Enzymatic studies on possible improvement of cytosine arabinoside treatment.

Initial phosphorylation and deamination of cytosine arabinoside (Ara-C) and the natural metabolite deoxycytidine (CdR) were estimated in cell free extracts of leucocytes from patients with CML and controls, Phosphorylation of CdR had been increased while deamination of Cdr in extracts of CML cells from peripheral blood had been decreased compared with normal leucocytes. Comparing the ratios between Ara-C and CdR phosphorylation it was revealed that these were twice as high in CML cells as in normal leucocytes, whereas no difference was found when comparing ratios between Ara-C and CdR deamination. From these discoveries it is proposed that Ara-C can be combined with CdR with advantage, because apparently CdR protects the normal cells more than the malignant ones.

Common variable immunodeficiency and purine nucleotidase and nucleoside phosphorylase deficiency. A case report.

The results of immunological and purine enzyme investigations in an adult male patient with common variable immunodeficiency, recurrent lymph node granuloma and splenomegaly are presented. Serum immunoglobulins were present in trace amounts only and a progressive loss of Ig-bearing peripheral lymphocytes were demonstrated. Furthermore, the mitogenic responses to PHA. ConA and PWM were markedly reduced and the ratio of T.m/T.g cells was decreased. Finally, a combined deficiency of lymphocyte purine 5-nucleoside phosphorylase was demonstrated in the patient.

Erythrocyte and leucocyte enzymes in a case of paroxysmal nocturnal haemoglobinuria.

In a patient with paroxysmal nocturnal haemoglobinuria (PNH) enzymatic activities of erythrocytes and leucocytes were studied. Studies of autohaemolysis were also performed. The following erythrocytary enzymes were measured: Glucose-6-phosphate dehydrogenase (G-6-PD), pyruvate kinase (PK), glutathione reductase (GR), and acetylcholinesterase (AcChE). The following enzymes were measured in leucocytes: Adenosine deaminase, purine nucleoside phosphorylase, adenine phosphoribosyltransferase, hypoxanthine phosphoribosyltransferase and adenosine kinase. Normal activity of G-6-PD, GR and PK in erythrocytes was found. In leucocytes and lymphocytes activity of purine nucleoside phosphorylase was reduced. Auto-haemolysis in vitro was increased, which could not be compensated by addition of glucose or ATP.

Mepacrine hydrochloride in the treatment of malignant pleural effusion. A controlled randomized trial.

The effect of local instillation of mepacrine hydrochloride (quinacrine hydrochloride U.S.P., Atabrine) was compared with triethylenethiophosphoramide (Thiotepa) applied locally and with pleurocentesis alone. Twentyfive patients with disseminated malignant disease were allocated at random to one of these three treatments. A finding of more than 500 ml effusion within 3 months of instituting treatment was regarded as a failure, and subsequently one of the other treatments was used at random. Sixty-four per cent of the patients treated with mepacrine responded, compared with 27% treated with triethylenethiophosphoramide, and 11% treated with pleurocentesis alone.
