Osteoporosis in patients with fractured femoral neck in Jerusalem.

In an epidemiological survey on fractures of the femur in Jerusalem, the highest incidence occurred in Oriental males and the next highest in European females. In the same series of patients the frequency of senile osteoporosis was noted to be higher in the Oriental group (both male and female) as compared with that in the European group. Similarly in a randomly chosen sample of the population, osteoporosis in the Oriental sector was twice as high as in the European sector. Osteoporosis was also seen to be three to four times more common in each age group when the fracture series was compared with the randomly chosen sample. No evidence of osteomalacia was noted in these patients.

Transplacental effects of thyrocalcitonin on intestinal calcium-binding protein, alkaline phosphatase activity and ossification of long bones in rat fetuses.

Pregnant rats were treated with 44, 88 and 176 Medical Research Council munits of thyrocalcitonin (TCT) twice daily during days 10 to 21 of gestation. Nonpregnant rats received the same treatment for 12 days. Administration of TCT to the pregnant rats increased the ash and calcium content of fetal bones and decreased the phosphorus content. The diaphyses were short and contained many persisting enchondral trabeculae and a reduced number of osteoclasts. TCT reduced the fetal intestinal alkaline phosphatase activity but elevated the intestinal calcium-binding protein content. In the pregnant and nonpregnant rats, treatment with TCT resulted in hypocalcemia and hypophosphatemia, and increased the calcium-binding protein content of the duodenal mucosa. In the fetuses, the calcium-binding protein content and alkaline phosphatase activity were higher in the jejunum and ileum than in the duodenum, and were much higher than the values found in adult animals. Our findings indicate that TCT passes through the rat placenta and affects the fetal skeleton and calcium metabolism directly, resulting primarily in decreased bone resorption.

Osteoporosis and fractures of the neck of the femur: some epidemiologic considerations.

The prevalence of osteoporosis in Jerusalem during the years 1967-71 was studied in a general population sample and in patients with fractures of the neck of the femur. Osteoporosis was determined by qualitative examination of lateral X-ray films of the lumbar spine. The prevalence of definite osteoporosis in a random population sample was 8.6%, and was higher in females (14.5%) than in males (2.9%). The prevalence of definite osteoporosis among 389 patients with fractures of the neck of the femur was 32.2%--37.8% in females and 17.4% in males. The results of this survey support the conclusion that osteoporosis plays an important role in the pathogenesis of fractures of the neck of the femur.

Value of early ambulation in patients with and without complications after acute myocardial infarction.

A prospective, controlled, randomized study was done to compare the effect of early and late ambulation in hospitalized patients with acute myocardial infarction. All patients surviving longer than the first five days were studied; 64 patients were mobilized on day six and discharged on day 12, and 65 were mobilized on day 13 and discharged on day 19. Follow-up observation lasted from six to 52 weeks. Of patients without complications until day six, eight out of 32 in the early and 16 of 35 in the late groups manifested complications during the follow-up period (p smaller than 0.05). Of those who had complications before day six, seven of 32 and 26 of 30 still had or acquired new complications until last seen (p small than 0.0001). The number of serious complications in the two groups was eight and 24 respectively (p smaller than 0.001). We conclude that early ambulation is beneficial irrespective of complications on admission.
