[Cancer of the testis in cryptorchidism. Apropos of 5 cases].

Five cases of cancer of the testicle in patients with cryptorchidism are presented. In one case the tumour occurred 16 years after cure for cryptorchidism at 15 years of age. In the other cases, the tumour developed on a testicle in an inguinal or intra-abdominal position. In two cases, it was impossible to remove the tumour which had developed on an intra-abdominal position. These patients died. These cases emphasize the importance of surgical cure in all cases of cryptorchidism, preferable before the age of 2 years, in order to improve functional prognosis and reduce the risk of cancerization. The gravity of cancer of the testicle in cases of cryptorchidism is often discovered too late.

[Takayasu disease. Diagnostic criteria and therapeutic procedure].

A study of 11 cases of Takayasu's disease, collected from the cardio-vascular surgical service of Sfax, has been realized during the 4 year-period from 1988 to 1991. Womanly predominance was neat. The average age was 40 years, with extremes of 24 and 56 years. Dominant revealing symptoms was upper limb ischaemia found in 9 cases, a reno-vascular hypertension rebelled to medical treatment was noted in 2 cases. Arteriography have showed a preferential localization of lesions in the subclavian artery essentially at the post-vertebral segment. Lesions types were dominated by stenosis and obliteration, then aneurysms were rare. Operative indication was posed in 9 cases. 11 revascularisations procedures were performed of which 10 arterial by passes and one resection graft. Two bypasses were obstructed, the 9 remaining bypasses have a good previous checked clinically and by angiography (or echo-doppler) with a mean follow-up of 14 months.

Osteoid osteoma of the spine: radiological aspects and value of arteriography. Four cases.

The authors present four cases of osteoid osteoma of the spine and review the usual clinical and paraclinical data leading to its diagnosis. The value of radionuclide bone scanning is emphasized: it supersedes the often disappointing radio-tomography and guides computerized tomography which, with modern scanners, virtually always demonstrates the lesion and locates it with accuracy. In the author's experience, spinal and spinal cord arteriography plays an important role: it identifies the radiculospinal vessels preoperatively and provides additional aetiological arguments based on the usual hypervascularity of osteoid osteomas and on the pain induced by injection of the feeding vessels. Data from the literature concerning the vascularization of the nidus suggest that treatment by embolization alone can be envisaged but must be demonstrated.

Spinal metastases of carotid paraganglioma. One case and review of the literature.

Paragangliomas of the carotid body are uncommon tumours usually regarded as benign or locally malignant. Metastasis occurs in 5 to 25% of the cases, involving mainly the liver, lung and bones. A case of carotid body tumour without histological evidence of malignancy but with local invasion and, subsequently, multiple spinal metastases is reported. Since the histological prognosis of the initial tumour is impossible to make in most cases, the authors underline the importance of signs of local invasion which is predictive of metastasis and requires regular monitoring with radionuclide bone scanning and MRI.
