Treatment of hepatic hydatid disease with mebedazole: preliminary results in four cases.

Mebendazole was given to four patients with hepatic hydatid disease. In three patients hydatidosis had remained after surgery, and in the fourth it could not be treated surgically. Mebendazole was given orally in maximum doses of 400-600 mg three times a day during courses lasting 21 to 30 days. Ultrasonic echotomography showed a complete regression of the intrahepatic cysts after four to 13 months in all four cases. In three patients the course of treatment had to be repeated. Mebendazole also induced clinical improvement and a progressive lowering of the concentration of specific IgE of Echinococcus granulosus. During treatment circulating blood levels of specific immune complexes of antigen 5 were increased. These observations indicate that mebendazole has a lethal effect on E granulosus cysts in primary hydatid disease in man and that the efficacy of chemotherapy can be assessed with ultrasonography and by measuring changes in the concentration of specific IgE of E granulosus and circulating immune complexes.

Histological study of the materno-embryonic interface in spontaneous abortion.

In a histological study of 184 specimens of complete spontaneous abortion, the following points were delineated. In cases of anomaly or death of the conceptus, there was a reduced trophoblastic penetration into the decidua and into the spiral arteries where physiological changes were limited or absent. Trophoblastic proliferation within the columns and the outer shell was limited with frequent disruption or even disappearance of the shell. These findings seem to be related to the untimely initiation of blood flow in the intervillous space which in turn is associated with arrest of pregnancy and eventual expulsion.

[Uteroplacental vascularization].

The intervillous space is considered as receiving the most important part of the uterine flow input (> 90%), reaching the placenta through the modified spiral arteries network. The low vascular resistance detected on the uterine arteries is thought to be due to the presence of the placental shunt. Using a 3D Colour Doppler technology (ATL 5000), a complex anastomotic and intra-myometrial network has been detected. This vascular network is always detected during the first 48 hours of after delivery. During this time, the placenta has been removed, the uterine muscle is contracted, a low resistance of the uterine flow is systematically detected. A extraplacental vascular component must be considered as taking a functional part in the foeto-maternal exchanges.

[Ultrasonography investigation of mammary calcifications (author's transl)].

Ultrasonography recordings in 140 patients with breast cancer, 119 with cystic mastopathy, and 299 with benign mammary tumours showed that calcifications are rarely demonstrated, and that diagnosis is not affected to any degree when they are present. Ultrasonography should be used for examination of the breast following the three traditional clinical, radiological, and cytological investigations. False negatives (7.9% of cancers) and false positives (4.2% of fibro-adenomas) are easily corrected from radiological and cytological findings which are still essential for the diagnosis of most mammary affections. Ultrasonography, however, enables an appreciable reduction in biopsy requests to be made in less than 6% of cases of atypical microcalcification in benign mammary lesions.
