they could indicate possible causes of the fatigue fractures of a denture, particularly in the area between the left and the right canines.

Some anatomical and anthropological measures of mandibular ramus in our population.

Beside the already analysed bioanthropometric characteristics of the lower jaw, additional measurement of parameters insufficiently discussed in reference literature were performed in our study. On the basis of the obtained data it was possible to define relevant biometrical relations in the region of the lower jaw branch and anatomical regions that are especially important to the use of conduction anaesthesia. All measurements were defined on the basis of anteroposterior (horizontal) and vertical directions. The relation between different anatomical structures of the lower jaw interior branch and foramen mandibulae served as a focal point. When related to anteroposterior plane the mandibular foramen was located precisely in the middle of the distance between crista temporalis and posterior ramus ridge. Viewed in vertical direction the lowest point of mandibular opening was slightly closer to the mandibular angle than to incisure of mandible, meaning that in the majority of macerated skulls mandibular foramen had a relatively low position.

[Comparison of location between nasion and orbital points on soft tissue and on bony structures, in articulatory technique].

A skin projection of the orbitale as palpated through soft tissues was designated on the left cheek of 22 subjects. The nasion, orbitale and cephalostat nasion direction medium were plotted on transparent paper after teleroentgenography. Mutually parallel lines were drawn through the points, the line obtained on the construction of the nasion direction medium used as a basic one. Distances between the lines were measured and results statistically processed. Transfer of the model into the articulatory region by means of the orbitale and buccal arch nasion direction appeared not to orientate reliably the model according to reference planes of the head. Our results are consistent with literature data available.

[Evaluation of the reconstruction of occlusal plane by use of Ponans angle].

In this study, teleroentgenocephalograms of 34 subjects with at least 20 natural teeth were used. Correlation between the angle formed by linking the PORION-NASION-SPINA NASALIS ANTERIOR (PONANS angle) points and the angle made by Frankfort horizontal and occlusion plane was studied. The coefficient of correlation was 0.1263. It is considered that there is no correlation if the coefficient of correlation is less than 0.5. Thus, there was no correlation in the study sample, which is consistent with the results reported by Karkazis and Polyzois, but in disagreement with those published by Monteith. In our sample, the occlusion plane could not be oriented in the articulatory space or in toothless mouth by means of the method proposed by Monteith.

[Glass-ionomer cements].

Glass-ionomer cements represent a new type of dental materials widely clinically applied owing to their good adhesion to dental tissue, caries-protective effects, biocompatibility and simple handling. They are two-component materials commercially available in forms of powder or liquid. The two components are mixed manually or automatically in a vibrator to produce a compact mixture. In addition to chemical, light-polymerizing hybrid glass-ionomer cements are also available. A description is given of physical and chemical features of these materials, their classification and use in particular clinical cases.

[Positions of the central relation and central occlusion as recorded by a functiograph in patients with a stomatognathic system dysfunction].

Positions of the central relation and central occlusion were intraorally graphically recorded in 100 patients with a stomatognathic system dysfunction and in 0 control subjects. Results of the study showed asymmetry of these two positions to play a major role in the etiology of dysfunction. Occlusal program had an almost two-fold impact on the graphic tracing of the central occlusion position, as compared to the influence exerted by the articulation and muscle on the central relation tracing. Instrumental functional analysis and the instrument used alike, are simple to use and allow an objective assessment of the state to be made.
