Termination of 2nd and 3rd trimester pregnancies with mifepristone and misoprostol.

OBJECTIVE: The purpose of this study was to evaluate our use of the association of mifepristone and misoprostol for terminating second and third trimester pregnancies. STUDY DESIGN: One hundred and six patients undergoing termination of pregnancy between January 1993 and June 1995 in our center were studied. Each patient received 600 mg of mifepristone followed 24 h later by 400 microgrammes of misoprostol every 6 h. RESULTS: The average interval from the first administration of misoprostol to expulsion was 12.5 +/- 7.5 h (interval markedly decreased to 9.6 +/- 6.3 h in cases of intrauterine fetal death). CONCLUSION: The efficacy of the association of mifepristone and misoprostol is comparable with that of current regimens with grealer ease of utilization and at a much lower cost.

[Mifepristone and misoprostol in therapeutic abortions].

OBJECTIVES: To report our experience with the mifepristone and misoprostol combination in second and third trimester medically induced abortion. TYPE OF STUDY: Prospective study without control group between January 1993 and december 1994. STUDY SITE: Saint-Antoine Hospital, Paris. PATIENTS: Fifty-three patients admitted for a medically induced abortion. RESULTS: Induction-to-abortion mean time was 12.9 +/- 7.5 hours (time distinctly decreased in fetal death: 8.3 +/- 4.4 hours). CONCLUSION: The combination of misoprostol with mifepristone has equivalent with the other protocol used for therapeutic pregnancy termination, is less expensive (25 to 100 times) and finally is simple and non-invasive.

[Preimplantation diagnosis of delta F508 mutation of mucoviscidosis in transgenic mice].

OBJECTIVE: The aim of our study was to master preimplantation diagnosis in transgenic mice. METHODS: Stage 4 or 8 cells preembryos were collected from C57BL6xCBAF1 mice, three days after mating with transgenic C57BL6xSJLF1 mice homozygous for the human gene delta F508 mutation of cystic fibrosis. A single blastomere was sampled by microbiopsy and the preembryos transferred in Swiss mice foster mothers. Molecular biology for delta F508 mutation on a single blastomere was performed with a double PCR technique on a Perkin Elmer Cetus 9600. RESULTS: The success rate of biopsy on the 122 preembryos with 4 cells was 90.2%, and 93.8% on the 128 prembryos with 8 cells. The rate of in vitro hatching was 84.6% after biopsy on 52 preembryos with 4 cells (92.2% for controls without biopsy, p > 0.05), and 90.0% for 50 preembryos with 8 cells (94.4% for controls without biopsy, p > 0.05). The rate of birth after biopsy and transfer of 42 preembryos with 4 cells was 66.7% (72.5% for controls transferred without biopsy, p > 0.05) and 70.3% for 37 preembryos with 8 cells (71.9% for controls, p > 0.05). No difference was evidenced for births weight or organ weight at 3 weeks between mice born from biopsied embryos and controls. Thirty-two double PCR were performed for the diagnosis of the cystic fibrosis delta F508 mutation, 20 on a single blastomere obtained by microbiopsy and 12 for various negative controls, with 100% specificity and 100% sensitivity. CONCLUSIONS: Microbiopsy of preembryos with 4 cells or 8 cells does not alter their viability or further development. The double PCR technique on a single blastomere for the cystic fibrosis delta F508 mutation is reliable with our current methods of investigation.

Normal human polyspecific immunoglobulin G (intravenous immunoglobulin) modulates endothelial cell function in vitro.

We have analysed the in-vitro effect of pooled normal polyspecific immunoglobulin G (IVIg) and F(ab')2 fragments of IVIg on the secretion of prostacyclin (PGI2), thromboxane A2 (TxA2), and endothelin from cultured human umbilical vein endothelial cells. The stable metabolites, 6-keto-PGF1 alpha and TxB2, as well as endothelin, were measured by radioimmunoassay after extraction from the culture supernatants. IVIg inhibited TxB2 and endothelin secretion in a dose-dependent manner, but not that of 6-keto-PGF1 alpha. Therefore the ratio of 6-keto-PGF1 alpha/TxB2 increased (+600%). The effect of F(ab')2 fragments prepared from IVIg was similar to that observed with IVIg, indicating that the effect of IVIg was mediated by the variable region of immunoglobulin. The results suggest that part of the effects of IVIg in inflammatory vasculitis may be due to a modification of the PGI2/TxA2 ratio and inhibition of endothelin secretion, and that natural IgG antibodies may participate in the homeostatic process of endothelium under physiological conditions.

[Cervical pregnancy: conservative treatment with primary embolization of the uterine arteries. A case report. Review of the literature].

Ectopic pregnancies are rarely located in the cervix. In most (50-70%) of the cases, haemostatic hysterectomy is usually performed. We attempted an original approach for ectopic pregnancies located in the cervix which allowed preservation of the uterus. The uterine arteries are embolized before evacuation of the pregnancy and haemostasis of the loge with a balloon probe. In the literature, the incidence has been reported at 1 cases/20,000 births. Past history of curetage is a favouring factor. Echography facilitates diagnosis. When possible, conservative treatment may involve surgery (arterial ligature, endocervical haemostasis), drug therapy (methotrexate) and radiography (arterial embolization).

Nicardipine versus metoprolol in the treatment of hypertension during pregnancy: a randomized comparative trial.

OBJECTIVE: To compare the effects of treatment with nicardipine and metoprolol in patients with hypertension during pregnancy. METHODS: One hundred pregnant patients with mild or moderate hypertension followed at the Centre Hospitalier Intercommunal de Créteil (France) were randomly allocated to treatment with either nicardipine or metoprolol. Changes in maternal blood pressure (BP), laboratory indices, umbilical Doppler velocimetry, and neonatal outcome were compared by means of Student t test, chi 2 test, and analysis of variance. RESULTS: Nicardipine decreased maternal systolic and diastolic BP more than metoprolol (P < .001). Umbilical artery resistance was lower in nicardipine-treated patients (P < .001). Plasma uric acid and creatinine concentrations were increased less markedly in the nicardipine group (P < .05 and P < .01, respectively). The incidence of cesarean delivery for fetal distress was lower in the nicardipine group (P < .01). There was a trend toward higher birth weights in the nicardipine group but no significant difference in neonatal outcome. CONCLUSION: Nicardipine is more effective than metoprolol in decreasing maternal BP; neonatal outcome is not significantly different.

[Castleman's disease localized in the uterus or an inflammatory uterine pseudotumor. A case report and review of the literature].

The authors report a case of Castleman's disease localised to the uterus. The treatment was surgical removal of the lesion with conservation of the uterus. Though in localised forms the prognosis is always favourable, multicentered lesions can suggest lymphoma. In our case that we report the anatomo-pathological appearances were not characteristic and this has led the authors to think of a differential diagnosis such as an inflammatory pseudo-tumour of the uterus of which only two cases have been previously reported.

[The effect of peridural analgesia on uterine contractions].

OBJECTIVE: To study the impact of 0.25% bupivacaine on the frequency and intensity of uterine contractions. METHODOLOGY: 52 patients in labour. Continuous internal tokographic measurement of uterine activity at 10 minutes before instilling the bupivacaine and at 30 minutes after doing so. The true frequency and intensity of the uterine contractions were the two parameters that were studied. RESULTS: There was a significant variation (matched Student's test) between the two parameters that were studied: there was transitory hypokinesia (p = 0.003) and intense prolonged hyperkinesia (p < 0.001). CONCLUSION: 0.25% bupivacaine produces hyperkinetic increases in intensity of the contractions of which the mechanism has not been elucidated; therefore it is inadvisable to add oxytocics in the minutes after instilling bupivacaine.

Nicardipine treatment of hypertension during pregnancy.

OBJECTIVE: To assess the effects of nicardipine, a dihydropyridine calcium channel blocker, on the fetus and mother in hypertensive pregnant women. METHODS: Forty pregnant patients with mild or moderate hypertension received oral nicardipine 20 mg three times a day (mean duration of treatment 9 +/- 2.1 weeks). Twenty patients with severe preeclampsia (diastolic blood pressure greater than 110 mmHg and 24-hour proteinuria greater than 500 mg) received intravenous (IV) nicardipine at 2, 4, or 6 mg/hour according to body weight (mean duration of treatment 5.3 +/- 3.6 days). RESULTS: Oral nicardipine induced a significant decrease in both systolic and diastolic blood pressures compared with pre-treatment values (P < .001). Intravenous nicardipine decreased diastolic blood pressure below 90 mmHg, at least temporarily, in all 20 patients. Nine patients taking IV nicardipine complained of headaches. Doppler velocimetry remained stable with both oral and IV nicardipine. Neonatal outcome showed no deleterious effect of the treatment. CONCLUSIONS: Oral or IV nicardipine seems to be safe in hypertensive pregnant patients with the doses used in our study. Assessment of its efficacy will necessitate further clinical randomized trials.

Plasma assay of salbutamol by means of high-performance liquid chromatography with amperometric determination using a loop column for injection of plasma extracts. Application to the evaluation of subcutaneous administration of salbutamol.

An isocratic high-performance liquid chromatography method with amperometric detection for the assay of plasma salbutamol is described. The plasma extract is injected into the chromatographic system via a loop column. This insures the purification of the injected extracts and allows a simple and rapid liquid-solid extraction procedure. The good reliability, as shown by the low limit of detection (0.5 ng/ml) and a precision ranging between 5 and 10%, has permitted the investigation of a new mode of administration of salbutamol using a portable subcutaneous infusion pump. Our results show that subcutaneous administration yields plasma levels comparable with those obtained after usual intravenous doses.

[Prevalence of hepatitis C virus antibodies (tests ELISA 2 and RIBA 2) in a population of pregnant women in France].

We studied the prevalence of anti-HCV antibodies in a population of 2,367 pregnant women attending three public Parisian suburban hospitals. Of this group, 1,614 (68 percent) were French and 753 (32 percent) were immigrant women. The geographic origin of the immigrant population was North Africa (40 percent), West Africa (33 percent), Asia (14 percent), and South Europe (13 percent). Anti-HCV antibodies were tested by the Ortho ELISA second generation test. If present, the Ortho's four-antigen RIBA test and serum alanine aminotransferase determinations were done routinely. The overall prevalence was 1.73 percent. It was 1.55 percent in French women and 2.13 percent in immigrant women. Risk factors associated with anti-HCV were found in 68 percent of the anti-HCV positive French women and in 44 percent of the positive immigrant women. Risk factors were significantly more frequent in anti-HCV positive women in both groups. Among the 41 women with a positive ELISA test, 16/25 French women (64 percent) and 8/16 immigrants (50 percent) had a positive four-antigen RIBA test. Thus, the prevalence of a positive RIBA test was similar in both groups (0.99 and 1.06 percent), due to a higher proportion of false positive ELISA tests observed in the immigrants. These results show that, in French pregnant women: a) the prevalence of anti-HCV is twice as high as that found in blood donors and is a better estimation of the actual prevalence of these antibodies in France; b) positive ELISA 2 tests are most often confirmed by a positive RIBA 2 test.(ABSTRACT TRUNCATED AT 250 WORDS)

The prevalence of autoantibodies during third-trimester pregnancy complicated by hypertension or idiopathic fetal growth retardation.

Lupus anticoagulant, anticardiolipin, antinuclear, anti-deoxyribonucleic acid, antithyroglobulin, and antithyroid microsomal antibodies were assayed during third-trimester pregnancy (100 normal, 100 with complications). In spite of a normal activated partial thromboplastin time in all instances, lupus anticoagulant was further investigated by three additional procedures: tissue thromboplastin inhibition time, platelet neutralization procedure, and cephalin neutralization test. The prevalence of autoantibodies in pregnancies with hypertension reaches 16% (four with lupus anticoagulant, two with anticardiolipin, and two with antithyroid microsomal antibodies), which is significantly greater than that for idiopathic fetal growth retardation (2%) (one with lupus anticoagulant antibodies) and normal pregnancies (3%) (two with antithyroglobulin and one with autithyroid microsomal antibodies) (p less than 0.01). Autoantibodies were equally distributed between patients with gestational hypertension and those with preeclampsia. When compared with the 42 patients with hypertension and no autoantibodies, the eight patients with autoantibody had a more frequent history of fetal growth retardation (p less than 0.05), but there was no difference in the severity of hypertension, the frequency of obstetric complications, or the outcome of pregnancy. They did not require any specific treatment.

[Malignant mixed mesodermal tumor detected by uterine inversion. Apropos of a case. Review of the literature].

The authors report the anatomoclinical observation of an 82-year old patient presenting a mixed malignant mesodermal tumour (MMMT) revealed by a uterine inversion. The histological examination revealed sarcomatous and carcinomatous lesions with zones of chondroid metaplasia infiltrating largely the myometrium. A total vaginal hysterectomy, accompanied by radiotherapy, got the better of an early local relapse. The histogenetic hypotheses of MMMTs, the particular circumstances of the tumour revelation, as well as the methods of treatment are reviewed.

Computed tomography of the brain in eclampsia.

Computed tomography (CT) of the brain was performed on 44 women with eclampsia, 31 during pregnancy and 13 postpartum. All CT scans were done within 24 hours of eclampsia, including 12 within 1 hour of the convulsions and eight before the repetition of additional seizures, 2 minutes to 14 hours later. Control scans were performed on 15 hypertensive pregnant patients and on ten normotensive pregnant women after fetal death or just after delivery. Among the women with eclampsia, 26 CT scans were considered normal with no evidence of cerebral edema, three displayed signs of cerebral hemorrhage or thrombosis, six showed areas of focalized hypodensity located in the cortical lobes and the subcortical white matter, and nine were classified as cerebral atrophy with enlarged cerebral ventricles. We conclude that radiologic evidence of diffuse brain edema is probably not a common feature in eclampsia.
