Pulmonary infection and rheumatoid arthritis.

Five of eleven patients with bronchiectasis and/or cystic fibrosis developed a polyarthritis with positive tests for rheumatoid factor. Possible mechanisms of this complication are discussed.

Clinical and laboratory studies of levamisole in patients with rheumatoid arthritis.

Two studies with levamisole in rheumatoid arthritis are reported. In the first study of 30 patients levamisole was superior to placebo in terms of pain relief, reduction in articular tenderness, duration of morning stiffness, erythrocyte sedimentation rate (ESR), and radioisotope uptake in knee and wrist joints. There were no consistent differences in lymphocyte function, immunoglobin or complement concentration, or polymorphonuclear granulocytic function. In the second study forty patients were commenced on treatment with either gold or levamisole. At the end of one year there were significant improvements with both regimens in pain score, joint tenderness and ESR. The patients on levamisole showed significant improvement of duration of morning stiffness, while the patients on gold showed significant improvement of rheumatoid factor titre and left hand grip, but there were no significant differences between the two regimens.

Assessment of insulin sensitivity in vivo in control and diabetic mice with a radioactive tracer of glucose transport: [125I]-6-deoxy-6-iodo-D-glucose.

BACKGROUND: Impairment of insulin-stimulated glucose transport is a characteristic of type 2 diabetes. A radioactive glucose analogue has been synthesized: [(125)I]-6-deoxy-6-iodo-D-glucose. Its biological behaviour in vitro is similar to that of 3-O-methyl-D-glucose, the reference tracer of glucose transport. The aim of the present study was to determine the ability of [(125)I]-6-deoxy-6-iodo-D-glucose to evaluate variations in glucose transport in vivo. METHODS: Biodistributions of [(125)I]-6-deoxy-6-iodo-D-glucose were performed with or without exogenous insulin (iv injection of 1.5 IU/kg) in db/+ non-diabetic control mice and in db/db type 2 diabetic mice, exhibiting a severe insulin resistance characterized by a lack of increase in glucose uptake in response to insulin. RESULTS: In db/+ mice, insulin increased [(125)I]-6-deoxy-6-iodo-D-glucose transport by 30% in most insulin-sensitive tissues (heart, diaphragm and skeletal muscle, p < 0.05) and had no effect in other organs. In db/db mice, [(125)I]-6-deoxy-6-iodo-D-glucose transport in these organs was not modified by insulin. CONCLUSION: [(125)I]-6-deoxy-6-iodo-D-glucose is able to trace in vivo an increase in glucose transport with insulin in non-diabetic mice and a defect of glucose transport in type 2 diabetic mice. It is the first time that an iodinated analogue of glucose has shown such promising results after in vivo injection. The use of this tracer to assess glucose transport in vivo in humans via nuclear imaging warrants further investigation.

Slow onset anti-rheumatic drugs in rheumatoid arthritis: could the presence of antinuclear antibody influence the therapeutic results?

The therapeutic value of sodium aurothiomalate, D-penicillamine and levamisole was evaluated in three comparable groups of 20 rheumatoid arthritis (RA) patients. There was no intergroup difference after a 3-month follow-up and all were significantly improved (p less than 0.001). To verify if, in these patients, the presence of antinuclear antibodies (ANA) had influenced the therapeutic results, each group was retrospectively subdivided according to the ANA status. To start with, all the sub-groups were statistically comparable on the basis of measures of disease activity. The D-penicillamine group could not be analyzed. In the gold treated group, the ANA negative patients were more improved than the ANA positive (p = 0.03), and very significantly more than the levamisole-treated ANA-negative patients (p = 0.005). The ANA negative patients taking levamisole had less pain relief (p = 0.01) and showed a tendency for less overall improvement (p = 0.15) than the ANA positive patients. This preliminary study supports the idea that systematic ANA testing in RA may be of practical and theoretical value.

Computerized system for the selection of the most HLA-compatible kidney recipient (France-Transplant).

The RITRAN program is applied to renal transplantation and operates within the framework of France Transplant. It makes it possible to choose the most compatible kidney recipient within the ABO blood group system and within the major histocompatibility system in man (HLA). The file presently comprises the data stored on tape concerning 1800 dialysis patients who are awaiting grafts. It is managed by UNIVAC 1108 computer. More than 25 medical-surgical transplant teams from France, Switzerland, Holland and Spain are connected with the computer. It was five years ago that the first interrogation system began operating in real time 20 hours out of 24, using a telex line, and it continues to provide complete satisfaction. The updating of the data is done by the RITREC system, also using a telex machine. An adaptation of the programs applied for choosing blood and platelet donors is in the process of being realized.

[Diastematomyelia. Study of 69 cases (author's transl)].

We wish to share our experience in the study of 69 cases of diastematomyelia in children. Our definition is very restrictive. There is an important female predominance noted (3.6:1) Patients usually consult for orthopedic problems. Skin lesions often associated with this condition, lead to radiological investigation, especially if there are also orthopedic and/or neurological problems. Neurological and sphingteral impairments were present mainly in our elderly patients. Myelogram with a liposoluble substance has been helpful in allowing us to propose surgery in most cases. Post-surgery complications are minimized if synthetic materials are avoided for duroplasty. Post-operative results are satisfactory and we recommend surgery for preventive more than curative reasons.

[Current problems posed by arterial injuries in hospital practice].

The authors analyse a series of 100 arterial lesions in respect to the localization, the time elapsed between the accident and reconstructive surgery, and the number of associated lesions (two or more). The results confirm the prognostic importance of the factor time: 83% of the good results are obtained in the first twelve hours, and only 67.5% after twelve hours. The results seen with popliteal and tibial lesions show a steady improvement from a 50% amputation rate twenty years ago to a 21.6% rate at the present time. The presence of associated lesions is equally important in the outcome: isolated arterial lesions (or accompanied by no more than two other anomalies) are successfully treated in 91% of the patients, opposed to 50% when loco-regional lesions are found (bone, skin, veins, nerves, muscles). The optional treatment of these complex traumatisms should be by means of a multidisciplinary approach.
