An approach to the management of the infertile couple in general practice is presented. Attention is paid particularly to those aspects of treatment which the interested practitioner may provide himself and the stage at which referral should be sought; insight is also given into some of the 'specialist aspects' of infertility.
Four pregnant patients with neurofibromatosis (Von Recklinghausen's disease) either developed hypertension during pregnancy or had an exacerbation of a pre-existing chronic hypertension. Two patients required early termination of pregnancy for severe hypertension; a third had severe intrauterine growth retardation resulting in intrauterine fetal death; and one had an otherwise uneventful pregnancy and a live healthy infant. Because of the association between neurofibromatosis and hypertension, patients with neurofibromatosis require special antenatal care and management.
The aetiology, pathological anatomy and management of the vesicovaginal fistula are reviewed. Closure of the vaginal route is preferred by most gynaecological surgeons and is suitable for low-lying fistulas. A suprapubic approach is more suitable for high or tethered fistulas and is favoured by urological surgeons. The importance of meticulous preparation and care in the management of this condition is stressed.
1 All enquiries received by the London Centre of the National Poisons Information Service [NPIS(L)] relating to household cleaning products were followed up by questionnaire for a 4-month period from November 1978 to February 1979. 2 130 reports (43% of total followed-up) were received. 3 The incidence of misuse of household cleaning products has remained largely unchanged since a less detailed survey was performed in 1974-1975. 4 The source of enquiries, age groups and products involved were similar to the earlier survey. 5 Sixty-two per cent of cases were asymptomatic and no serious or life-threatening reactions were reported. 6 Although an increasing number of patients were admitted to hospital, little treatment was needed and the use of gastric lavage and aspiration had declined markedly. 7 Household cleaning products in the UK still cause no serious poisoning when misused or accidentally ingested.

The authors found that marital coitus had ceased for a definable period (median=8 weeks) in one-third of a sample of 144 men and 221 women who were relatively young and had been married an average of 11 years. An analysis of factors related to the social background and marital relationship of the respondents indicated that the cessation behavior of men is more highly predictable than that of women and that antecedents of this behavior differ markedly between the sexes. The authors suggest that, even among relatively young couples, marital intercourse is discontinous and problematic.

Ethological studies suggest that animal populations that live in crowded conditions display a number of behaviors that tend to limit the size of the population, such as aberrant forms of sexual behavior, small litter sizes, a higher incidence of spontaneous abortion, ineffectual maternal care, and even cannibalism of their young. Studies of household crowding in North America cities have produced only modest and selective evidence that crowding has similar effects among humans. In this paper, we examine the effect of household crowding on marital sexual relations, on desire for additional children, and on fetal and child loss in Bangkok, Thailand, a city with a much wider range of household crowding than is typically found in North America. In spite of the wider range, and higher mean level of crowding, we find that both the objective and subjective dimensions of crowding have only modest selective effects on sexual and reproductive behavior.

The loss of physician autonomy, the changing shape of physicians' practices, and efforts to control the cost of health care have left American physicians increasingly dissatisfied with the U.S. health care system. A survey of 300 office and hospital-based physicians shows 59 percent favor reform of the U.S. system; only 31 percent favor retaining the current system. Doctors face increased competition for patients (the supply of physicians has increased three times faster than the population), reduced autonomy because of intervention by government and other third party payers, pressure from patients to provide unnecessary care including expensive new technology, and increased cost containment. Yet a majority of physicians believe the causes of rising health care costs are patient demand for services and the current medical malpractice system. A minority (23 percent) blame hospitals and physicians for rising costs.
