High plasma levels of endothelin-1 and atrial natriuretic peptide in patients with acute ischemic stroke.

The plasma levels of endothelin-1 (ET-1) and atrial natriuretic peptide (ANP) have been measured in 37 patients with acute ischemic stroke, on admission and 3 and 7 days thereafter. The plasma ET-1 levels at the onset of symptoms were about two-fold those observed in age-matched normal volunteers (3.5 +/- 2.26 v 1.54 +/- 0.9 pg/mL, respectively; P < .001). These levels remained significantly elevated during the 7-day study period. The neurologic deficit was assessed daily by Mathew's modified scale (MS). A significant correlation was found between neurologic status on admission and ET-1 plasma values; patients with worse neurologic status (MS < 45 points) had higher ET-1 plasma values than those with better neurologic status (MS > 45 points) (5.4 +/- 2.34 v 3.05 +/- 2.04 pg/mL, respectively, P < .05). The plasma ET-1 values did not correlate either with the site of the infarction or with its primary cause (cardioembolic, lacunar, or atherothrombotic). No significant differences were seen in plasma ET-1 concentrations between patients who eventually died and those who survived the acute event. The plasma ANP were about 18-fold higher in ischemic stroke patients on admission than in controls at admission (110.9 +/- 29.5 v 5.84 +/- 3.96 pg/mL, respectively, P < .01). These values remained significantly elevated on days 3 and 7. There was no correlation between the ANP plasma values and the neurologic status, the site or mechanism of the stroke, or the plasma ET-1 levels. In conclusion, ischemic stroke is associated with marked acute and long-duration increases of ET-1 and ANP.(ABSTRACT TRUNCATED AT 250 WORDS)

Necrotizing sialometaplasia (adenometaplasia) of the trachea.

Necrotizing sialometaplasia is a benign condition first described in minor salivary glands of the soft palate with morphological changes which can be misinterpreted as squamous-cell carcinoma. Similar lesions have been subsequently reported in other locations including major salivary glands, lip, breast and skin (the term syringometaplasia has been applied for the latter). We report three cases of such a process involving submucosal glands in the trachea following prolonged translaryngeal intubation.

Endobronchial hamartoma. Report of 7 cases.

Seven cases of endobronchial hamartoma are presented. Preoperative fiberoptic bronchoscopic biopsy provided the histologic diagnosis in two cases, but was inconclusive in the others. The tumor was removed by endoscopic resection (2 cases), local resection via a bronchotomy (1), lobectomy (2) or pneumonectomy (1). These six patients are alive and well 2-5 years postoperatively. The seventh patient refused treatment.

Effects of sire of fetus upon subsequent milk production and reproduction of Jersey cows.

Estimates of variation associated with sire of fetus effects were obtained from 14,147 Jersey records that originated in four southeastern states. After adjustment for age of cow and length of record, the complete model included sire of fetus, sire of cow, herd-year-season effects, and effects of all possible interactions. Deletion of some or all interactions was necessary for mixed model analyses. Completely random models yielded unrealistically high estimates for sire of fetus variances for milk and fat yields and for fat percent, as had previous research. Mixed model analyses suggested that sire of fetus effects were small, albeit real and measurable. As a portion of total variability they accounted for: in milk and fat yields, 1 to 2%; fat percentage, 1 to 4%; days open, 0 to 1%. Gestation lengths were affected by fetal sire, regardless of whether the model was random or mixed; variances were 9.3 to 13.6% of the total. Preliminary estimates of interactions involving sire of fetus, sire of cow, and herd-year-season suggested that they are not zero and warrant further study.

The ultrastructure of the epicardium epithelium of the slug, Arion ater (Linnaeus, 1758), (Pulmonata, Stylommatophora).

In the present paper we investigated the fine structure of terrestial slug Arion ater (L.) Interacisternal microtubules were formed in Arion ater, however they do not correspond to interacisternal polycylinders because they differ in size and they are not continuous with the membrane cistern. From our study it may be concluded that these intracisternal microtubules suggest an accumulation of protein.

Effects of retained fetal membranes on milk yield and reproductive performance.

Effects of retained fetal membranes on subsequent productive and reproductive performance were measured in a single Florida Holstein dairy herd (n = 216 retained fetal membranes and 208 normal cases). Mean performance was peak daily milk yield, 31.9 kg; 305-d milk yield, 6317 kg; days to first service, 60; days open, 115; and services per conception, 2.3. Yield was unaffected by retained fetal membranes; however, retention resulted in an increase of 4 d to first service, 19 d open, and .2 services per conception. Detrimental effects of parturitions in hot weather (July through September) compared with cool (December through February) were peak daily milk yield, 5.5 kg; 305-d milk yield, 512 kg; days to first service, 3; days open 39; and services per conception, .9. Interactions could not be detected between diagnosis (retained or normal) and season for any response. Sex of calf effects could not be detected for any response. Continued research to reduce occurrence of retained fetal membranes or to obviate its effects should lead to improved reproductive performance.

Lung cancer and pleural effusion. Clinical significance and study of pleural metastatic locations.

We reviewed results of 78 diagnostic thoracoscopic examinations of patients with lung cancer and homolateral pleural effusion. The study stresses the main locations of pleural metastasis in cases of lung cancer and correlates these data with the pleural spread from extrathoracic primary neoplasms. Also pointed out is the operability of the present series and the different behavior in pleural metastatic spread according to the histologic type with correlation between gross appearance of the effusion and the cytology.

Points to consider when choosing a biopsy method in cases of pleurisy of unknown origin.

Blind pleural needle biopsy and diagnostic thoracoscopy are procedures sometimes used in the work-up of a patient with pleural effusion of unknown origin. We reviewed 203 diagnostic thoracoscopies in patients with malignant pleural effusion to show the different location of pleural metastasis, some out of the reach of blind needle biopsy. Based on these data, we find diagnostic thoracoscopy a superior procedure because of its higher reliability, faster diagnostic results, slight or no complications, and the possibility of carrying out pleurodesis in the same examination.

Intracisternal polycylinders: a cytoplasmic structure in cells of the terrestrial slug Arion empiricorum Férussac (Pulmonata, Stylommatophora).

Study of the digestive organs of the slug Arion empiricorum with the electron microscopy has revealed cytoplasmic structures that we call intracisternal polycylinders (ICPC). They consist of cylinders of cytoplasm (about 550 A in diameter) arranged in sheafs within cisterns of the endoplasmic reticulum. They appear in different cell types, being most common in the digestive epithelium of the midgut. Their morphology and apparent association with other cytoplasmic organelles such as mitochondria, peroxisomes, multivesicular and residual bodies suggests that the ICPC might be involved in exchange, transport and oxidation processes, contributing to the excretory function at a subcellular level.
