[Preoperative assessment of aortic dissection by three-dimensional CT angiography].

In 21 patients with aortic dissection including 12 patients with acute aortic dissection, aortic lesions were evaluated preoperatively by newly developed three-dimensional CT angiography (3 D-CTA) using helical scan CT. Rapid and safe three-dimensional evaluation of aortic dissection was achieved with no complications. Three-dimensional structure of aortic dissection was well reflected in the image of 3D-CTA. Preoperative three-dimensional evaluation was useful to determine and proceed the operative procedure for the repair of aortic dissection. All patients were alive and results of surgical treatments were excellent. Intimal flaps and entries were visualized by multiple threshold display (MTD) method of 3D-CTA. Multiple threshold display method is an important three-dimensional reconstructive method and 3D-CTA may be a new useful and powerful diagnostic modality for aortic dissection.

[A case of liver metastasis of leiomyosarcoma of residual stomach successfully treated by TAE and intra-hepato-arterial chemotherapy with infusion-a-port with administration of epirubicin].

A 60-year-old man who had a partial gastrectomy for gastric cancer 10 years ago was referred to our hospital for further examination for liver tumor. He was diagnosed as leiomyosarcoma of residual stomach associated with liver metastases. After preoperative transarterial embolization (TAE) with epirubicin of the anterior branch of right hepatic artery, residual stomach was resected, and the reservoir catheter and infusion arterial port were implanted. He was treated by intra-hepato-arterial chemotherapy of epirubicin. The patient has remained well for 2 years and 3 months without apparent complications.

p-Aminohippurate transport in apical and basolateral membranes of the OK kidney epithelial cells.

p-Aminohippurate (PAH) transport in apical and basolateral membranes of OK cells was studied to characterize each membrane process and to determine which process is important for the vectorial transport of PAH across the OK cell monolayers. PAH uptake from the basal side into OK cells was much higher than from the apical side, and was inhibited almost completely by anion transport inhibitors (probenecid and furosemide) and low temperature. On the other hand, PAH efflux from OK cells to the apical side was higher than to the basal side. The efflux across the apical membrane also was inhibited by the transport inhibitors and low temperature. The uptake of PAH across the basolateral membrane was inhibited by dicarboxylates such as alpha-ketoglutarate, and kinetic analysis showed that the inhibition was a mixed-type. These findings suggest that PAH transport in apical and basolateral membranes of OK cells is a specifically mediated process, and both processes contribute to the vectorial transport of PAH across the cell monolayers. PAH/dicarboxylate exchange may be involved in PAH transport in the basolateral membrane of OK cells.

[Preoperative evaluation of a distal aortic arch aneurysm by spiral CT with three-dimensional reconstruction].

Sixty-seven-year-old male with a distal aortic arch aneurysm was evaluated by spiral CT with three-dimensional reconstruction. Spiral CT allows rapid data acquisition through an entire anatomic region during a single breath hold. The helically acquired data undergo interpolation. Transverse images are reconstructed with slice intervals 1-4 mm apart. Three-dimensional images were generated by these data with the transverse images. The entire anatomical correlation with the aneurysm and the aortic arch branches was clearly recognized by these three-dimensional images. Preoperative evaluation of the distal aortic arch aneurysm by spiral CT with three-dimensional reconstruction was useful for the surgical treatment.

[A case report of the preoperative diagnosis of thoracoabdominal aortic aneurysm by three-dimensional CT angiography].

A sixty-year-old woman with the thoracoabdominal aortic aneurysm (7 cm in the diameter) was evaluated by the newly developed three-dimensional CT angiography. The thoracoabdominal aortic aneurysm was scanned by spiral CT and three-dimensional images were generated preoperatively. The three-dimensional structures of the thoracoabdominal aorta with the aneurysm, the celiac trunk and the superior mesenteric artery were clearly visualized by this method. The thoracoabdominal aorta was replaced by the graft with the reconstruction of celiac trunk. The post-operative course was uneventful. The preoperative diagnosis of thoracoabdominal aortic aneurysm by this method was very useful in the determination of operative procedure in this case. Three-dimensional CT angiography may be the useful method in the preoperative diagnosis of the aortic surgery.

[Preoperative diagnosis of the thoracic aortic aneurysm by three-dimensional CT angiography].

Serial eight patients with thoracic aortic aneurysms were evaluated by a newly developed three-dimensional CT angiography (3D-CT) from December 1992 to January 1993. The patients include 3 aortic dissections, 3 aortic arch aneurysms, one descending aortic aneurysm and one thoracoabdominal aortic aneurysm. The surgical treatment was performed after the evaluation of 3D-CT, and the operative findings were compared to the three-dimensional images reconstructed by 3D-CT in all patients. Three-dimensional displays were achieved using the unique method of data collection of the helical (spiral) scanner with continuous tube rotation and continuous table feed. A intravenous contrast material was used to image the thoracic aorta and major aortic branches with the single-breath-hold technique. Two and three-dimensional images reconstructed by 3D-CT were displayed within 10-20 minutes after the scanning. These three-dimensional images of the aortic lesions could be displayed in any angle we chose. Three-dimensional structures of the thoracic aorta and major aortic branches were clearly visualized and easily recognized by 3D-CT. These images were similar to the intraoperative findings and were quite useful to determine the operative procedure. The successful repair of thoracic aortic aneurysm was achieved in all cases. 3D-CT is a new and attractive modality to assess the vascular system. Although our experience is limited, 3D-CT may be a useful and powerful diagnostic method for the surgical treatment of thoracic aortic aneurysm.

[A case of renal hemangiopericytoma].

A 27-year-old female was referred with an abdominal mass. Examination revealed a non tender firm mass in the right flank and hypertension (200/100 mmHg). An angiomyolipoma was suspected on computed tomography and arteriography and a radical nephrectomy was performed. On cut section, the kidney was occupied by a well-capsulated, grayish tumor measuring 10 x 9 cm. Pathological diagnosis was a renal hemangiopericytoma without involvement of the capsule. Her blood pressure has normalized after the operation. She has no evidence of recurrence after 18 months' follow-up.

Effects of age and body mass of recipients on survival of transferred embryos in mice.

To examine the effects of age and body mass of recipients on embryo survival during gestation, ICR embryos were transferred to nulli- or primi-parous females of 4 strains of mice. Out of 530 recipients, 383 (72.3%) produced a litter. Transfer of 3,465 embryos to 383 recipients resulted in production of 1,637 pups born alive, an overall rate of embryo survival of 47.2 +/- 1.1%. Regression analyses revealed that embryo survival decreased in older recipients and increased in heavier recipients. The results were consistent in each parity/strain group and all groups combined.

[Radiotherapy of uterine cervix carcinoma].

Seventy-eight patients with uterine cervix cancer were treated radically with standardized radiation therapy at Teikyo University Hospital in Tokyo from January 1979 to December 1985. The age of these patients ranged from 32 to 88 years old (average age 66.0). The pathology and the stage of them were 71 cases of squamous cell carcinoma (1 in stage I, 11 in stage II, 58 in stage III and 1 in stage IV) and 7 cases of adenocarcinoma (6 in stage III and 1 in stage IV). The cumulative survival rates for 5 years by Kaplan-Meier method were 71.5% for squamous cell carcinoma stage II, 47.4% for squamous cell carcinoma stage III and 0% for adenocarcinoma stage III. Radiation injury was studied by the grading system of Kottmeier-NIRS in Japan. The incidence of the injuries for grade 2 and 3 was 12.8% (10/78), and the items of those were rectal injury 5.1% (4/78) and sigmoidal colonic injury 7.7% (6/78). The results of survival rate were fair for the squamous cell carcinoma compared with the other reports but poor for adenocarcinoma stage III. Concerning the therapy for advanced adenocarcinoma of the uterine cervix, special consideration should be given for irradiation doses or infusion chemotherapy.

[Visualization of the renal venous system by renal arteriography with digital subtraction angiography].

The purpose of this study was to obtain vivid and precise images of intrarenal venous branching using DSA for renal arteriography. The type of system used was an ADAC DPS-4100C with 70-80 kVp, and 320 mA, 25-50 msec at 6 frames/sec. The duration was 10 sec and the matrix size was 512 X 512. In order to retain clear subtracted images of intrarenal venous branching with minimal noise the three frames were selected on the time-density curve of DSA: 1) a frame on which renal arteries disappear (frame X), 2) the frame with highest renal venous density value as (frame Y), 3) the difference of the nephrogram density between X and Y frames in which only the density of the nephrogram decreases (frame Z). The mask image is the image subtracted by the equation of the weighted averaging method on X and Z frame and is subtracted from the image of Y frame in the final step. By this method, 40 kidneys in 36 patients were studied, and the intrarenal venous branches up to the interlobar vein was clearly demonstrated in 28 of these cases (70%). This method is useful to estimate the extent of intrarenal lesions and detect abnormal renal blood flow compared with conventional temporal subtraction method.

Primary structure of chicken pituitary prolactin deduced from the cDNA sequence. Conserved and specific amino acid residues in the domains of the prolactins.

The perform of chicken prolactin (PRL) deduced from the cDNA sequence contains a signal peptide of 30 amino acid residues followed by a mature PRL of 199 residues. Chicken PRL shows 77, 68, 67, 58, and 31% identity of amino acid sequence with whale, human, ovine, rat, and salmon PRLs, respectively. Elucidation of the primary structure of avian PRL enabled extended analysis of the specific and conserved amino acid residues and domains of the PRL molecules. The mammalian, teleostean, and avian PRLs share 32 common residues, and these conserved residues are observed to cluster in four distinct domains (PD1 to PD4), corresponding to four of five conserved domains of the growth hormones. Of the 32 residues, 8 residues in the PD2 and PD4 domains, including 4 cysteines, are conserved by other members of the growth hormone family, which indicates that these 8 residues may be essential for common structural features of the gene family. On the other hand, 13 other residues distributed among all four domains are conserved almost exclusively in the PRLs, suggesting that these residues are indispensable for specific binding of the PRLs to their receptors.

[Improved contrast enhancement with a positive pressure drip infusion method and 350 mg I/ml high concentration contrast media in routine contrast enhanced CT].

Sixty cases of routine contrast enhanced CT were analyzed about the utilities of positive pressure drip infusion method using high concentration contrast media 350 mgI/ml, 100 ml. They were divided into three groups and each twenty cases were received positive pressure drip infusion method using Iohexol 350 mg/ml, 100 ml (350PP), positive pressure drip infusion method using Iohexol 300 mgI/ml, 100 ml (300PP) or conventional drip infusion method using Iohexol 300 mgI/ml, 100 ml (300DI). Enhancement effects of the liver parenchyma, the aorta and the spleen at upper, middle and lower levels of the liver were evaluated with increased attenuation on pre- and postcontrast CT. And intrahepatic contrast was evaluated with attenuation difference between liver parenchyma and intrahepatic vessels on postcontrast CT. In result, 300PP kept better enhancement effects and obtained better intrahepatic contrast than 300DI at the each level of the liver, and 350PP was still better than 300PP. The analysis of relationship between intrahepatic contrast and body weight suggested that 300PP was the optimal choice for the cases with 40-50 kg body weight and 350PP was for the ones with 50-60 kg as the infusion method of routine contrast enhanced CT. And the dose of contrast media seemed to be more needed for the cases over 60 kg body weight. 350PP and 300PP were concluded as useful, safe and simple methods for routine contrast enhanced CT of the liver.

Growth and reproduction of mice developed from bisected embryos.

Embryos from three groups of mice, ICR (I), a synthetic (S), and an F(1) hybrid from C57BL males and S females (F), were used to examine effects of embryo bisection on subsequent viability, postnatal growth and reproduction. In two experiments, Group S females used as recipients received the following combinations of whole (W) and/or bisected (B) embryos: W(I) and W(F), W(I) and B(F), B(I) and W(F), and B(I) and B(F) in Experiment 1 and W(I) and W(S), W(I) and B(S), B(I) and W(S), and B(I) and B(S) in Experiment 2. Eight to 12 embryos of both types were transferred surgically to two horns of the uterus. Overall survival rate of whole embryos (32.6%) was significantly greater (P < 0.001) than that for demi-embryos (13.2%). The gestation period after transfer was significantly longer (P < 0.05) for demi-embryos (17.5d) than for whole embryos (16.4 d). Mice developed from demi and whole embryos were not different in mean body weight at birth and at 21, 42 and 63 d of age. Fertility and litter size at first parity of mice that developed from bisected embryos were comparable with those that developed from whole embryos. Hence, bisection decreased embryo viability, increased gestation period of recipients but did not affect postnatal growth and reproduction.

Molecular cloning of cDNA encoding 20 kDa variant human growth hormone and the alternative splicing mechanism.

cDNA encoding the 20 kDa variant form of human growth hormone has been cloned, and its sequence analysis verified the alternative splicing mechanism for the mRNA synthesis. The cDNA sequence lacked 45 nucleotides corresponding to the sequence of 15 amino acids in the 22 kDa form of growth hormone. The cDNA clones for the 20 kDa variant hormone had the homogeneous 5'-ends, while the clones for the 22 kDa form showed a minor heterogenity, having two transcription initiation sites. The percentage of 20 kDa variant cDNA clones was approx. 7.7% of the total human growth hormone cDNA clones, consistent with the contents of 20 kDa hormone protein in human anterior pituitary and plasma.

cDNA cloning of human chorionic somatomammotropin-1 mRNA whose transcription was initiated at the 5' region of the TATA box.

cDNA of human chorionic somatomammotropin (hCS) mRNA, whose transcription had been initiated 26 nucleotides upstream from the ordinary TATAAA sequence, was cloned from term placenta. This variant cDNA clone was 56 nucleotides longer than the usual one at the 5'-noncoding region, consistent with the previous report that placental poly(A+) RNA contains longer transcripts probably directed by a CATAAA sequence located 5' to the TATAAA sequence (Selby, M.J., Barta, A., Baxter, J.D., Bell, G.I., and Eberhardt, N.L. (1984) J. Biol. Chem. 259, 13131-13138). The nucleotide sequence analysis revealed that the variant cDNA clones were derived form the hCS-1 gene but not from the hCS-2 gene. The variant clones accounted for approximately 9.7% of the total hCS-1 cDNA clones. In the 5'-flanking region around the TATAAA sequence, the hCS-1 gene showed a less stable dyad symmetry structure as compared with that of the hCS-2 gene.
