Changes in pituitary and ovarian hormones following injection of ultra low dose LH-RH in normal and amenorrhoeic women.

The results of the ultra low doses of synthetic LH-RH, 1.56, 3.125 and 6.25 mug, are presented in 8 normal women during the first week of the menstrual cycle and 14 patients with secondary amenorrhoea. Seven of these patients had "stress" amenorrhoea, and 7 had preceeding oligomenorrhoea. There was no significant difference between the mean basal gonadotrophin levels in the normal volunteers and those patients with secondary amenorrhoea. There was no significant difference in basal gonadotrophin levels or the response to LH-RH, between the two groups of amenorrhoea, stress and previous oligomenorrhoea. Basal FSH and oestradiol levels do not appear to influence the response to LH-RH. However, those patients with secondary amenorrhoea and a low basal LH (5 patients) had a significantly higher response to LH-RH at 1.56 and 3.125 mug than the amenorrhoeic patients with normal basal LH levels. Two patients with stress amenorrhoea and weight loss had an exaggerated response to LH-RH. The significance of this is discussed.

Intrauterine contraception with the copper 7: evaluation after two years.

From 1,156 insertions of the copper 7 contraceptive device with 15,044 women-months of use we conclude that it offers advantages in intrauterine contraception. Continuity is improved for all users. It is a useful method for those who have not been pregnant and as an exchange device for patients having problems with other intrauterine devices or contraceptive methods. These advantages must be set against the necessity of replacing the copper 7 after a limited life span. In no cases in our series did cervical cytological examination show anything abnormal, and a 98.1% follow-up was achieved.

Hospital family planning: termination of pregnancy and contraceptive use.

A review of termination of pregnancy in a district hospital catchment area for 1970 to 1972 shows how a decrease in total numbers of terminations and repeat terminations may be effected by the use of intensive family planning. In 1972 over 80% had first trimester abortions, but most of the second trimester abortions came from the manual skilled, semi-skilled, and unskilled occupational groups. Over half those attending for termination had never used any method of contraception. Acceptance of family planning after termination was high but motivation poor, emphasizing the suitability of intrauterine devices inserted at the time of termination. The complication rate after termination was low.

Effect of synthetic luteinizing hormone releasing hormone (LH/FSH-RH) in women with menstrual disorders.

Synthetic luteinizing hormone/follicle stimulating hormone-releasing hormone (LH/FSH-RH) (50 mug) was given intravenously to six women with oligomenorrhoea and to four women with secondary amenorrhoea. Peripheral venous blood was withdrawn at regular intervals over a 24-hour period. The concentrations of LH, FSH, and oestradiol-17beta were determined by radioimmunoassay. In all subjects there was a variable rise in LH (3-16 times the mean basal level): in six a small rise in FSH (two to three times the mean basal level) and in seven a twofold to threefold rise in oestradiol three to eight hours after the rise in gonadotrophins.
