Alpha spectrometry for measuring activity of glass-embedded 210PO isotope.

Measurements of exposure to radon are performed using numerous research methods which register either temporary or periodic radon concentrations. At the end of the 1990s reports on the possibility of defining past exposure to radon on the basis of measurement of the contents of 210Pb embedded in surface layer of glass were published. The registration of alpha particles emitted with glass was made using the spectrometric system. Use of the system for alpha spectroscopy leads finally to a significant lowering of the bottom threshold of detection and limits the influence of the negative background on the examined glass.

[A comparison of clinical and surgical-pathologic staging of advanced vulvar cancer].

A surgico-pathological staging system introduced in 1988 by FIGO replaced previously used clinical classification of vulvar cancer extent. The aim of our study was to compare retrospectively the prognostic value of both of these staging systems (with the modification of surgical staging introduced in 1994) in the group of 123 vulvar cancer patients undergoing radical surgery including lymph node dissection in our institution. The comparison of both staging systems is presented in the table. Ninety patients (73%) have been assigned to the same stage in both systems, in 27 patients (22%) the use of surgical system resulted in upstaging, as compared to the clinical classification and 6 patients (5%) have been down-staged by the surgical system. In the analysis of prognostic value of both classifications the surgical staging system proved to have better discriminating power for prediction of survival in vulvar cancer patients.

[Analysis of intra- and postoperative complications and postoperative course in patients surgically treated for vulvar cancer].

Surgical complications were evaluated in 162 vulvar cancer patients. Assessed were: the frequency and type of postoperative complications, the incidence of perioperative blood transfusions, the course of postoperative wound healing, the length of postoperative hospital stay and reasons for perioperative mortality. An analysis of factors influence the risk of complications was conducted.

[Borderline ovarian tumors: clinical analysis of 114 cases].

OBJECTIVES: The aim of this study was to analyse the group of patients with borderline ovarian tumours. DESIGN: The analysis included 114 patients, operated for ovarian tumours of borderline malignancy in the Gynaecological Department of Medical University of Gdańsk between 1978-1997. The study takes into account comparison of: age of patients, obstetrical past, clinical signs and symptoms, clinical stage (according to FIGO), type of surgery, tumour pathology, post surgical treatment. Furthermore, long-term follow up was assessed. RESULTS: Middle age in the group was 48 years, main symptoms: pain of lower part of abdomen (47%) and ascites (26%). 92% of tumours were recognised in stage I (FIGO). 44.7% of the tumours were histological serous, 36% were mucinous. All patients were treated by surgery and 12% received additional treatment. Mean follow up was 104 months (1-247). 9.6% of the patients died because of main disease, next ten persons for reasons not connected with the main disease. 5 year survival rate was 91.2%, 10-year 84.2%. CONCLUSIONS: 1. Borderline ovarian tumours are most frequently recognised in stage I. 2. Serous and mucinous borderline ovarian are dominant. 3. Type of surgery is dependent on age of patients, obstetrical past, clinical stage and tumour pathology. 4. Prognosis in borderline ovarian tumours is excellent.

Measurement of 210Po atoms content in glass as an indicator of long-term exposure to radon.

Measurements of exposure to radon are performed using numerous research methods which register either temporary or periodic radon concentrations. The method presented below allows for the estimation of average radon concentration in the past. This is possible due to indirect measurement of the contents of 210Pb embedded in glass structure. The half-life of 210Pb is about 22 years. A number of exposures of window glass to radon have been carried out in laboratory conditions (radon chamber) and the obtained results were used to calculate the coefficient that renders it possible to define global indoor exposure to radon. The registration was made using a track detector CR-39, which records alpha particles resulting from the disintegration of 210Po, one of 222Rn decay products.

[Pregnancy and delivery after liver transplantation: case report].

We report a case of successful pregnancy in a 22-year-old primigravida who became pregnant 19 months after orthotopic liver transplantation for end-stage liver dysfunction caused by Wilson disease. The mother continued immunosuppressive therapy (tacrolimus) during the pregnancy. No complications were observed. Labour started spontaneously at 40th week's gestation. As a result the healthy male infant weighing 3900 g was born. Patient and the baby were discharged from hospital, both in good condition, on the fifth day after delivery.

[Problems of contraception in women with Wilson disease].

Because of the constant progress in medical therapy, the fertility status of patients with Wilson's disease may be preserved. Contraceptive counselling is, therefore, essential to these women. Some contraceptive methods can have a negative influence on hepatic function or hepatic insufficiency may reduce the efficacy of other contraceptives. Different contraceptive options for patients with Wilson's disease are described. Second generation intrauterine devices with copper and estrogen-containing oral contraceptives are contraindicated. Spermatocide and barrier contraceptives and progesterone-only preparations can be safely used.

[Hyperthyroidism and pregnancy].

DESIGN: The aim of our study was to describe a course of pregnancy, delivery and puerperal time in women hospitalized in our Clinic during their pregnancies because of hyperthyroidism. MATERIALS AND METHODS: Sixteen case reports (thirteen pregnant women) were retrospectively analysed. Our study embrace data from the recent ten years. RESULTS: In all cases pregnancies, deliveries and puerperal time proceeded without any serious complications. Infants were delivered in good condition and no congenital defects were reported. CONCLUSIONS: 1. A proper treatment of hyperthyroidism in a pregnant woman determines the correct course of pregnancy, delivery and puerperal time. 2. We have not found any congenital defects, hypotrophy cases and infants' death reports in our material. 3. Hyperthyroidism in pregnancy does not determine cesarean section as a way of delivery.

[Pregnancy in women with multiple sclerosis (MS): a report of 2 cases].

We report two cases of pregnancy in patients with multiple sclerosis. Both of women were primigravidas. In both cases pregnancy were ended by cesarean section: in first case due to obstetric indications, in second due to neurological indications. Our experience and a review of current literature are presented.

[Contraception and pregnancy in severe systemic lupus erythematosus: a case report].

Systemic Lupus Erythematosus (SLE) during pregnancy may cause serious complications in the mother and fetus. We report a case of pregnancy in 23 years old woman with SLE. Pregnancy was complicated with spontaneous abortion and septic shock. Furthermore in our paper we discussed contraceptive methods proper for women with SLE. The choice of an optimal contraceptive method (OC) therapy is a significant problem in female SLE patients. In view of the influence of sex hormones on the evolution of SLE, oral contraceptive (OC) therapy has to be efficient, reversible and safe, without aggravating disease activity and causing metabolic and vascular side effects. As regards oral contraceptives, all authors agree that oestrogen-progestin combination pills are harmful, but the best alternative hormonal contraception remains to be determined. The management of the case and the review of current literature are presented. Our experience demonstrates the importance of interdisciplinary care.

[Pregnancy, labor and puerperium in patients after surgery for ovarian tumors of borderline malignancy].

OBJECTIVES: The aim of the study was to assess procreation in patients treated surgically for borderline tumours of the ovary. DESIGN: The analysis included 114 patients operated for ovarian tumours of borderline malignancy in the Gynaecological Department, Medical University of Gdańsk between 1978-1997. Evaluated were: the incidence of pregnancy, age of patients, tumour pathology, type of surgery, the course of pregnancy, labour and puerperium. RESULTS: In the group of 37 stage Ia patients, who had undergone conservative surgical procedure, 7 patients conceived and 9 healthy children were born. All patients are free of disease. CONCLUSIONS: Conservative surgery for borderline ovarian tumours allows for retaining procreational potential and normal conception in young women.

[Hypothyroidism coexisting with pregnancy].

DESIGN: The aim of the study was to describe pregnancies, deliveries, and puerperal times, and to compare them with the reached scientific communications. MATERIAL AND METHODS: Four pregnancies in women with the hypothyroidism were described. All cases were taken from the group of twenty pregnancies (18 women) who were treated because of thyroid gland diseases in our clinic. RESULTS: We found that every infant was born in good condition, deliveries and childbeds were uncomplicated. CONCLUSIONS: Our notices are similar to new scientific communications, that hypothyroidism coexists with pregnancy much often then everybody, till now, suspected and infants have not so often congenital defects.

[A clinical analysis of risk factors in women operated because of endometrial cancer].

OBJECTIVES: The aim of the study was a clinical analysis of risk factors of endometrial cancer such as diabetes mellitus, hypertension, obesitas, using oral contraceptives, smoking cigarettes and neoplasm diseases in a family. MATERIAL AND METHODS: Between 1983-1997, 336 patients was operated in 2nd Department of Obstetrics and Gynaecology Medical University of Gdańsk because of endometrial cancer. 117 women, who answered inquiry form questions was selected. RESULTS: The median age of patients was 58.9 years (41 to 83 years). In 71.79% the neoplasm disease was found in postmenopausal age. Nulliparas was 16.2%. In 96 cases (82.5%) an adenocarcinoma was found, in 14 cases (12.0%)--a adenosquamous cell carcinoma and in 1 case (0.8%)--a squamous cell carcinoma of uterine corpus was found. In 51 patients (43.6%) a obesitas was found, in 44 patients (43.6%)--hypertension and in 14 patients (11.9%)--diabetes mellitus. In 11 cases (9.4%) diabetes mellitus and hypertension coexist and in 9 cases (7.7%)--all diseases. Only 2 women (1.7%) used oral contraceptives, so it is impossible to estimate its influence on risk of endometrial cancer. 18 patients (15.4%) was smokers and 49 women (41.9%) had a neoplasm diseases in their families (the most often--43.7%--a neoplasm disease was in families of women with adenocarcinoma). CONCLUSIONS: The percentage of nulliparas was lower then it is described in literature (40-50%) and was 16.2%. The highest percentage of nulliparas was found in a group of women with adenosquamous carcinoma. The most often histological type of endometrial carcinoma was high-differentiated adenocarcinoma.

[A complementary treatment of patients with endometrial carcinoma after surgical treatment].

OBJECTIVES: The different methods of a complementary treatment used in a group of 117 patients, aged 41 to 83, after surgical treatment because of endometrial cancer in 2nd Department of Obstetrics and Gynaecology Medical University of Gdańsk between 1983-1997, were analysed. RESULTS: The hysterectomy with removal of uterine adnexa was performed in all cases, in 18 patients supplemented by pelvical lymphadenectomy. The median age of women was 58.9 years. In 96 cases (82.5%) an adenocarcinoma was found, in 14 cases (12.0%)--a adenosquamous cell carcinoma and in 1 case (0.8%)--a squamous cell carcinoma of uterine corpus was found. In 52 patients (44.4%) a complementary treatment was used: in 43 patients (82.7%)--radiotherapy, in 4 patients (7.7%)--chemiotherapy, in 5 patients (9.6%)--radio- and chemiotherapy and in 1 case (1.9%)--radio- and hormonotherapy. In 8 women (6.8%) a second laparotomy was made but only in 1 case it was caused by ileus because of neoplasm metastases. The median survival of all patients was 6.1 years; without complementary treatment--6.48 years, after radiotherapy--6.25 years, after chemiotherapy--2.75 years and in a group of women after associated treatment--4.4 years. CONCLUSIONS: Between all patients with endometrial cancer after complementary treatment, the longest survival was found in a group of women after complemetary radiotherapy then in a group of patients after chemiotherapy or associated treatment (radio- and chemiotherapy). The highest percentage of women without any symptoms of disease was in a group of patients after radio- and chemiotherapy or only chemiotherapy against to patients after chemiotherapy.

[The evaluation of forceps delivery complications].

141 forceps deliveries were studied. The frequency, number and type of complications were evaluated. Decreased number of forceps delivery and relatively high risk of complications in mothers and neonates were indicated. More then half of examined women in childbirth had various injuries of their birth canal. About 30% of new-borns were born with different types of birth's complications. Our results indicate that after forceps delivery the women and new-borns require special care in the perinatal period due to relatively high risk of serious complications.

[The evaluation of mean indoor concentration of radon based on the measurement of 210Pb concentration in glass].

An average dose received by an inhabitant of the terrestrial globe from all natural sources of ionizing radiation occurring in the nature accounts for 2.6 mSv annually. Nearly 50 of this dose comes from radon and products of its decomposition which accumulate in our dwellings. Bearing in mind changes in radon concentrations over the time it would be advisable to develop a method for the evaluation of mean long-term concentrations of radon. Such an evaluation can be carried out by measuring the amount of 210Pb implanted in glass. These measurements are most frequently performed by means of ionization chambers or trace detectors which record the number of alpha particles produced by 210Pb decomposition.

A role for baculovirus GP41 in budded virus production.

Insect cells infected with tsB1074, a temperature-sensitive mutant of Autographa californica nuclear polyhedrosis virus, exhibit a "single-cell-infection" phenotype whereby the infection progresses through the very late phase culminating in occlusion body formation, but neighboring cells do not become infected. Marker rescue mapping and DNA sequencing correlated a single nucleotide substitution within the baculovirus gp41 gene with the temperature-sensitive phenotype of tsB1074. The product of the gp41 gene, GP41, is an O-glycosylated protein found in occluded but not budded virions [M. Whitford and P. Faulkner (1992) J. Virol. 66, 3324-3329]. However, budded virus was not produced in tsB1074-infected cells at the nonpermissive temperature of 33 degrees, indicating an additional role for GP41 in budded virus formation. Electron microscopy revealed that nucleocapsids were produced but retained in the nucleus of tsB1074-infected cells at 33 degrees. Thus, GP41 was required for the egress of nucleocapsids from the nucleus in the pathway of budded virus synthesis.
