Flavonoids with anti-cataract activity from Brickellia arguta.

Six flavonoids were isolated from Brickellia arguta and identified using chemical and spectral methods. The isolation and spectral data of a new flavonoid, 6- methoxykaempferol 3-O-beta-D- robinobioside (3), are reported for the first time. Three of these flavonoids were tested and showed inhibition of rat lens aldose reductase.

Nursing intervention for self-esteem disturbances.

In 1988, the broad diagnosis of self-esteem disturbance was refined into three distinct problems: defensive coping, situational low self-esteem, and chronic low self-esteem. The rationale for this refinement is discussed, and distinct nursing interventions are proposed that differ by type of self-esteem problem and nursing expertise.

Validation study for impaired personal boundaries, proposed nursing diagnosis.

The authors propose a new nursing diagnosis of impaired personal boundaries based on their experience in psychiatric nursing, a concept analysis of the literature and diagnostic content validation by a panel of experts. Impaired personal boundaries are common in a number of mental disorders including, schizophrenia, borderline personality, substance abuse and enabling. Recognition of this patient problem has implications for further research and nursing interventions.

A businesslike hospital gets business's attention.

Silicon Valley, home of the avant garde in American industry, is also the home of El Camino Hospital, a well-respected institution where cost containment has long been emphasized. When Hewlett-Packard was looking for a PPO-like relationship with a hospital that could assure high-quality care, it joined with El Camino in a carefully structured--and already money-saving--arrangement. That "EPO," and other programs of this businesslike hospital, are described by its CEO and Associate CEO.

Community organized food and nutrition education: participation, attitudes and nutritional risk in seniors.

UNLABELLED: Evergreen Action Nutrition (EAN) is a health promotion program designed to facilitate relatively healthy members of a seniors recreation center to maintain their nutritional health as they age. A main goal of this project was to demonstrate the feasibility and relevance of using the community organization approach to develop a nutrition education program for seniors. METHODS: Using the current membership list, seniors were randomly selected to receive a mailed baseline (n=247) questionnaire. A follow-up survey (n=251) was sent out to randomly selected members three years later to determine participation in EAN and reported behavior change. Although not the same individuals, responses were compared to baseline to determine changes in nutritional risk. Items from the Diet and Health Knowledge survey were compared by EAN participation. RESULTS: The program had a large reach with 162 survey respondents (64.5%) reporting some level of participation and 51% reporting "frequent" participation. Use of informal forms of education predominated (e.g. displays). Significant differences were found between baseline and follow-up for risk attributed to low intake of fruits and vegetables and frequency of eating, with EAN participants having reduced risk of low fruit and vegetable intake. Those participating in formal education (e.g. food workshops) reported more frequent changes in food practices than those participating in informal activities. EAN participants appear to have more healthy nutrition attitudes/beliefs. CONCLUSION: The community organization approach to program planning and delivery leads to the development of diverse and appropriate nutrition education activities for seniors. Informal and formal health promotion activities can be successfully implemented in recreation centers.

Decision support and outcomes of nurses' care planning.

One hundred forty-three nurses served as their own controls in planning care for hypothetical patients under three different manipulated conditions of decision support: (A) use of traditional resources; (B) use of a printout of a computerized protocol for care planning (NPC); and (C) use of an NPC protocol and a self study guide. A series of 3 x 5 repeated measures analyses of variance (ANOVAs) were used to evaluate the main effects and interactions of the experimental treatment conditions and the two nurse attribute variables of education and experience (each with five levels). The case problems and conditions were counterbalanced to control for extraneous variables. The repeated measures ANOVA demonstrated significantly higher mean ratings of quality for care planned under both computerized decision support conditions than the traditional condition on all three patient problem situations F(2, 270) = 7.9, p less than .01. Time on task was also significant F(2, 272) = 5.8, p less than .01 with use of the decision support consultants tending to increase time in planning.

Psychometric analysis of the problem rating scale for outcomes.

The current emphasis on outcomes management and outcomes research projects is stimulating interest in the psychometric properties of computerized clinical data bases among nurses and other health care providers. The knowledge, behavior, and status subscales of the Omaha System's Problem Rating Scale for Outcomes (PRSO) were evaluated for interrater reliability and content validity. The authors describe the methods used and the results of that research.
