Automated serial ECG-analysis within an epidemiological study.

Since 1980 we have performed a cohort study in order to develop diagnostic and predictive algorithms for myocardial infarction. These algorithms shall be based on cardiovascular risk factors, ECG-measurements and serial measurement changes. An intermediate analysis of 5524 study participants describes observed measurement changes stratified by age and sex. The values thus obtained may serve for reference purposes and illustrate the influence of age and sex on intraindividual ECG-measurement changes. Specific suggestions are made for future program developments in order to overcome some present problems in serial ECG-analysis.

Matrix degrading proteinases from human granulocytes: type I, II, III collagenase, gelatinase and type IV, V-collagenase. A survey of recent findings and inhibition by gamma-anticollagenase.

Three human matrix degrading leukocyte proteinases, type I collagenase, gelatinase and a new type IV collagenase were isolated in latent and active form. Activation of all three latent enzymes could be achieved by treatment with either organomercurials or with trypsin. In addition the 90 kDa latent type I-collagenase could be activated by disulfides, while a newly discovered 70 kDa latent form could be activated with organomercurials or with trypsin. The active type I collagenase was inhibited by gamma-anticollagenase from human serum (and the leukocyte type I collagenase inhibitor, while the newly found type IV collagenase was inhibited only partially. The complexes formed from gamma-anticollagenase with type I collagenase, i. e. latent enzyme, are not reactive site associated complexes. The binding is not of a substrate-like and competitive manner. After inhibition of the enzyme though inactive against its natural substrates it is still hydrolyzing the synthetic low molecular weight octapeptide DNP-Pro-Gln-Gly-Ile-Ala-Gly-Gln-D-Arg-OH.

Compliance with salt restriction as a limiting factor in the primary prevention of hypertension.

It is an important but still unresolved question whether reduction of salt intake in the offspring of hypertensives (a high risk group) prevents the development of the disease. Therefore, 178 offspring (14-26 years old) of hypertensives were enrolled in a 2-year pilot trial aimed mainly at a reduction in salt consumption. For the intervention group (n = 99) a behavioural approach was chosen with extensive counselling by experienced dietitians. The controls (n = 79) received no continuous dietary advice. Both groups showed a small decline in sodium intake over time, but the differences between the two groups were not significant. Division into subgroups with and without sodium reduction revealed no differences in blood pressure. We conclude that the inherent resistance to any change of lifestyle among healthy subjects may require new and more comprehensive motivational approaches.

Prospective study of suspected associations between certain drugs administered during early pregnancy and congenital malformations.

From 1964 to 1976, a cohort study was performed in West Germany to study the possible influence of various factors on pregnancy and child development. Results of the evaluation of 13,643 pregnancies are given with respect to possible teratogenetic effects of antiemetic drugs and sex hormones administered in early pregnancy. There was no evidence of an increased risk of major malformations following the intake of certain antiemetic drugs and progesterone. Also, the use of a hormonal pregnancy test was not significantly associated with an increase of major malformations.

[Cooperative documentation of childhood malignancies (author's transl)].

Since January 1980 a nationwide documentation of childhood malignancies is performed in the FRG. The cooperative documentation system is supported by the Stiftung Volkswagen-werk and centralized at the Institut für Medizinische Statistik und Dokumentation (IMSD) of the university of Mainz. During the first year of the project the incidence of 1002 malignancies has been reported from a total of 62 hospitals with a still increasing number of cooperating centers. The documentation scheme is based on recommendations of the ADT (working group of tumor centers in the FRG) which are in accordance with international recommendations by the W.H.O., U.I.C.C. etc. A newly diagnosed malignancy is reported by a short standardized form to the IMSD which in turn sends out tumor-specific questionnaires of modular structure for obtaining relevant baseline variables. Patient follow-up is achieved by obtaining the status of all patients at least twice a year which is sent in a condensed format to the IMSD. Besides obtaining basic epidemiological data the system will provide detailed analyses on the course of the different malignancies, allow comparisons between different disease groups as well as different hospitals and will serve as a recruiting basis for controlled clinical trials.

[Is hormonal therapy still justified in imminent abortion? (author's transl)].

In a prospective study, 300 women were treated during 1976 to 1978 alternatingly with hormones or placebo, the comparability of both groups being established in respect of certain influencing factors. No statistically confirmed difference was found in the total success rates of both types of treatment. The age of the pregnant women, and any previous abortions, did not represent any risk factor for imminent abortion. A better success rate was obtained with gestagen therapy, depending on the gestation period. (= 9th week of pregnancy).

[Study of a selection procedure designed for students of schools for medical technicians. (Completory note on the experiences with an enlarged sample using the same statistical methods) (author's transl)].

In a pilot-study aiming at an applicable procedure to select students for schools of medical technicians, 84 students were examined by psychological methods. The problem in question was, whether it is possible to estimate examination-marks on the basis of psychometric variables. The application of different statistical procedures (correlation, mutiple regression) showed, that the most exact estimation was obtained with a multiple regression employing six psychometric variables. Using the same methods on an enlarged sample (N = 180) the following results were received: 1. Taking the same six psychometric variables of the initial study for the multiple regression 63 per cent of the applicants would have qualified if a selection rate of 50 per cent was employed, according to Taylor and Russell. Own computations showed, that about half of the number of students who failed their examinations would not have been accepted. 2. A stepwise-regression-analysis was applied again to select the six "most favorable" variables for the enlarged collective. With the help of a new multiple regression another estimation of examination-marks showed the percentage of the qualified students to rise to a total of 69 (Taylor and Russell). The result of our computations were, that out of 53 students who failed their final examination, 40 would not have been accepted if a selection had taken place. These results are probably too positive. In spite of the relatively large sample and the given number of variables the problem of the vicious circle might occur. 3. Employing this selection of variables the psychological examination will take three hour.
