A review of antiphospholipid antibody syndrome.

PURPOSE: To review the pathophysiology, clinical presentation, and management options for antiphospholipid antibody syndrome (APS), a potentially life-threatening coagulation disorder. DATA SOURCES: Selected scientific literature, consensus guidelines, and expert opinion. CONCLUSIONS: Clinical features that should alert the clinician to consider APS include recurrent fetal loss, arterial or venous thrombosis, thrombocytopenia and livedo reticularis. One should be suspicious of this diagnosis in a younger patient, one with an autoimmune disease, or family history of autoimmune disease. To confirm the diagnosis one needs both clinical and laboratory abnormalities. IMPLICATIONS FOR PRACTICE: The signs and symptoms of APS are varied and could be confused with many disorders. The primary care provider needs to be aware of this syndrome in order to include it in the differential diagnosis and appropriately recognize and refer the patient in a timely manner.

Graphical simulation of early development of the cerebral cortex.

Much experimental data exists concerning the development of the cerebral cortex. There is a need for a common vehicle to integrate this data and to allow the testing of hypotheses concerning development. Computer simulation and visualization are powerful mechanisms for hypothesis testing. Our long-term goal is to create a robust, extensible, portable tool for simulation and visualization of cortical development to serve both research and educational purposes. This paper describes a simulation program, SimCortex, which models the early stages of development of the cerebral cortex of the mouse. Version 1.0 of SimCortex models the proliferation of progenitor cells in the pseudostratified ventricular epithelium (PVE), the generation of young neurons and their migration into the cortical plate, which is the forerunner of cell layers II through VI of the adult cortex. We present an overview of the design and implementation of SimCortex, sample output of the system and a comparison of our results with experimental data. We conclude with a brief overview of proposed future enhancements of the system.

Extension metacarpal osteotomy in the treatment of trapeziometacarpal osteoarthritis: a biomechanical study.

The biomechanical effects of extension metacarpal osteotomy on contact pressures in the trapeziometacarpal joint during lateral pinch were studied in 20 anatomic specimens using pressure-sensitive film. The gross appearance of joint surfaces was classified by severity of arthritic disease and correlated with specimen x-ray films taken before the osteotomy. Extension metacarpal osteotomy effectively unloaded the palmar contact area in nonarthritic and moderately arthritic specimens; primary contact areas and zones of peak pressure were shifted from the diseased palmar compartment to the normal dorsal compartment. In contrast, the pathologically congruent contact pattern seen in end-stage osteoarthritic joints was unaffected by osteotomy. The data demonstrate the efficacy of extension metacarpal osteotomy in unloading the palmar contact areas of normal and moderately arthritic joints but provide no biomechanical rationale for metacarpal osteotomy as originally described in treatment of advanced trapeziometacarpal osteoarthritis.

Evaluation and management of the patient with ulcerative colitis.

Ulcerative colitis is an inflammatory bowel disease that affects the large intestine. Ulcerative colitis is chronic and can be debilitating. The disease process waxes and wanes, which sometimes gives patients a false sense of cure. The endoscopy laboratory nurse is at the forefront to provide patients with education and offer community support resources. By educating the patient and introducing community outreach programs, the patient can learn to participate and be a partner with the provider in his or her healthcare management. The case study will help outline pertinent information for educating the patient in the management of ulcerative colitis.
