Biodegradable p(DLLA-epsilon-CL) nerve guides versus autologous nerve grafts: electromyographic and video analysis.

The aim of this study was to evaluate the functional effects of bridging a gap in the sciatic nerve of the rat with either a biodegradable copolymer of DL-lactide and epsilon-caprolactone [p(DLLA-epsilon-CL)] nerve guide or an autologous nerve graft. Electromyograms (EMGs) of the gastrocnemius (GC) and tibialis anterior (TA) muscles were recorded 3.5 and 5 months after bridging the nerve gaps. Furthermore, the rats' gait was recorded on video and the quality of the gait was analyzed. EMG patterns of the contralateral nonoperated side were essentially normal. The EMG patterns on the operated side were irregular in all animals, but the quality of gait was better in the nerve guide group. We conclude that the surgical technique (nerve guide or nerve graft) does not influence the occurrence of abnormal EMG patterns, but gait improves to a greater extent when the nerve gap is bridged by a nerve guide.

Brachial plexus neuropathy. A long-term outcome study.

This retrospective study assessed the long-term outcome of brachial plexus neuropathy in 16 patients. The mean follow up was 8 years. Nine patients complained of persistent pain and muscle weakness, four had continuing problems with various activities of daily living and 11 had trouble with some housekeeping activities. Furthermore, eight of the patients had to change their occupation.

Bilateral eyelid edema: cutis laxa or blepharochalasis?

A 59-year-old woman with massive bilateral edema of the upper and lower eyelids is presented. The edema occurred suddenly and without provocation. No cause could be identified despite a multitude of examinations. Initially the patient was diagnosed as having blepharochalasis, but later skin biopsy suggested an acquired form of cutis laxa. Most likely the cutis laxa was a result of angio-edema. Pharmacological treatment was not successful, but surgical intervention provided an acceptable result. The differential diagnosis of periorbital edema is discussed with special attention to cutis laxa, angio-edema, and blepharochalasis.

Ectopic bone formation after temporal muscle transposition for facial paralysis.

This report describes the case of a patient with congenital bilateral facial paralysis in whom ectopic bone formation developed following temporal muscle transposition. Ectopic bone formation was first noticed 4 years after surgery. Whether the ossification is a result of the transfer of periosteum or the osteogenic capacity of muscular tissue is still unknown.

[Risks of silicone breast implants unproven].

The moratorium in the USA on inserting breast implants filled with silicone gel caused global alarm by inaccurate media reports resulting from settlements and verdicts against the manufacturers. In fact, there still is no scientific evidence that the implants have negative effects on health. On the contrary, there is an increasing number of studies that were unable to prove a link between the implants and diseases, specifically cancer, autoimmune disease or connective tissue disease. The European organization of plastic surgeons again stressed this point in June 1998. In the Netherlands it has been obligatory since June 1998 for all medical implants to carry a quality mark. In addition, it is the intention that data on silicone breast implants will be recorded in a national registration.

[Paget's disease of the scrotum].

Paget's disease was diagnosed in a scrotal biopsy specimen of a male aged 70 who for three years had had a red scaly skin lesion of the penoscrotal borderline area. Eight local excisions failed to achieve radical removal of the tumour. Before the 9th excision, the skin was pretreated with fluorouracil cream for 10 days. Ultimately, the lesion could be removed radically. Extramammary Paget's disease is a rare condition. There is no consensus on the strategy to be adopted. Surgical treatment appears indicated.
