Granulomatous enteropathy in common variable immunodeficiency: a cause of chronic diarrhoea.

Gastrointestinal disease is a well recognized feature in patients with common variable immunodeficiency, and is often due to infection with a variety of organisms. Symptoms usually improve with appropriate antibiotic therapy and replacement gammaglobulin. We describe three middle-aged female patients with common variable immunodeficiency who had protracted diarrhoea and weight loss. Despite extensive investigation no infectious cause was found. All patients had granulomas distributed throughout the gastrointestinal tract, but no features of inflammatory bowel disease. There was a poor response to gammaglobulin replacement therapy, antibiotics or symptomatic treatment. We suggest that granulomatous enteropathy is another gastrointestinal manifestation of common variable immunodeficiency.

Smoking and diabetes mellitus. Findings from 1988 Behavioral Risk Factor Surveillance System.

OBJECTIVE: To examine the association between smoking and self-reported diabetes mellitus. RESEARCH DESIGN AND METHODS: The study was comprised of 3006 people greater than or equal to 18 yr of age with diabetes and 52,750 without diabetes. RESULTS: Twenty-six percent (SE 1.8) of the diabetic population were current smokers, 25.9% (SE 1.4) were former smokers, and 48.1% (SE 2) had never smoked. Similar percentages were found among the nondiabetic population. Compared to respondents without diabetes, the prevalence of current smoking was notably higher among respondents with diabetes who were between the ages of 18 and 34 yr (36.1%, SE 4.1), who had not graduated from high school (44.9%, SE 3.7), and who were men of African-American origin (54.8%, SE 5.8). CONCLUSIONS: Despite the excess risks, the overall prevalence of smoking in the diabetic population is comparable to that of the general population. Our finding of a higher prevalence of smoking among young people, people who had not graduated from high school, and African-American men with diabetes suggests that additional educational efforts should be targeted at these groups.

Can knee replacements be assessed by post?

The audit of knee replacement surgery requires long-term follow-up. The aim of this Bristol study was to examine whether an accurate assessment of knee replacement surgery could be undertaken by post, thus obviating the need for patients to visit hospital. A patient's questionnaire was designed to complement the clinic therapist's assessment form currently in use, and this was completed by 73 patients without supervision prior to the therapist's assessment. A comparison of the two assessments shows a significant discrepancy between their results, particularly in the assessment of pain, walking distance, and range of movement. The authors conclude that postal follow-up of knee replacements should be viewed with caution.

The purification and preliminary X-ray diffraction studies of recombinant Synechococcus ribulose-1,5-bisphosphate carboxylase/oxygenase from Escherichia coli.

X-ray crystallographic diffraction data has been collected for recombinant hexadecameric ribulose-P2 carboxylase from the cyanobacterium Synechococcus PCC6301 expressed in Escherichia coli. The enzyme has been purified and then crystallized in a number of crystal forms from polyethylene glycol solutions. The best crystals were obtained with enzyme that was first activated with the cofactors CO2 and Mg2+ in the presence of the tight-binding intermediate analogue, 2'-carboxyarabinitol 1,5-bisphosphate. One crystal form with plate-like morphology diffracts beyond 2.5 A but has one axis greater than 350 A. A second crystal form that diffracts to similar resolution grows with space group P212121 and unit cell dimensions of a = 223.9 A, b = 111.9 A, and c = 199.7 A. The crystal forms used to collect the diffraction data have been redissolved to determine that the recombinant ribulose-P2 carboxylase L8S8 molecule is indeed composed of equal numbers of large and small subunits and also that a quaternary complex between activated ribulose-P2 carboxylase E.CO2.Mg2+, and the analogue was present in the crystals. Denaturation of the redissolved enzyme in the absence of thiol-reducing agents established that the L-subunits of the L8 core are substantially dimeric, cross-linked by a disulfide bridge. Crystals of spinach ribulose-P2 carboxylase were likewise analyzed to show that dimers of the L-subunit were also predominant. This report identifies a single cysteine residue in the L-subunit that forms a bridge between those L-monomers that compose the four putative functional dimers of the L8 core.

Preactivation--a novel antitumour and antiviral approach.

Merocyanine 540 was activated by exposure to 514 nm laser light. This preactivated merocyanine 540 was then mixed (in the dark) with tumour cells, normal cells and envelope viruses to assess its antiproliferative activity. This treatment resulted in 70-90% killing of tumour cells from different cell lines while 85% of normal human peripheral blood mononuclear cells survived the treatment. However, not all types of tumour cells were affected. Preactivated merocyanine 540 was also effective in virtually completely inactivating cell-free herpes simplex and human immunodeficiency viruses. Preactivated photoactive compounds can exert their toxic effects in the dark without further dependence on light and may have potential systemic use.

Malaria in children.

Fifty-four cases of malaria in children seen during an 11-year period are described. There have been more cases seen in recent years, and the proportion of Plasmodium falciparum malaria has increased. Compliance with chemoprophylaxis is poor, but compliance does not guarantee protection. Malaria is the primary diagnostic consideration in any child recently returning from an endemic area who presents with fever. Two cases are described.

Rapid intravenous rehydration in the pediatric emergency department.

Children suffering from mild to moderate (3 to 6%) dehydration likely caused by viral gastroenteritis are often hospitalized because they are unable to tolerate oral fluids. We studied 17 such children, aged one to six years, who were otherwise healthy. All had isonatremic dehydration and were treated with 30 ml/kg of 3.3% dextrose and 0.3% saline over a period of three hours in the emergency department before being discharged. No patient required admission to the hospital. Only one patient required another course of rapid intravenous rehydration and subsequently improved without hospitalization. Although all our patients experienced vomiting before treatment, 65% had no vomiting after treatment. Rapid intravenous rehydration is an effective treatment, for children with mild to moderate dehydration secondary to presumed viral gastroenteritis, that obviates the need for hospitalization.

Breastfeeding problems associated with the early introduction of bottles and pacifiers.

Breastfeeding is a natural physiologic process upon which human survival has depended for uncounted generations. Natural selection over millions of years has ensured that breastmilk contains all the nutritional requirements of the newborn period and beyond. In order to prevent problems for the few, modern management of labour, delivery and the postpartum period has subjected most mothers and infants to routines which are contrary to the physiologic principles underlying successful breastfeeding. The early introduction of bottles may render suckling less effective or may result in breast refusal, thus paving the way for failure to thrive, hyperbilirubinemia, "colic" and crying, prolonged and frequent feedings, sore and cracked nipples for the mother, and it may contribute to the onset of plugged ducts and mastitis. Alternatives to bottles can be used when supplementation is thought to be indicated. They include using a lactation aid, finger feeding, or feeding with a cup, spoon, or eyedropper.

Lack of prognostic value of nucleolar organizer region enumeration in transitional cell carcinoma of the bladder.

Enumeration of silver-stained nucleolar organizer regions is of limited use in grading transitional cell tumors of the bladder. We attempted in this article to establish whether or not such a technique would be helpful in predicting the prognosis of such tumors within rather than between grades. In the study population of 11 high-grade transitional cell carcinomas of the bladder, no correlation was found between clinical outcome and mean nucleolar organizer region count.

Training in liposuction for dermatologists.

Dermatologists helped to develop the specialty of liposuction surgery and contributed to many of the innovations and instrumentation in this field. Dermatologists were trained side by side with other cosmetic surgeons as the procedure was introduced into the United States. Dermatologists have been important in teaching liposuction with the American Society of Liposuction Surgery and have been in the forefront of this developing new area of cosmetic surgery. Liposuction surgery for dermatologists is equal to that for all other surgical specialists.
