[Basic studies in intra-arterial chemotherapy with degradable starch microspheres (DSM) on human gastric cancer xenografts in nude rats].

Enhancement of the antitumor effect of adriamycin (ADR) was investigated by using degradable starch microspheres (DSM) and pharmacokinetics of ADR in combination with DSM. An intra-arterial chemotherapy model of the nude rats transplanted of human cancer xenografts (H-154 gastric cancer) in the lower limbs was used for this study. Drug was administered through a catheter inserted into the carotid artery with the tip in the common iliac artery. DSM 30 mg/kg, which causes temporary arrest of blood flow in the tumor, had an only weak effect on tumor growth, whereas. DSM 30 mg/kg, mixed with ADR 3 mg/kg solution, was more effective than ADR solution. Furthermore, DSM 30 mg/kg mixed with ADR 3 mg/kg had a greater effect on the tumor growth than DSM 15 mg/kg mixed with ADR 3 mg/kg. In the pharmacological study, increase of the regional uptake of ADR and decrease of systemic distribution of ADR were recognized in some degree. It seems that embolization by DSM, retention of ADR in regional tissues and cytotoxic effect of ADR contributed to such a strong effect of ADR mixed with DSM on tumor growth.

[Tumor markers in gastrointestinal cancer and its clinical significance].

The clinical usefulness of tumor markers was discussed in gastrointestinal cancer. SCC antigen (subfraction of TA-4) was useful marker in esophageal squamous cell cancer, but was not so helpful in cancer treatment. CEA and CA19-9 were also available as tumor markers of gastric and colorectal cancers, but extremely increased levels were found in only advanced but in early cancer. There was no significant correlation between CEA and CA19-9 so the combination assay of a few kinds of tumor markers, especially CEA and CA19-9 were helpful in cancer treatment.

[Clinical evaluation of astromicin administered by intravenous drip infusion. Report II. Bacterial infections in the field of surgery].

A clinical evaluation of astromicin (ASTM) administered by intravenous drip infusion against infections in the surgical field was made, and the results were summarized as follows. Excellent effect was observed in 19 out of a total of 44 cases, good effect in 19, fair in 1 and poor in 5. The efficacy rate calculated from the 38 cases of "excellent" and "good" was 86%. In stratification by disease, the efficacy rate was 91% in localized peritonitis (31/34 cases) and 63% in diffuse peritonitis (5/8 cases); the overall efficacy rate in peritonitis was 86%. The efficacy rate in 2 cases infected by Gram-positive bacteria was 50%, and that in 16 cases by Gram-negative bacteria was 94%. The disappearance rate of Gram-negative bacteria was 93%, and this drug was especially effective against E. coli. There were no subjective or objective side effects and no abnormal laboratory test values that were related to the administration of ASTM.

[Therapeutic effect of dried ion exchange resin-treated human immunoglobulin (SM-4300) against severe bacterial and/or fungal infections in surgery].

A newly developed human immunoglobulin for intravenous use, SM-4300, purified by cold ethanol precipitation and ion exchange resin, has been studied in the surgery. SM-4300 has been evaluated clinically against inpatients with severe bacterial and/or fungal infections in the combined use with the antibiotics which were resistant to antibiotic therapy. Total number of 13 patients affected with various severe infections were treated with SM-4300. Clinical effects of SM-4300 were excellent in 2 cases, good in 6, fair in 3 and poor in 2. The efficacy rate was summarized as 61.5%. No subjective and objective clinical side effects and abnormal laboratory findings were observed. In conclusion, combination therapy with SM-4300 and antibiotics was considered to be safe and effective against severe bacterial and/or fungal infections in the surgery.

[Mass screening for breast cancer--the results for 15 years].

Mass screening for breast cancer has been carried out in 13 cities in Osaka Prefecture for the past 15 years. The screening method was inspection and palpation by the physician, and examination by mammography was performed for the women with abnormal findings as the second screening. Among the total of 73,488 examinees (actual number was 44,835 examinees) 3,022 (4.1%) were examined by mammography and 106 (0.14%) patients with breast cancer were detected. Seventy-two of them were found at the first screening, 15 at the subsequent screening, and 19 were interval cases. Sixty-two cases of breast cancer (63.9%) were without lymph node metastasis, and cancer in Stage I and TIS reached 37.8% in this series. These results show that our screening method is valid for detecting the earlier stages of breast cancer.

Metastasizing eccrine sweat gland carcinoma. Report of a case.

We report a case of eccrine sweat gland carcinoma arising from the occipital region, which later metastasized to the regional lymph nodes and spread widely to the viscera. After wide local excision of the primary tumor, it recurred locally, and then metastasized presumably via regional lymph nodes. Widespread metastasis to visceral organs occurred after a ten-year latent period from the appearance of the original tumor. Histochemical and enzymatic histochemical examinations of the specimens from the original tumor and metastasized lesions obtained at autopsy disclosed the characteristics of eccrine sweat gland tumor.

[Experimental studies on the optimal therapeutic mode of intra-arterial infusion of 5-fluorouracil. (1) Preparation of an animal model for continuous intra-arterial infusion and comparison of the therapeutic and adverse effects of 5-fluorouracil delivered by various modes].

5-Fluorouracil (5-FU) is one of the most popular anticancer drugs, especially in treatment of for adenocarcinoma; however, an optimal therapeutic schedule of 5-FU is not determined even now. An animal model for continuous intraarterial infusion in rats was constructed using retrograde cannulation to aorta via right femoral artery in order to study the intraarterial and systemic effects of 5-FU on Yoshida sarcoma implanted in left fore limb and hind limb subcutaneously. By implanting Yoshida sarcoma in fore and hind limb and cannulating and infusing aorta, the regional and systemic antitumor effects could be compared in the same animal. Antitumor effect and toxicity after intraarterial 5-FU administration was studied at various infusion time and doses. The results revealed that intraarterial infusion of 5-FU was more effective than its systemic administration from view points of the antitumor effect. The antitumor effect after one shot intraarterial injection of 5-FU was ar superior to that after continuous infusion of 5-FU. However, toxicity after one shot injection was much more severe than continuous infusion. Consequently, the desirable administration schedule of 5-FU appears to be an intraarterial infusion as short time as possible within tolerable toxicity.

[Experimental studies on the optimal therapeutic mode of intra-arterial infusion of 5-fluorouracil. (2) Concentrations within the tumors and normal tissues and therapeutic and adverse effects of 5-fluorouracil delivered by various routes].

5-Fluorouracil levels in Yoshida sarcoma implanted subcutaneously and some organs of rats were measured following intraarterial or intravenous administration of 5-FU at various infusion time and doses, and the relationship between 5-FU level in tumors and antitumor effect was examined. The antitumor effect following intraarterial infusion of 5-FU was superior to its systemic administration. With intraarterial infusion, higher levels of 5-FU in tumors could be expected during infusion and immediately after injection. One-shot intraarterial injection of 5-FU was much more effective than continuous infusion of 5-FU from view points of the antitumor effect. This reason might be attributed to that, 5-FU level in the tumor after one shot intraarterial injection was maintained higher for many longer period than its level during continuous infusion. 5-FU levels in the liver following and during intraaortic infusion of 5-FU showed interesting change, i.e. if 5-FU levels flowing into the liver was low, 5-FU in the liver was not detected at all, however, if high level of 5-FU was flown into, the liver 5-FU was measured even 24 hours after injection. It was supposed that the low level of 5-FU was degraded by the liver easily, but the high level of 5-FU flown into the liver could not be metabolized. From these experiments it may be concluded that infusion time of intraarterial administration of 5-FU must be as short as possible within tolerable toxicity, and infusion doses must be as high as possible that could be degraded by the liver for reduction of toxicity.

Evaluation of effectiveness of mass screening for colorectal cancer.

As the first step in the epidemiological evaluation of the effectiveness of mass screening for colorectal cancer, we compared clinicopathological features and survival rates of patients with cancer detected by mass screening (screened group) with those for patients treated in our outpatient clinic in the same period (outpatient group). The screened group consisted of 53 patients with colorectal cancer detected by 2-day or 3-day screening for fecal occult blood by guaiac slides. Their background factors were comparable to those of 120 patients of the outpatient group in regard to age, sex ratio, location of cancer, and histological type of cancer. In the screened group, 90% of patients had no complaints, and positive occult blood tests led to the detection of cancers. More than 60% of the patients had Dukes' A and B1 early cancers while only about 30% had advanced cancers. In the outpatient group, nearly 90% of patients were symptomatic, most commonly from rectal bleeding. Early-stage cancers made up only 20%, and large, advanced-stage cancers accounted for 80%. The 5-year survival rate of the screened group was 91.5%, being significantly higher than the 60% survival rate for the outpatient group. It is anticipated that mass screening for colorectal cancer by guaiac fecal occult blood testing will significantly reduce the mortality due to this neoplasm.
