Lymphocyte transformation in newborns, women and those who had used oral contraceptives.

Lymphocyte transformation, utilizing the mitotic index parameter has been assessed in newborns' cord blood, adults and women who had been on oral contraceptives and their babies. The results show that the newborns' cord blood response is significantly higher to PHA when compared with the respective adults' response. No significant difference was observed between the women taking oral contraceptives and their respective controls.

A cytogenetic study in women who had used oral contraceptives and in their progeny.

A study has been made of the chromosome numbers and characteristics of leucocytes cultured from mothers (a total of 77) who had taken one or more types of oral contraceptive, and their babies (108), and compared with similar numbers of control subjects. When the total cytogenetic results from test and control mothers were compared, no significant differences in frequency or type of chromosome abnormalities was observed. However, with certain types of oral contraceptive, minor differences in the numbers of chromosome abnormalities were found between test and control groups and the significance of these findings are discussed. The only significant abnormalities observed in the babies from mothers taking oral contraceptives were slight increases in the numbers who had cells with less than 46 chromosomes and who exhibited chromosomal gaps. The number of abnormal cells was significantly higher in babies born to mothers who had been taking norethynodrel (Conovid) (4.6% abnormal cells in test babies; 2.0% in controls).

Humoral immunity to a metabolite of halothane, fluroxene, and enflurane.

Trifluoroacetate, a common metabolite of halothane, fluroxene, and enflurane, conjugated to guinea-pig albumin elicits specific serum antibody in guinea pigs. Two classes of antibodies were found: hemolytic, gamma-2, and anaphylactic, gamma-1. Repeated injections of the antigen, trifluoroacetyl-guinea pig albumin, often led to disappearance of circulating antibodies.

Dobutamine: development of a new catecholamine to selectively increase cardiac contractility.

We systematically modified isoproterenol's chemical structure to reduce chronotropic, arrhythmogenic, and vascular side effects. Experiments on dogs showed that the resulting drug, dobutamine, had an inotropic efficacy as great as that of epinephrine due to a direct action on beta1 cardiac receptors. However, unlike epinephrine, dobutamine's effect on alpha and beta2 vascular receptors was slight. At equivalent inotropic doses, dobutamine had less than a fourth of the chronotropic effect of isoproterenol. Desmethylimipramine (DMI), which blocks the sympathetic nerve fiber uptake mechanism, had no effect on dobutamine's actions. In contrast, DMI antagonized dopamine's inotropic effect, and marked chronotropic and pressor responses occurred when we used doses of dopamine large enough to elicit a direct inotropic effect. Dobutamine increased the contractility of isolated cat papillary muscles more but the automaticity less than did isoproterenol. In ischemic dog hearts, dobutamine lacked significant arrhythmic activity, whereas dopamine, norepinephrine, and isoproterenol caused severe ectopic activity. In dogs with experimentally induced low cardiac contractility, low cardiac output, and hypotension, dobutamine produced dose-related increases in cardiac contractility and output, restored arterial blood pressure, and reduced total peripheral resistance slightly. In contrast, isoproterenol failed to restore blood pressure, had only a meager effect on cardiac contractility and output, cuased extreme tachycardia, and lowered peripheral resistance more than did dobutamine. Norepinephrine, which did not increase cardiac contractility or output as much as dobutamine, excessively elevated peripheral resistance and arterial blood pressure.

The role of valve replacement in the treatment of fungal endocarditis.

The role of valve replacement in the treatment of fungal endocarditis has not been defined. The diagnosis of fungal endocarditis was made in 11 patients, 8 before death and 3 at autopsy. Nine patients were infected with Candida species, one with Torulopsis glabrada and one with Aspergillus. None of the 8 patients whose disease was diagnosed during life was cured despite up to 160 days of amphotericin B therapy. All required valve replacement for heart failure (1 patient), emboli (1 patient, or both (6 patients). We recommend early valve replacement for fungal endocarditis before the onset of heart failure and emboli.

Suicide behavior: community attitudes and beliefs.

A survey of community attitudes and beliefs concerning suicidal behavior is reported, in which women from two suburbs differing widely in their rates of hospital treated self-poisoning and self-injury were interviewed. Sympathetic attitudes to suicidal behavior and to those who engage in it were significantly more prevalent in the low-risk area. Community beliefs regarding lethality, etiology, and ability to distinguish suicide and "attempted suicide" are reported, and the relationship of these beliefs to attitudes is discussed.

Inhibition of mitosis and tritiated DNA precursor uptake in human leucocyte cultures by rat lymph node extract.

A water soluble extract was prepared from the lymph nodes of rats of different age groups. This extract, when added to human peripheral blood leucocyte cultures, inhibits DNA synthesis and mitosis when added in different concentrations for varying lengths of time in culture. Also, it was observed that the extract from old rats was not as effective as that from younger animals.
