BACKGROUND: Reversal of ischemia after myocardial infarction by revascularization is worthwhile only if viability exists in a sufficiently large portion of the left ventricle. METHODS AND RESULTS: To determine myocardial hypoperfusion reversibility and its influence on segmental and global function, we studied 50 patients after myocardial infarction. Three technetium 99m-tetrofosmin scintigraphies were performed: 1 at rest, 1 after 0.6 mg sublingual nitroglycerin (NTG), and 1 after injection at peak stress. First-pass multigated radionuclide angiography was obtained at rest and after NTG. Each patient also underwent a stress redistribution-reinjection thallium-201 scintigraphy. During stress 99mTc-tetrofosmin, 104 segments had normal uptake, 51 showed moderately reduced uptake, and 186 had severely reduced uptake. Of these 186 segments, 33 (18%) improved at rest, and 41 (22%) improved only after NTG. Fifty-nine (79%) of these segments with improved uptake were also found to have reversible defects on 201TI imaging. In the 26 patients with ventricular dysfunction, a 73% agreement was found between the functional and 99mTc-tetrofosmin uptake post-NTG improvement, whereas a 69% agreement was found with thallium reinjection. No significant differences were seen between 99mTc-tetrofosmin and 201T1 imaging. CONCLUSION: Nitroglycerin administration during 99mTc-tetrofosmin scintigraphy improves the detection of myocardium with reversible hypoperfusion in patients with a previous myocardial infarction.
Methicillin-resistant Staphylococcus aureus (MRSA) infection is a well recognised problem, especially in vascular surgical patients with synthetic bypass grafts. This is to our knowledge the first report in the literature of the development of anastomotic false aneurysms at both ends of an autologous vein graft, as a result of MRSA infection within the vascular wall.
PURPOSE: To compare the diagnostic abilities of the standard bracketing strategy (BR) and a fast strategy, the tendency-oriented perimetry (TOP). METHODS: Seventy-seven controls and 91 eyes from patients with glaucoma were analyzed with the strategies TOP and BR. Sensitivity (Se), specificity (Sp), the area under the receiver operating characteristic (ROC) curve (AC) and the optimum cutoff value (CO) were calculated for the visual field indices mean defect (MD), the square root of the loss variance (sLV) and the number of pathological points (NPP). RESULTS: In the glaucoma group, the mean MD value using TOP and BR was 7.5 and 8.3 dB, respectively. The mean sLV value using TOP and BR was 5.0 and 5.3 dB, respectively. Indices provided by TOP had higher ROC values than the ones provided by BR. Using TOP, the index with the best diagnostic ability was sLV (Sp = 94.8, Se = 90.1, AC = 0.966, CO = 2.5 dB), followed by NPP and MD. Using BR, the best results were obtained for MD (Sp = 92.2, Se = 81.3, AC = 0.900, CO = 2.5 dB) followed by sLV and NPP. CONCLUSIONS: A fast strategy, TOP, had superior diagnostic ability than the standard BR. Although TOP provided lower LV values than BR, the diagnostic ability of this index was higher than that of the conventional strategy.
PURPOSE: We present the clinical, genetic and histopathologic findings in two siblings with Muscle-Eye-Brain Disease (MEB-D), an autosomal recessive disease characterized by mental retardation, muscular dystrophy, retinal hypoplasia and brain abnormalities. METHODS: Clinical, histopathologic and gene mapping studies of a family with two normal and two children with MEB-D. RESULTS: Two siblings presented in the first few months of life with developmental delay, hypotonia, and strabismus. MRI of the brain showed colpocephaly, pontine and cerebellar atrophy, and diffuse white matter disease. Both patients were blind and had high myopia, strabismus, and retinal and optic nerve abnormalities. The older boy had glaucoma. Both children died from uncontrolled seizures. There was retinal, choroidal and RPE atrophy and optic nerve hypoplasia on ocular histopathology. Both patients shared the same parental haplotypes at the MEB locus on chromosome 1p, while an unaffected sibling did not, indicating possible linkage to the MEB locus. CONCLUSIONS: Patients with MEB-D have severe visual impairment from retinal and optic nerve hypoplasia. High myopia appears to be a consistent finding. The ocular manifestations of MEB-D appear to be distinct from those of patients with Walker-Warburg syndrome.
PURPOSE: The G1-TOP program is a short automated perimetric strategy which sub-divides the G1 grid of 59 points into four sub-grids. Each point is tested only once, but each patient's response is used to modify that particular point and the surrounding ones from the remaining sub-grids. This study compared the results of the G1-TOP program with the Standard Bracketing strategy. METHODS: Eleven participating institutions provided data from 213 patients (406 eyes). The main group consisted of 284 glaucomas and 55 glaucoma suspects. Other groups included 31 eyes with neurological disorders, 20 with chorioretinal lesions and 16 normal eyes. Mean age was 62.7 +/- 15.4 (range 14-88) years. All subjects had previous perimetric experience and visual acuity better than 0.5. Examination included G1-Standard Bracketing and G1-TOP testing, in interchangeable order, with the Octopus 1-2-3 perimeter. RESULTS: The correlation coefficient for mean defect (MD) was 0.95. Standard error (YX) for MD, square root of loss variance (LV) and individual thresholds were 1.86 dB, 1.29 dB, and 4.72 dB, respectively. Mean sensitivity values were similar (difference 0.04 +/- 1.87 dB) (p>0.05). Mean duration for G1-TOP was 2.19 +/- 0.26 min, while G1-Standard Bracketing took 11.51 +/- 1.52 min (ratio 1/5.1, or a net reduction of 80.4%). The sensitivity of G1-TOP versus G1-Standard Bracketing was: glaucoma 77.1/78.5, glaucoma suspects 38.2/47.3, neurological disorders 87.1/87.1 and chorioretinal lesions 80.0/85.0. CONCLUSIONS: The G1-TOP program gave very similar results to G1-Standard Bracketing in only 20% of the time required by the standard strategy.
PURPOSE: To evaluate the diagnostic capability of tendency oriented perimetry (TOP) in glaucoma. METHODS: A): The diagnostic accuracy of mean defect (MD), square-root of the loss variance (s LV), and number of pathologic points (NPP) was calculated in 295 normal and 414 glaucoma eyes (179 early, 112 moderate, and 123 advanced) examined with TOP. B): Threshold fluctuation (F) and its relationship with the loss variance (LV) was measured in 34 normal and 33 glaucoma eyes (mean MD=3 dB; SD=3.9) for TOP and for full-threshold perimetry (FT). C): Twenty-eight eyes with stable glaucoma (mean MD=9.5 dB; SD=7.2) were examined six times to quantify LV error. D): TOP and FT were tested with the simulation program PeriSim using different behavior models. RESULTS: A): The best diagnostic index in early glaucoma (MD<6dB) was sLV (specificity=90.2%, sensitivity=84.9). The three indices had similar precision in moderate and severe glaucoma. B): Threshold fluctuation and sLV were better correlated in TOP (r=0.72, p<0.01) than in FT (r=0.62, p<0.01). For normal subjects, in FT the incidence of F<2 dB was 8.82% and s LV<1.5 dB 5.88%. The same frequencies in TOP were 67.65% and 55.88%. C): Averaging six examinations reduced the sLV val ue by 22%. D): The threshold estimation error increased 1 dB in TOP in relation to FT for the same patient's behavior, but the error in TOP was lower than i n FT when the worst behavior was modeled. CONCLUSIONS: TOP is a good discriminator between glaucoma and normality. Perimetry results overestimate the real sLV value. TOP's high diagnostic ability is probably associated to the algorithm design and to less contaminating influences during the examination.

By administering acetyl-homocysteine and D-Penicillamine to the guinea pig, ageusia and hipogeusia respectively were obtained. Copper concentration was estimated in serum, saliva, liver, salivary glands, muscle, heart and tongue. A significant decrease in the concentration of this metal in the liver was observed and a slight increase in the rest (average values). The results were similar for both drugs. We believe that copper does not play any role in the taste process. The fact that this and other metals restore taste would indicate that those metals bind the SH groups of the drugs and that they are probably responsible for the ageusia.
The study area is characterized by a maritime Mediterranean climate, abundant presence of Chenopodiaceae in the vegetation, and a high incidence of pollinosis caused by the pollen of this family. A Hirst-type sampler was used to determine pollen concentrations in the air of the city of Huelva during 3 consecutive years (1995-1997). The total annual amount of Chenopodiaceae-Amaranthaceae pollen was between 10.59% and 6.28% of the pollen spectrum of the city, depending on the year. The annual pattern of variation in pollen concentration (5-day means) was very similar in the 3 years, and no statistically significant differences were found between years. This pattern is characterized by concentrations not exceeding 20 grains/m3 between April and the beginning of August, with an obvious seasonal variation in the second half of August and September, when the 5-day mean concentrations exceeded 40 grains/m3 and the daily maxima exceeded 100 grains/m3. This coincided with the flowering of most of the species in the group. The meteorological parameters with a statistically significant effect on daily pollen concentration during the pollen season (August 15 to September 20) included mean temperature and south wind (positive correlations) and relative humidity of the air (negative correlation). The highest intradiurnal concentrations were found between 10:00 a.m. and 5:00 p.m.

A case of phenothiazine-induced recrotizing colitis in a 41-year-old man is presented. A positive rechallenge was observed after introduction of another phenothiazine. The mechanism of this adverse drug reaction is discussed.
INTRODUCTION: The addition of typical and atypical antipsychotics in patients with obsessive-compulsive disorder (OCD) resistant to serotonin reuptake inhibitors (SRI) has been reported as a useful augmentation strategy. Although antipsychotic monotherapy has been associated with ineffectiveness and even increase of psychotic symptoms (especially in psychotic patients), antipsychotics as concomitant medications have proven to be effective in several case series and pilot clinical trials. The objective of this case series was to evaluate effectiveness of risperidone as add on therapy to current SRIs treatment in OCD refractory to treatment patients. MATERIAL AND METHOD: Risperidone add on therapy in moderate and severe treatment resistant OCD patients was reviewed. Case reports were patients fulfilling the following criteria: a) treatment follow-up of at least 12 weeks; b) SRI adequate doses, y c) Y-BOCS score higher than 16 score before starting treatment. A three month follow-up period was reviewed. Risperidone starting dose was low (mean 1.5 mg/day) and was increased following clinical criteria. Therapeutic response and tolerability were evalated with the following scales: Y-BOCS, CGI of change, UKU (neurological subscale) and spontaneous reported adverse events. Response criteria were the following: at least 35% of reduction in Y-BOCS from basal score and final score less than 16 and CGI-C "much improved" or "very much improved" (score 1 or 2). Intention to treat analysis was performed (patients who reported at least one risperidone dose and effectiveness measure). RESULTS: 31 patients had at least one effectiveness evaluation and 21/31 patients (67.8 %) were considered treatment responders. Mean risperidone dose was 3.8 mg/day. In general, risperidone was well tolerated: serious or unexpected adverse event were not reported. CONCLUSION: Risperidone as add on therapy to SRI in moderate-severe, refractory to treatment OCD patients, may be an effective and safe strategy.
