[The vision of welders in France].

PURPOSE: A study was conducted to measure the impact of welding on the vision of welders. METHODOLOGY: This study was conducted in France by the occupational medicine staff of large companies on 1.131 people, namely 850 welders and 281 control subjects. This investigation included two examinations at the beginning and the end of a year. The investigative procedure examined the different welding processes, the percentage of working time spent on welding activity, the length of exposure in years, as well as the medical variables: the optical correction type and history of ocular traumatology. The Visiotest or the Ergovision were used for the visual examination, equipment in common use by occupational medicine departments. RESULTS: The welders were comparatively young (59.53% of them were less than 45 years old). Moreover, for 69.75% of the welders, more than 75% of their activity was devoted to welding. All currently used welding processes were represented, including the modern PLASMA-TIG welding process. No excessive blood alcohol levels were observed in all subjects, but welders did smoke slightly more than the control subjects (40% vs 33%). Self-medication was rather less frequent among the welders, except as regards the use of eye drops, where the proportions were clearly inverted. Optical correction for hyperopia was similar between the two groups; however, as regards myopia, the welders were corrected less often. Lastly, contact lens use was exceptional among the welders. Nearsightedness varied logically with age, but also, inexplicably, with the welding processes. Vision recovery time after exposure to glare was much longer among the welders, except for the PLASMA-TIG processes. No difference was observed in the other parameters of the study. No change in the visual functions studied was noted between the two examinations. DISCUSSION: The examination techniques used showed no impairment of the studied visual functions, probably because companies use protective and preventive eye care methods. Moreover this study is the first to examine the type of welding used by workers and particularly the modern PLASMA-TIG process. The vision recovery time after exposure to glare seems better for the PLASMA-TIG process may be the result of the lower luminous intensity of this process. CONCLUSION: This study was conducted for preventive purposes to contribute to better monitoring of safety and comfort for welding workers and has shown no alteration of the visual function among welders in general.

Aircrew fitness decisions and advances in refractive surgery techniques.

Refractive surgery is a well mastered technique for the correction of ametropia. The results in terms of recovery of visual acuity are generally very satisfactory. However, when patients are applicants for pilot training it is necessary to consider the structural effects of the operation and its repercussions on the visual function. Since 1981, we have examined 273 eyes of subjects applying to flight crew. The alterations, in the corneal structure, in the sensitivity to contrasts, and in the resistance to glare have dampened the former enthusiasm of the operated. We have evaluated the functional and clinical consequences of each individual case as well as the administrative decisions made concerning these applicants. The outcome is that on average one of three applicants obtained a dispensation within a time limit ranging from 1 to 5 years after surgery, according to the field applied for.

[Visual handicap, autonomy and third party compensation. Evaluation trials].

Nowadays visual disability compensation, as for other disabilities, is mainly contingent on the patient's obtaining an allowance for a third party, but this is increasingly related to the concept of autonomy in the main activities of everyday life. This concept cannot be applied to visual handicaps. A special allowance, however, which need not be identical to that of third party compensation but would cover the poor-sighted currently in groups 2 and 3, whatever the extent of their handicap, may be the right solution for the Authorities to adopt, due to the specific nature of the visual handicap.

[Aids for low vision--strategic approach].

There are currently methods to rehabilitate the remaining visual function of visually impaired people. Treatment stages and tests are based on classical physiology of vision and still remain strictly experimental. Recent investigations showed that the visual system, as a whole, operates as a space frequency analyzer. There are reception to each spatial frequency. They have to be assessed with accuracy before any rehabilitation attempting to develop channels which are still functional by elective stimulation. This stimulation must have a maximum determined energetic efficiency. The study of the Fourier spectrum on two different images demonstrates this possibility and starts opening the way for specific research.

[Light vision and chorioretinal circulation. Study of the effect of procyanidolic oligomers (Endotelon)].

Light vision is a major element of the visual function. However, it varies from one subject to another. Many factors may modify it in the absence of amy retinal pathology (âge, fatigue, stress). A lower resistance to glare and an alteration of scotopic vision are often mentioned during medical visits. We endeavored to obtain a protection with Endotelon in subjects presenting no major retinal or ophthalmological pathology. Endotelon comes in tablets of 50 mg of procyanidolic oligomers. This compound of vegetal origin (from grape seeds) belongs to the chemical class of flavones which selectively fix on collagen and elastin fibers, thus reinforcing the structure of the vascular connective tissue. The drug efficiency on the resistance to glare and on the night morphoscopic vision was demonstrated by means of three tests: Comberg's nyctometer, Beyne's lantern and ergovision. One hundred subjects were tested in two center. In both center, a treated group received 4 tablets of Endotelon a day for five weeks and a control group received no treatment. Two patients stopped the experiment. Nyctometer: the improvement in the visual performances after glare as well as the rapidity with which these results were obtained is very significant compared to the results obtained by the control group. Visual adaptation to low luminances is also improved under treatment. Ergovision tests were performed in two light environments: a standard one and glare. These results support the previous ones. There is a narrow correlation between the decrease in visual performances after a quick retinal glare and the increase in the environmental luminance at the time of inclusion.(ABSTRACT TRUNCATED AT 250 WORDS)
