In Cuba and in the German Democratic Republic (GDR) a total of 24,412 pregnant women were tested for maternal serum alpha-fetoprotein (MSAFP) at the 16th to 20th week of gestation. An inexpensive and partly automated ultramicroliter enzyme immunoassay (ELISA) with final volumes of 10 microliter was used to analyze simultaneously 50 samples. The intraassay coefficient of variation (CV) of 5-8% and day/day CVs of 6-10.5% were obtained with a test frequency of 320 assays/day. A cut-off level of twice the median value (MoM) was chosen. An amniocentesis was done in a total of 0.5% (in the GDR) and 0.7% (in Cuba) of the screened women. The prevalence of open neural tube defects (ONTD) was calculated from the present study and was 1.43% in Cuba and 1.34% in the GDR. Through MSAFP screening 88.2% ONTD were detected. There was no therapeutic abortion of a normal fetus. The approximate cost for this program was about 2.36 marks-GDR per patient screened, or about 2,048 marks per ONTD detected.
The intracellular distribution of 67Ga was studied in solid hepatoma 22 implanted in C3Ha/6 mice and in normal liver tissue from the same animals at different time intervals. The tissues were fractionated according to differential centrifugation principles, and subcellular fractions were isolated consecutively. The enzyme activities and the accumulation of 67Ga were determined in each fraction. The subcellular distribution of 67Ga in the tumor tissue was different compared with normal liver; in tumor it was found mainly in the nuclear fraction and this distribution was independent of time, but in normal liver the accumulation was mainly in the mitochondrial fraction, this was time-dependent and the maximal uptake was found 48 h after 67Ga administration.

A compound was synthetized, which by X-Ray diffraction studies proved to be hexahydrated sodium tripolyphosphate. By means of in vitro procedures, it was demonstrated that this compound was able to complex trivalent cations, and that 113mIn could be completely incorporated to it. In vivo studies in rats, and scans performed with this new radiopharmaceutical in humans showed its highly selective accumulation in metastasic bone tumors.
The study was aimed at evaluating the effect of pyridinolcarbamate in patients with atherosclerosis obliterans of the lower limbs. A single intravenous administration of the drug did not induce changes in resting blood flow, but elicited a significant decline in capillary filtration rate. In a double-blind cross-over trial patients reported a significant prolongation of walking distance after 6 months of pyridinolcarbamate treatment. Sensation of cold in the lower limbs was not affected. Pyridinolcarbamate did not influence peak values of blood flow through the calf but they appeared significantly earlier. No significant changes were seen at the evaluation of the elevation and dependency test and performance using tiptoeing test. Likewise the findings of systolic pressure in the lower limbs, pulse wave morphology, and claudication distance on treadmill did not confirm a therapeutic effect of pyridinolcarbamate.

INTRODUCTION: Despite the growing recognition of the potential applications of cost-effectiveness assessments, a criterion to establish what is an efficient health technology does not exist in Spain. The objective of this work is to describe the limits and the criteria used in Spain to recommend the adoption of health interventions. METHOD: A review of the economic evaluations of health technologies published in Spain from 1990 to 2001 was conducted. Complete economic assessments in which the cost-effectiveness ratio was expressed as cost per life-year gained (LYG), cost per quality-adjusted-life-year (QALY) or cost per saved live were selected. Those interventions in which the authors established recommendations (adoption or rejection) and the criteria used were analyzed. RESULTS: Twenty (20%) of the 100 complete economic evaluations fulfilled the selection criteria. In16 studies, the results were expressed as cost per LYG, in 6 studies as cost per QALY and in 1 as cost per saved live. A total of 82 health interventions were assessed and some kind of recommendation was established in 44 of them. All technologies with a cost-effectiveness ratio lower than 30,000 euros (5 million pesetas) per LYG were recommended for adoption by the authors. Up to that limit there was no a clear tendency. CONCLUSIONS: Although the results must be interpreted with much precaution, given the limitations of the study, the limits of cost-effectiveness presented in this work could be a first reference to which would be an efficient health intervention in Spain.
Hospitals and physician practice management companies are making offers to many group practices. It's important for group practice managers to know what to look for if a buy-out deal is offered. First, make sure the proposed partner will invest in front-line medicine and has stable management. Secondary considerations include: making sure buyer and seller goals align, accurately projecting capital needs and who will pay for them, guarding against provider overcapacity, clearly delineating parameters for control and limiting bureaucracy.
A rapid analytical method is proposed for the determination of simazine, terbuthylazine, and their chloro dealkylated metabolites (simazine-desethyl, simazine-bisdesethyl, and terbuthylazine-desethyl) in soil. A sonication micromethod is presented for the extraction of -triazine herbicides and their metabolites. Final determination is by gas chromatography (GC) with nitrogen-phosphorus detection. The identity of all compounds was confirmed by GC with mass selective detection in the selected-ion monitoring mode. All chromatograms were very clean, without interfering peaks, and no cleanup was needed. The limits of detection were 1 pg for simazine-bisdesethyl; 5 pg for simazine, terbuthylazine, and terbuthylazine-desethyl; and 10 pg for simazine-desethyl. The limits of quantitation were 1, 5, and 10 ppb, respectively. Mean recoveries from fortified soils ranged from 76% for simazine-bisdesethyl to 102% for simazine-desethyl, with relative standard deviations of 3-6%.
A newly formed pediatric faculty practice plan had to overcome numerous hurdles blocking profitability, patient satisfaction, and time for clinical care, teaching and research. The authors describe the seven-point strategy that took the Connecticut Children's Medical Center Faculty Practice Plan from instability and disorganization to success as a high-volume clinical enterprise.
