Biochemical events during initiation of rat hepatocarcinogenesis.

Carcinogenesis is a multistep, multistage process that begins with irreversible, but heritable damage to a single cell. The partial hepatectomy/diethylnitrosamine (DEN) model of rat hepatocarcinogenesis has been well characterized and many aspects of the stage of initiation are known. Recently, it has been suggested that hepatocytes expressing the placental isozyme of glutathione S-transferase (PGST) may be one population of initiated cells. Male Fischer rats were subjected to a 70% partial hepatectomy and at the peak of cell proliferation 24 h later were administered either the solvent trioctanoin, or 10 mg DEN/kg. The rats were administered 100 mg bromodeoxyuridine (BrdU)/kg 1 h prior to death at various times after DEN administration. Since initiation of the carcinogenesis process requires the division of cells containing DNA damage to induce mutations, we examined the concentration of alkylated adducts and the labeling index at various times after DEN administration. In addition, the time course of hepatic PGST expression was determined concurrent with the adduct concentration and labeling index. During the first day after DEN or solvent administration to a rat subjected to a 70% partial hepatectomy, a diurnal variation in labeling index was observed. A recovery to postsurgical labeling index levels was demonstrated for both the solvent- and DEN-treated groups by 7 days. The concentration of three promutagenic lesions was maximal at 6 h after DEN administration. The detectable level of the O6EG adduct was negligible by 24 h after DEN administration, while the two O-alkylpyrimidines, O2ET and O4ET, were retained for much longer periods. Single hepatocytes expressing PGST were observed by 2 days after DEN administration, while small foci of PGST-expressing hepatocytes could be reliably detected by 2 weeks. Two phases of PGST expression in single hepatocytes were observed. The first phase was maximal at day 3 and complete by day 6, while the second reached a plateau by day 8 and was maintained for the 28 days of the study. The presence of the three O-alkylation adducts during a time of enhanced cellular proliferation suggests that all three promutagenic adducts may contribute to the initiation that results in the partial hepatectomy/DEN model of rat hepatocarcinogenesis.

Absorption of organic anions as model drugs following application to rat liver surface in-vivo.

Absorption of organic anions (phenol red, bromphenol blue and bromosulphonphthalein) has been studied after their application to rat liver surface in-vivo, employing a cylindrical glass cell (i.d. 9 mm, area 0.64 cm2). Each drug appeared gradually in the blood with the peak level at about 1 h, after which its concentration declined slowly. Absorbed model drug was efficiently excreted into the bile. These observations appear to indicate the possibility of drug absorption from liver surface membrane. Absorption of model drugs was estimated to be more than 59% in 6 h. The biliary recovery and metabolism of phenol red did not change as compared with that after intravenous administration.

Effect of preservatives on systemic delivery of insulin by ocular instillation in rabbits.

The effects of absorption promoters and ophthalmic preservatives on the systemic absorption of insulin through the ocular route were investigated in albino rabbits. Insulin absorption was evaluated by its hypoglycaemic response. Although ocular instillation of insulin alone did not decrease the serum glucose concentration, instillation of insulin with absorption promoters such as EDTA and saponin decreased it. The promoting effect depended on the concentration of the absorption promoters. Of ophthalmic preservatives investigated (benzalkonium chloride, paraben, 2-phenylethanol, benzyl alcohol and sorbic acid), benzalkonium, chloride and paraben showed promoting effects. The promoting effect for benzalkonium chloride was reversible and dependent on concentration of both benzalkonium chloride and insulin in the formulation.

[Comparison of the prevalence of symptoms, elevated serum antibody, antibody types and complications between two outbreaks of influenza in a ward].

We experienced two outbreaks of influenza in the respiratory ward of our hospital in Feb. 1990 and Feb. 1993. Influenza-like symptoms were recognized in 42 of 67 cases (63%) in 1990, and in 22 of 56 cases (39%) in 1993. In the former outbreak, the prevalence of the serum CF titer for anti-influenza antibody was elevated in 25 of 42 cases (60%) (only the A2 antibody titer in 2 cases, B antibody titer in 4 cases in the latter one). Among asymptomatic inpatients, the serum titer was elevated in 3 of 25 cases (12%) in the former outbreak, but in 1 of 8 cases which the serum CF titer for anti-influenza antibody was measured in the latter one. A respiratory complications secondary to influenza were observed in 6 (2 cases pneumonia, 3 cases lower respiratory tract infection, 1 case asthma attack) of 67 cases during the former period, but only 1 of 56 cases during the latter period. In the two outbreaks of influenza in the respiratory ward we found the same results concerning the occurrence of influenza-like symptoms, an elevation of the serum antibody titer, but a significant difference was noted with regard to the phenotype of the prevalent influenza and the occurrence of complications.

A phase II clinical trial of recombinant human tissue-type plasminogen activator against cerebral vasospasm after aneurysmal subarachnoid hemorrhage.

The results of a Phase II clinical trial of intrathecal recombinant tissue-type plasminogen activator for the prevention of vasospasm were reported. The subjects were 53 patients with aneurysmal subarachnoid hemorrhage (SAH), Groups 2 to 4 in Fisher's preoperative computed tomography classification and Grades II to IV in the Hunt-Kosnik classification. Twenty-four hours after surgery, tissue-type plasminogen activator (TD-2061) was intracisternally administered via a catheter (0.1, 0.2, or 0.4 mg, three times daily for 5 days). The clot-dissolving effects assessed as "effective" and "markedly effective" were virtually the same in the 0.1- and 0.2-mg groups (66.7% and 64.3%, respectively) but slightly lower (53.3%) in the 0.4-mg group, suggesting an adequate effect in the 0.1- and 0.2-mg groups. Severe angiographic vasospasm was not observed in any of three groups. No intergroup differences were noted in the incidence of symptomatic vasospasm, low density on computed tomography 1 month after SAH, and functional prognosis. Bleeding complications were noted in 4 patients (7.5%), including 1 case of SAH in the low 0.1-mg group, 2 cases of SAH in the 0.2-mg group, and 1 case of epidural hematoma in the 0.4-mg group. In overall safety rating, 3 cases with increased SAH and 1 case of epidural hematoma were assessed as "safety doubtful." Other minor side effects such as headache and hepatic dysfunction attributed to the effect of other simultaneously used drugs were assessed as "almost safe," and the rate of "almost safe" and "better" for all dose groups was about 90%, suggesting a safe dose level for all groups. These results suggest that repeated intrathecal administration of tissue-type plasminogen activator is useful for preventing vasospasm even in the low dose of 0.1 mg.

[Evaluation of local treatment for unresectable primary liver tumor].

To ascertain the effect of local treatment for unresectable primary liver tumor, 59 patients were investigated retrospectively. Patients were classified into four groups; transcatheter arterial embolization (TAE) group, intermittent intra-arterial infusion chemotherapy group, combined therapy group (TAE+intermittent intra-arterial infusion chemotherapy) and a group without adjuvant therapy. The results revealed that TAE and intermittent intra-arterial infusion chemotherapy both prolonged the survival period. We found that the survival rate depends largely on the therapeutic method. No correlations were confirmed with the liver function nor stage grouping of the tumor. Therefore, we concluded that intermittent intra-arterial infusion chemotherapy is a beneficial treatment, considering its minimal adverse effect, broad indication and the fact that it does not require hospitalization.

[Effects of mianserin hydrochloride on delirium: comparison with the effects of oxypertine and haloperidol].

Mianserin (10-60 mg), haloperidol (2-6 mg) and oxypertine (20-60 mg) were administered once before bedtime in patients with delirium in Kurume University Hospital. The therapeutic effects of these drugs were investigated in the pre- and post-treatment using Delirium Rating Scale (DRS: Trzepacz et al., 1988). Mianserin was observed to be effective on 67% of the cases, and haloperidol and oxypertine in about 60%. Based on the clinical results and on the measurement of plasma concentration of mianserin, mianserin was considered to be an effective drug particularly against delirium in elderly patients because the effect and plasma concentration of mianserin were observed in the early stage of treatment without any side effects. The reduction of plasma free-MHPG was accompanied with improvement in the delirious state. The reason for the changes in plasma free-HVA was unclear.

[Endocrinological examination and its clinical significance in manic-depressive illness].

Neuro-endocrine test is one of the useful strategies in examining the pathophysiology of manic-depressive illness (MDI). Unfortunately, however, the pathophysiology of MDI has not yet been clarified and the specific biological markers of MDI have still not been found. In the present paper, a brief general survey as to main clinical points of endocrinological examination and its results towards the hypothalamic-pituitary-adrenal axis, the hypothalamic-pituitary-thyroid axis, the hypothalamic-pituitary-gonadal axis, growth hormone, melatonin, etc. are given and some mentions of biological markers in MDI are made.

[Sleep deprivation and continuous sleep treatment for depression].

We explained about sleep deprivation (SD) and continuous sleep treatment (CST) such as nonpharmacological approaches for depression. Total sleep deprivation (TSD) showed dramatic, but transient, improvement in severe depression following one or more nights of SD. We reported the effects of TSD on 8 depressives. Consequently, after the TSD, the patients were divided into the groups of 3 responders and 5 non-responders in the score of on the basis of a 50% reduction in the score of Hamilton Rating Scale. The minimum rectal temperature (RT) of the non-responders was significant higher on the TSD night compared to the baseline night, but no significant change was observed in the responders. In addition, the minimum nocturnal RT of the responders before TSD tended to be higher. Nowadays, it is not so popular to use CST for depression because the technique is difficult.

[The late-onset rapid cycling affective disorder].

166 depressive outpatients in Kurume University Hospital who were over sixty years old in 1993 were analyzed. Only 3 cases (1.8%) of the depressives were satisfied with the Dunner's criteria, which repeats affective episodes at least more than 4 times a year. Two of eight cases which were diagnosed the manic depressive illness satisfied the criteria. Most of the depressives were stable states and the depressive episodes did not change through one year. We presented a female case (aged 71), satisfying the DSM-3-R criteria for bipolar disorder. In this case, we monitored her polysomnography, and rectal temperature. Consequently this case showed remarkable changes in these parameters under manic, depressive and remissional phase, respectively. We conclude there are a few cases of the rapid cycling affective disorder over 60 years.

[Assessment of vascularity and permeability in brain tumor using SPECT and 99mTc-DTPA-human serum albumin in relation to 201Tl SPECT].

Single photon emission computed tomography (SPECT) using technetium-99m-DTPA-human serum albumin (99mTc-HSA-D) and thallium-201 chloride (201Tl) was simultaneously performed on 25 patients with brain tumors; 10 with brain metastasis, 8 with astrocytoma (Gr. 3) and 7 with meningioma. The early image was obtained 10 minutes after 99mTc-HSA-D (740 MBq) injection, and the delayed image was taken 5 hours after the injection. HSA-D index, based on the ratio of 99mTc-HSA-D uptake in the tumor versus the cortical area, was calculated on each image, and compared with Tl index (tumor/contralateral cerebrum ratio). HSA-D delayed index was significantly greater than HSA-D early index in all tumor types (p < 0.05 by the Wilcoxon ranked sign test). Linear correlation between HSA-D early index and HSA-D delayed index was significant in astrocytoma (Gr. 3) (p < 0.01) and meningioma (p < 0.001), and a linear correlation between HSA-D delayed index and Tl index was significant in astrocytoma (Gr. 3) (p < 0.05). It is concluded that HSA-D early index and delayed index could reflect tumor vascularity and permeability, respectively, and provide supplementary information for Tl index.

Assessment of postischemic reperfusion and diamox activation test in stroke using 99mTc-ECD SPECT.

To evaluate the cerebral distribution of 99mTc-ethyl cysteinate dimer (99mTc-ECD) at blood flow levels beyond the normal range, we investigated postischemic reperfusion and acetazolamide (Diamox) activation test in stroke patients. The postischemic reperfusion was studied in 10 patients who showed a postischemic hyperperfusion area on other single photon emission computed tomography (SPECT) studies using N-isopropyl-rho-[123I]iodoamphetamine ([123I]IMP), 99mTc-hexamethyl propyleneamine oxime (99mTc-HMPAO), or 133Xe. 99mTc-ECD SPECT demonstrated a hyperactive area in one case, an isoactive area in four, and a hypoactive area in five. Correlations with CT findings revealed hyperactive areas without any abnormality, isoactive areas with perifocal rim, perifocal edema, or diffuse cerebral edema, and hypoactive areas with an infarct core. The Diamox activation test was studied in eight other patients with atherothrombotic stroke, and a limitation in vasodilative capacity was classified into three grades: Gr. 0 (none to minimal), Gr. I (mild), and Gr. II (moderate). [123I]IMP SPECT showed Gr. II and limitation in all eight cases. However, 99mTc-ECD showed Gr. II in three cases and Gr. I in five, and 99mTc-HMPAO revealed Gr. II in two cases, Gr. I in three, and Gr. 0 in three. We suggest that a lack of retention of 99mTc-ECD in a postischemic reperfusion area indicates the severity of the initial brain damage. Although the limitation in vasodilative capacity under Diamox-activated conditions was underestimated using 99mTc-labeled CBF tracers as compared with [123I]IMP, a retention of 99mTc-ECD in the unaffected area with an increased CBF under Diamox activation could be relatively superior to 99mTc-HMPAO.

Effect of ethanol treatment on the activity and amount of stimulatory GTP-binding regulatory protein (Gs) in S49 murine lymphoma cells and human erythrocytes.

Treatment of wild-type S49 murine lymphoma cells with 50 mM ethanol for 24 hr caused a 14% decrease in the Gs activity (an ability to compensate the deficiency of stimulatory GTP-binding regulatory protein, Gs, in membrane from CYC-S49 murine lymphoma cells) and a 29% decrease in the amount of alpha-subunit of Gs, Gs alpha. The Gs activity also decreased by treatment of the cells with methanol, propan-1-ol and butan-1-ol with an increasing efficiency according to the carbon chain length of each alcohol. A metabolite of ethanol, acetate, showed no reduction in either the Gs activity or the amount of Gs alpha. The Gs activity in membrane preparations from S49 cells was stable in a solution containing 50-250 mM ethanol or 0.01-10 mM acetaldehyde. When human erythrocytes were treated with 50 or 100 mM ethanol under the same conditions used for S49 cells, the Gs activity did not decrease but rather increased slightly, without any change in the Gs alpha content. These results indicate that the reduction in Gs activity and in Gs alpha content depends on the altered metabolism of S49 cells that can be induced by ethanol itself. Human erythrocytes lack such a responsiveness.

Ambient glucose and aldose reductase-induced myo-inositol depletion modulate basal and carbachol-stimulated inositol phospholipid metabolism and diacylglycerol accumulation in human retinal pigment epithelial cells in culture.

Physiological hyperglycemia has been speculated to alter phosphoinositide (PPI; inositol phospholipid) signal transduction in cells prone to diabetic complications by two separate mass-action mechanisms with antiparallel putative effects on diacylglycerol (DAG): (i) sorbitol-induced depletion of myo-inositol leads to diminished PPI synthesis and turnover and DAG release, and (ii) elevated glucose-derived DAG precursors enhance de novo DAG synthesis. Because the first mechanism is mediated by aldose reductase (AR2), which converts glucose to sorbitol, the effects of glucose on basal and stimulated PPI signaling were explored in lines of cultured human retinal pigment epithelial cells differing widely in their basal AR2 gene expression and enzymatic activity. The results suggest that the effects of glucose on PPI signaling vary inversely with the level of AR2 activity and parallel the extent of AR2-induced myo-inositol depletion.

Adenosine A1 antagonists. 3. Structure-activity relationships on amelioration against scopolamine- or N6-((R)-phenylisopropyl)adenosine-induced cognitive disturbance.

The effects of a variety of adenosine A1 and A2 antagonists on N6-((R)-phenylisopropyl)adenosine (R-PIA)- and scopolamine-induced amnesias were investigated in rodents in order to clarify the role of adenosine receptors in learning and memory. Some of the selective adenosine A1 antagonists exhibited antiamnesic activities at several doses where they did not induce an increase of spontaneous locomotion. These results suggest that the blockade of A1 receptors is more important than that of A2 receptors in learning and memory. Detailed studies of structure-activity relationships of adenosine A1 antagonists in two amnesia models demonstrated that there were three types of adenosine A1 antagonists: (A) Compounds 3-5 (8-substituted 1,3-dipropylxanthines) ameliorated the shortened latency in both models. (B) Compounds 7-11 (8-substituted 1,3-dialkylxanthines) and 19-21 (imidazo[2,1-i]purin-5(4H)-one derivatives) ameliorated the shortened latency in the (R)-PIA-induced amnesia model but not in the scopolamine-induced amnesia model. (C) Compounds 14-16 ameliorated the shortened latency in the scopolamine model but not in the (R)-PIA model. Aminophenethyl-substituted compounds C did not exhibit adenosine A1 antagonism in vivo presumably due to rapid metabolism. The dramatic change in the activities of A and B could not be explained by their simple pharmacokinetic differences because both types of compounds showed clear blockade of central adenosine A1 receptors in the (R)-PIA model. 8-(3-Dicyclopropylmethyl)-1,3-dipropylxanthine (5) (KF15372) was chosen for further studies and is currently under preclinical development as a cognition enhancer.

Fully methylated oriC with negative superhelicity forms an oriC-membrane complex before initiation of chromosome replication.

In an in vitro assay, the oriC DNA has been shown to bind to the outer membrane fraction only when it is hemimethylated (G.B. Ogden et al., Cell, 54, 127-135,1988). In this report, however, we demonstrated that a significant amount of the oriC DNA was recovered from the cells just before initiation with the oriC DNA being fully methylated. Formation of this preinitiation oriC-membrane complex and following initiation of chromosome replication were strongly inhibited by novobiocin, a DNA gyrase B subunit inhibitor, which reduced the superhelicity of the reporter plasmid in the cells. On the other hand, both reactions proceeded in the presence of nalidixic acid, a DNA gyrase A subunit inhibitor, which did not have the effect of reducing the superhelicity. These results suggest that the negative superhelicity of the DNA is required for preinitiation oriC-membrane complex formation and following initiation event of replication.

Relationship between alcohol consumption and the activity of GTP-binding regulatory proteins in human erythrocyte membranes.

Activity of stimulatory GTP-binding regulatory protein (Gs) in human erythrocyte membranes was assessed by activation of adenylate cyclase in S49 murine lymphoma variant cells to elucidate a relationship to alcohol consumption. In apparently healthy subjects, alcohol consumption < 50 g ethanol per week did not alter the Gs activity, but it was significantly higher (14.3%, P < 0.05) in moderate drinkers (50-150 g/week) than non-drinkers. Then, the Gs activity declined with a further increase in alcohol consumption (150-550 g/week). Those subjects with drinking levels of > 50 g/week also showed significant increases in other alcohol-related markers, Na+, K(+)-ATPase and gamma-glutamyltransferase. The Gs activity was significantly low in alcoholics (a 34.9% reduction). No such reduction was noted in patients with other diseases. The results indicate that the Gs activity in erythrocyte membranes is an alcohol-related marker in humans. The variation of Gs activity is distinctive from those of other alcohol-related markers.
