Double-blind trial of oral carbuterol in bronchial asthma.

A double-blind comparison of 2 and 3 mg oral doses of carbuterol, a beta sympathomimetic drug with preferential selectivity for bronchial smooth muscle, was made with 25 and 35 mg doses of ephedrine sulfate over a six-month period in 26 patients with bronchial asthma. Spirometry was done before and three hours after administration of 3 mg of carbuterol or 35 mg of ephedrine sulfate at intervals of two weeks and one, four and six months. The data suggest a cumulative improvement in pulmonary function status with continued oral carbuterol use over the six-month period. However, the clinical effectiveness of the drug on pulmonary function was not clearly distinguishable from that of ephedrine in the present study.

Niridazole as an immunosuppressive agent.

The immunosuppressive properties of niridazole, an antihelminthic drug, have been investigated in rats. When given orally at a dosage of 50 mg/kg it extended the median survival of cardiac allografts from 7 to 20 days. The immunosuppressive effect was not increased by giving either azathioprine or prednisolone concurrently, but when all three drugs were combined the immunosuppression was profound, and only 2 of 8 grafts were rejected. Drug combinations incorporating niridazole at a lower dosage or for a shorter period were less effective, and azathioprine and prednisolone on their own or together prolonged graft survival only marginally in this model.

Double-blind one-year clinical study of fenoterol metered dose inhaler: preliminary results of a cooperative study.

As part of a controlled multi-center study 44 chronic asthmatics were treated for one year with either fenoterol (0.4 mg t.i.d. or q.i.d.) or isoproterenol (0.15 mg t.i.d. or q.i.d.) by metered dose inhaler. According to spirometric tests, fenoterol significantly improved pulmonary function until the end; isoproterenol became less effective after six months. Periodic global evaluations also gave no evidence of tolerance developing to femoterol. Side effects remained within acceptable limits.

Intracranial aneurysms: a clinical, angiographic and computerized tomographic study.

Fifty-six patients with 63 aneurysms were selected from a larger group of patients because they had at least one angiographic and one CT scan study. A total of 102 CT scans and 136 angiograms were analyzed. In five patients the aneurysm was visualized on the CT scan. There was a tendency for the location of the subarachnoid hemorrhage and intracerebral hematomas visualized on the CT scans to correlate with the site of the aneurysm as seen on the angiogram. The CT scan was useful in demonstrating the evolution of hydrocephalus and in differentiating hematoma from edema. The patients with normal CT scans were more likely to have good neurological grades. No single angiographic or CT scan feature correlated significantly with survival at lower than the 5 percent level.

Prenatal diagnosis of genetic disease in Canada: report of a collaborative study.

A study of 1223 amniocenteses carried out during 1020 pregnancies in 990 women showed that 2nd-trimester amniocentesis at about 16 weeks' gestation is a safe, accurate and reliable procedure for the diagnosis of certain classes of genetic disease when it is monitored by ultrasound, performed by a trained obstetrician and carried out in a major health sciences centre. The percentage of fetal losses (4.7%) and neonatal deaths (0.5%) during the study was not greater than in control samples for women 35 years of age and older. The best results were obtained when needles of gauge 20 or 21 were used. The use of needles of gauge 19 or larger and more than two insertions during a single amniocentesis were associated with a significantly greater frequency of fetal loss than a second or even a third amniocentesis during the same pregnancy. For 39 fetuses (3.8%) a diagnosis of a genetic abnormality was made and 23 male fetuses were found to be potentially hemizygous for an X-linked gene. There were 51 therapeutic abortions as a result of the diagnosis. Sixty-six tests (5.4%) gave an inconclusive result and seven (0.6%) gave an erroneous diagnosis; five of the latter (two false-positives and three false-negatives) resulted from the alpha1-fetoprotein test for neural-tube defects and in two cases the sex was incorrectly determined. The frequency of all chromosome abnormalities was 1:20 when the mother's age was 40 years or more and 1:60 when the mother's age was between 35 and 39 years. When a mother had previously had a child with a chromosome abnormality the risk of recurrence of such an abnormality was 1:100 when the age of the mother was 35 years or more.

Studies on the antimicrobial activity of amniotic fluid.

Sixty-one amniotic fluid samples from women in their second and third trimesters of pregnancy were examined for antimicrobial activity. Seventy per cent of the fluids were found to be active. The factor or factors responsible for this activity were present in low concentrations. The presence of spermine in the fluids accounted for some of the antimicrobial activity.

Amphetamine poisoning in dogs.

Amphetamine intoxication in dogs referred to the Veterinary Diagnostic Laboratory or the Veterinary Hospital of the University of Minnesota was characterized by excitement, agitation, hyperthermia, and convulsive episodes that could be confused with other convulsant poisonings. Extraction procedures on stomach contents or urine enabled indentification of the drug, using ultraviolet spectrophotometry.

Therapeutic efficacy of a new anhydrous theophylline dosage form.

A clinical study was performed on 41 asthmatic patients. After complete physical and spirometric examinations, they received capsules containing 200 mg anhydrous theophylline per capsule t.i.d. for three weeks. All other antiasthmatic medications were omitted during this study. The results showed that these capsules (Elixophyllin Capsules) produced statistically significant improvement in wheezing, dyspnea, global clinical assessment, vital capacity, and forced expiratory volume at 1 second. Side effects, which were generally mild and transient, were those usually reported for theophylline and were observed in 11 of the 41 patients. One patient dropped out of the study because of extreme nausea. It was concluded that theophylline as a single entity can provide significant improvement in asthmatic patients at a total daily dose of 600 mg.

Immunologic monitoring and aspergillosis in renal transplant patients.

Three cases of pulmonary aspergillosis in a "high risk" population of renal transplant recipients are presented. The source of infection was traced to the forced air exhaust system of the Transplantation Unit. Early definitive diagnosis of the infection was very important for effective management. Immunologic monitoring was demonstrated to be instrumental in indicating the early presence of infection, and as a guideline to reduced immunosuppression during therapy. Bronchoscopy with brushings and endobronchial cavitary biopsy were valuable methods for obtaining the infected tissue. Amphotericin B was effective when therapy was started early. Adequate levels of the drug were obtained by varying the dose and frequency of administration according to serum inhibitory titers. Control of infection was aided by immunologic monitoring at regular intervals.

Multidimensional locus of control in sociopolitical activists of conservative and liberal ideologies.

Three studies were conducted to examine the relationship between a multi-dimensional measure of locus of control and sociopolitical activism, controlling for the effects of political ideology. In Study 1, 98 male college students completed a measure of conservatism-liberalism. Kerpelman's Activism scale, and locus of control scales designed to differentiate between two types of externals: belief in powerful others versus belief in chance forces. In Studies 2 and 3, female college students (Ns=26 and 40) who differed in the extent of their participation in leftist political activities or feminist causes responded to the multidimensional locus of control scales. As predicted, results from analyses of variance and trend analyses indicate that for liberals, increases in expectancies of control by powerful others are positively associated with increases in activism, while for conservatives, there is a negative relationship. The importance of controlling for ideology and the implications of the differentiated view of externality for understanding social action are discussed.

The excretion of ibuprofen by the horse - a preliminary report.

The anti-inflammatory drug Ibuprofen [(+/-)-2-(p-isobutylphenyl) propionic acid] was estimated in the blood and urine of a horse using gas-liquid chromatography of the silylated derivative. Levels of the drug in the two body fluids were measured over a period of about 24 hours after administering a 12 gm dose of Ibuprofen. Plasma peak levels were observed within 30 to 60 min, and the drug was no longer detectable in the plasma by 8 hr. Urinary peak levels were observed 200 to 300 min after dosing, and the drug was no longer detectable in the urine by about 28 hr. It was observed that only 2% to 6% of the free unchanged drug was excreted in the urine.
