[Simultaneous pancreas-kidney transplantation. First cases in Chile].

We report two insulin dependent diabetic patients with a past history of 21 and 30 years complicated with retinopathy, neuropathy and nephropathy with arterial hypertension and kidney failure. Simultaneous pancreas-kidney transplantation was done 8 and 18 months after starting hemodialysis, performing a double intraperitoneal implant with pancreato-duodeno-vesical anastomosis and contralateral kidney grafting with uretero vesical anastomosis using antireflux techniques. In the second case, a second kidney transplant from the same donor was needed, due to a thrombosis of renal vein. There was one rejection episode in each case but renal or pancreatic function was not impaired. Other observed complications were metabolic acidosis and hyperkalemia due to urine loss of bicarbonate, moderate arterial hypertension and bacterial and fungal infections. There was a graft dysfunction due to the association of vancomycin and cyclosporin. In conclusion, in the two presented patients, simultaneous pancreas-kidney transplantations were successful and they remain free of insulin or dialytic therapy 4 and 9 months after the operation.

Inhibition of sexual behavior in male mice infected with Taenia crassiceps cysticerci.

Prominent estrogenization and deandrogenization ensue in male mice as a consequence of experimental intraperitoneal infection with Taenia crassiceps cysticerci. The impact of these endocrine changes upon sexual behavior was explored in a group of infected Balb/c male mice at weekly intervals for 15 wk and compared with the behavior of otherwise paired, nonparasitized male mice. Mounting, intromission, and ejaculation responses markedly declined as infection progressed. Six weeks after infection, none of the infected mice displayed ejaculation, the number of mounts and intromissions gradually decreased, and their latencies increased, until, by the 13th wk, none of the parasitized mice showed any sexual response toward female mice. Fifteen weeks after infection, the number of metacestodes per host increased to a couple of thousand, the mean serum estradiol level was approximately 50 times higher than the normal value, and testosterone fell to 5% of its normal level. To fully assess that the inhibition of sexual behavior resulted from the decrease in testosterone levels, a group of 8-wk-infected mice received testosterone, and complete restoration of their sexual behavior was observed. Inhibition of masculine sexual behavior during the infection period is the result of hormonal changes, estradiol being ineffective in maintaining copulation.

Efficient plasmid DNA replication in Xenopus egg extracts does not depend on prior chromatin assembly.

Small plasmids replicate efficiently in unfertilized Xenopus eggs provided they are injected before rather than after activation of the cell cycle. Here we use Xenopus egg extracts to test the hypothesis that efficient replication results from chromatin assembly prior to activation giving preloaded plasmids a head start toward the formation of a replicating pseudonucleus (Sanchez, J.A., Marek, D., and Wangh, L.J. (1992) J. Cell Sci. 103, 907-918). As in ovum, plasmid DNA preincubated in unactivated egg cytoplasmcytostatic factor extracts) replicate more efficiently after extract activation than does the same DNA added to the same extract after activation. Unlike in ovum, however, plasmids that replicate efficiently in vitro do not assemble into chromatin during preincubation and become topologically knotted instead. But even DNA knotting does not explain subsequent efficient replication. Also, plasmids preassembled into chromatin in vitro do not replicate efficiently in activated egg cytoplasm unless first preincubated in a CSF extract. We conclude that unactivated eggs contain replication-enhancing activities that can act independently of plasmid chromatin assembly and DNA topology. These postulated "preloading" factor(s) may be related to licensing factor, an activity that controls initiation of DNA replication in eukaryotic cells. The experimental conditions described here will permit characterization of preloading/licensing factor(s) in the context of a small plasmid substrate.

Phosphorylation of elongation factor-1 (EF-1) by cdc2 kinase.

Elongation factor-1 (EF-1) is a major substrate for cdc2 kinase in Xenopus oocytes. The guanine-nucleotide exchange factor EF-1 beta gamma delta, appears to have a highly complex macromolecular structure containing several GTP/GDP exchange proteins, valyl-tRNA synthetase, and a putative anchoring protein EF-1 gamma. During meiotic cell division, the factor becomes phosphorylated by cdc2 kinase, not only on EF-1 gamma, but also on two different phospho-acceptors on EF-1 delta. Phosphorylation is concomitant with changes in protein synthesis in vivo. Xenopus oocytes, and potentially all cells, contain a multitude of heteromeric forms of the complex which postulates that EF-1 beta gamma delta is not a "house keeping" factor but a sophisticated regulatory element.

Sex hormone changes induced by the parasite lead to feminization of the male host in murine Taenia crassiceps cysticercosis.

Female mice are more susceptible to Taenia crassiceps (TC) infection than males. However, after a month parasite load increases massively in both genders reaching thousands of parasites per host. The possibility of hormonal changes in the infected mice was envisaged. Sex hormones levels were assayed after different periods of infection, the parasites present in the peritoneal cavity were collected and gonads, uterus and seminal vesicles were weighed. In male mice, serum estradiol increased to levels 200 times their normal values whilst those of testosterone decreased 90% relative to controls. The weight of seminal vesicles was significantly diminished. Infected female mice also showed a slight increase in estrogen blood levels after 8 weeks of infection and the weight of the uterus was significantly increased relative to controls. Serum estradiol and testosterone were almost undetectable after gonadectomy. Cytokines such as IL-6 are capable of stimulating aromatase activity and we found that splenocytes from infected mice produced amounts of IL-6 higher than control as measured by ELISA. In conclusion T. crassiceps infection triggers a feminization process in the infected hosts. The gonads are required for the parasite to induce higher estrogen synthesis. IL-6 could be involved in the immunoendocrine mechanism used by the parasite to maintain a highly permissive environment for its rapid growth.

Islet cell antibodies and beta-cell function in gestational diabetic women: comparison to first-degree relatives of type 1 (insulin-dependent) diabetic subjects.

In order to further characterize women with islet cell antibodies (ICA) at the diagnosis of gestational diabetes (GDM), we aimed to compare titres and persistence as well as B-cell function with those of ICA+ first-degree relatives of Type 1 (insulin-dependent) diabetic subjects. Titres at detection of ICA were compared between 69 women with GDM and 53 relatives. Persistence of ICA positivity was investigated in 33 ICA+ gestational diabetic women and 39 relatives (mean follow-up: 13 months). Assessment of the acute insulin response, through an intravenous glucose tolerance test (IVGTT), was carried out in 9 ICA+ women with previous GDM and normal oral glucose tolerance, and their results were compared to those of a control group (9 women) and a group of 12 adult ICA+ relatives. In comparison with first-degree relatives, women with GDM had a higher ICA prevalence, especially with titres of < 20 JDF, and a similar persistence at follow-up. Women with ICA at diagnosis of GDM and normal oral glucose tolerance after pregnancy showed a decreased insulin response to glucose as compared to the control group. Results of the IVGTT closely paralleled those from adult ICA+ relatives. It is concluded that ICA+ gestational diabetic women share with ICA+ first-degree relatives of Type 1 diabetic subjects metabolic and immunologic disturbances.

TAP polymorphism in patients with Behçet's disease.

OBJECTIVE: To determine if susceptibility to Behçet's disease (BD) is associated with polymorphism of HLA-DRB1, HLA-DQB1, DQB1, and TAP1 and TAP2 genes. METHODS: Fifty eight Spanish BD patients and 116 ethnically matched unrelated healthy subjects were typed at the HLA-DRB1 and HLA-DQB1 loci using polymerase chain reaction/sequence specific oligotyping (PCR/SSO). TAP1 and TAP2 alleles were assigned using amplification refractory mutation system-PCR. RESULTS: TAP1C was absent in BD patients, but was found in 12.1% of control subjects (pcorr < 0.05; relative risk = 0.06). Additionally, a linkage disequilibrium between HLA-DQB1*0501 and TAP2B was observed in BD patients (delta = 0.095, pcorr < 0.02), but not in the control group (delta = -0.0031, p > 0.05). CONCLUSIONS: The complete absence of TAP1C alleles in BD patients may indicate that TAP1 polymorphism is not without some significance in the development of BD. Furthermore, the existence of a linkage disequilibrium between HLA-DQB1*0501 and TAP2B in our patients suggests that the gene conferring susceptibility for BD is inherited as an extended haplotype in the population studied.

Elongation factor EF-1 delta, a new target for maturation-promoting factor in Xenopus oocytes.

A new physiological target for Cdc2 protein kinase has been identified. It corresponds to a protein EF-1 delta, a constituent of the nucleotide exchange factor EF-1 beta gamma delta, involved in the elongation step of protein synthesis. EF-1 delta is phosphorylated by Cdc2 kinase on threonine and serine residues. Threonine has been identified as Thr122 in the sequence VQVTPAAK. During oocyte maturation, Thr122 is phosphorylated at metaphase, when p34cdc2 is active. Phosphorylation studies revealed the presence of two post-translational regulated forms of EF-1 delta protein. Identification of two isoforms of the delta protein, together with the presence of two guanine-nucleotide exchange proteins (beta and delta) and physiologically regulated phosphorylation sites by Cdc2 kinase on gamma and delta proteins, implicate that EF-1 beta gamma delta exists in the cell under a multitude of macromolecular forms which suggests that EF-1 beta gamma delta is a sophisticated regulatory factor rather than a "housekeeping" element of the cell.

The leucine-zipper in elongation factor EF-1 delta, a guanine-nucleotide exchange protein, is conserved in Artemia and Xenopus.

Elongation factor 1, a complex involved in protein biosynthesis, contains two guanine-nucleotide-exchange proteins EF-1 beta and EF-1 delta. The sequence of EF-1 delta of Artemia was determined with the purified protein. When compared to EF-1 delta from Xenopus, a high degree of identify (80%) was found in the C-terminal domains of the proteins, which contain the guanine-nucleotide-exchange activity. The N-terminal domains share only 23% of the amino acids at identical positions, and therefore they were further analysed for less obvious types of homology. To this end, a published approach for sequence analysis, which can detect peculiar amino acid patterns in proteins was applied. In this way, a weak albeit unmistakable similarity between the two EF-1 delta proteins was demonstrated in the region of the leucine-zippers, apart from the leucine repeat itself. Apparently, they display a common structural pattern in their N-terminal domains, which so far has been observed mainly in transcription factors.

Mandibular infected buccal cyst: case report and review of literature.

A rare lesion of unknown etiology, the mandibular infected buccal cyst, is associated with the buccal surfaces of vital mandibular first molars in pediatric patients. The exact histogenesis is uncertain, but it is believed to be an odontogenic cyst of inflammatory origin. A well-documented example of this rare entity is presented here.

cdc2 kinase sets a memory phosphorylation signal on elongation factor EF-1 delta during meiotic cell division, which perdures in early development.

EF-1 delta is a physiological substrate for cdc2 protein kinase in Xenopus oocytes. The protein is part of the nucleotide exchange factor EF-1 beta gamma delta, involved in the elongation step of protein synthesis. We show that EF-1 delta exists under four isoforms in the prophase oocyte, all phosphorylable by casein kinase II. Each of the prophase isoforms was further separated into a 36 and a 38 kDa form upon phosphorylation by cdc2 kinase which therefore reveals the existence of eight different isoforms. Phosphorylation by cdc2 kinase can be monitored as the electrophoretic mobility dedoublement 36/38 kDa. Developmental regulation of EF-1 delta was analyzed. The cdc2 kinase-induced change occures at meiotic division, after complete oogenesis and perdures during early development. It is therefore a phosphorylation memory signal for early development.

The human leucine zipper-containing guanine-nucleotide exchange protein elongation factor-1 delta.

Copy-DNA clones containing the complete coding region of the human elongation factor-1 delta (EF-1 delta) mRNA have been isolated and characterized. We present the deduced amino acid sequence and observe in it a leucine zipper motif seen recently in EF-1 delta from Artemia and Xenopus laevis. The human EF-1 delta sequence shows a strong conservation in its C-terminal domain. The homology between the N-terminal domains of EF-1 delta proteins is low and almost exclusively limited to the leucine zipper motif.
