Clinical gait assessment in the neurologically impaired. Reliability and meaningfulness.

This study of subjects with multiple sclerosis or hemiparesis assessed the interrater and test-retest reliability of temporal-distance (TD) gait measures and examined the relationship of TD values to functional ambulation ability. Sixty-one subjects ambulated twice on a 9-m (30-ft) paper walkway and rested 15 minutes between trials. An ink footprint record and ambulation time were used to calculate velocity, cadence, step and stride lengths, stride length to lower extremity length ratio, and step- and stride-time differentials. Subjects were rated on a scale that assessed the amount of manual assistance required for ambulation. Interrater and test-retest reliability were high for all TD measures except stride-time differential for the total sample and within diagnostic categories. The TD values were highly reliable in all functional categories except one. All TD measures except stride- and step-time differential displayed a strong linear relationship to the functional ambulation category. Implications for using TD measures in clinical settings are discussed.

Effects of retinal image degradation on ocular growth in cats.

High-powered negative and positive contact lenses have been used to produce a state of continuous retinal defocus in the eyes of 11 kittens in an attempt to induce a predictable amount of axial lengthening and myopia. Another group of six kittens had one eye continuously atropinized and a third group of four animals had the lids of one eye sutured. The axial dimensions and refraction were measured using ultrasonography and retinoscopy respectively. Although the treated eyes of eight kittens tested behaviorally were shown to be amblyopic, no myopia appeared in any animal at any stage during development and only three cats showed a small axial length difference between the two eyes. These results differ from other retinal deprivation studies reported on kittens and no satisfactory explanation for this discrepancy can be offered. However, it is suggested that the lack of change may be associated with the gross anatomy of the cat's eye, and it is concluded that myopia cannot be induced reliably in kittens by retinal image degradation.

Evaluation of Syva's enzyme immunoassay for the detection of Chlamydia trachomatis in urogenital specimens.

A newly developed microwell enzyme immunosorbent assay (EIA) system by Syva Company (Palo Alto, CA) can detect Chlamydia trachomatis in < 3 hr. It uses a polyclonal antibody to chlamydial lipopolysaccharide and end points are determined with a spectrophotometer. Three clinical trial sites (University of California Medical Center, San Francisco, CA; University of Washington, Seattle, WA; and Louisiana State University Medical Center, New Orleans, LA), compared this EIA with tissue culture (TC) for identifying Chlamydia in urogenital specimens. Overall prevalence by TC was 10.4% (136 of 1306). When tests were compared with TC (using vials or microtiter plates and a fluorescent antibody stain), we found an EIA sensitivity of 93.4% (127 of 136) and a specificity of 98.1% (1148 of 1170). This EIA has a performance profile that is, at the very least, comparable with other nonculture methods for diagnosing genital tract infections with C. trachomatis.

Open enrollment and students with disabilities: issues, concerns, fears, and anticipated benefits.

Open enrollment is a hotly debated form of educational choice. More than 25 states have legislation establishing or expanding public school choice options. At an issues clarification working session, professionals, legislators, policymakers, parents, and students described issues and generated implications for serving students with disabilities. Participants identified three kinds of issues for districts and students: outcome, implementation, and demographic. Participants also expressed other concerns about choice: pupil benefit, parent involvement (or convenience), teacher/administrator job protection, change, and teacher workload.
